1. Automatic Meter Reading (AMR) service provides for the remote reading of metering equipment located on a subscriber's premises by a Utility company or its designated agent (Utility), providing gas, water, or electric services.

2. AMR is classified as a competitive local exchange and interexchange telecommunications

service. AMR consists of central office facilities which provide a transmission path between the Utility's computer terminal and the meter on the Utility customer's premises via the local exchange network.

3. AMR provides the Utility a signaling path to a subscriber's Exchange line without ringing or

interfering with the subscriber's use of the line. 4. The AMR central office common equipment consists of a Controller, which directs the AMR

intraoffice connecting facility to provide access to the subscriber's Exchange line; and an intraoffice connecting facility which allows use of the subscriber's line for the transport of meter reading data between the Utility data collection device and the AMR central office common equipment. AMR is accessed by the Utility via non-dedicated telecommunications equipment using a Utility-specific code. Because the central office equipment is shared, immediate access is not guaranteed.

5. A Meter Interface Unit (MIU) provided by the Utility in accordance with PART 68 of F.C.C. Rules

and Regulations and compatible with the Company provided central office Controller and facilities as outlined in Part 2, Section 9, must be located on the subscriber's premises. The MIU connects the subscriber's Exchange line to a utility meter.

6. Meter Reading Terminal Equipment (MRTE) shall be defined as the MIU as described above, and

all associated facilities, wiring and equipment installed and maintained by the Utility on the customer's premises up to the NETPOP.

AUTOMATIC METER READING (cont’d) B. Regulations

1. AMR is offered subject to availability of central office facilities and equipment. 2. AMR is provided in connection with Exchange service. Utility customers must have Exchange

service in order to participate in AMR. 3. AMR central office common equipment is required in each central office where AMR service has

been requested. 4. The liability of the Company for providing the service herein shall be as set forth in Part 2, Section

2. 5. The Company shall have no liability to the Utility for service line outages related to their

customers' Exchange line. 6. It is the sole responsibility of the Utility to obtain subscribers' telephone numbers and the consent

of the subscribers for the use of their Exchange lines. Furthermore, it shall be the responsibility of the Utility to install and maintain any required terminal equipment on their premises and on their customers' premises.

7. Nothing contained in this guidebook shall be construed as establishing any agency agreement, or

partnership between the Company and the Utility. The Utility shall be responsible for obtaining all applicable licenses, permits and authorizations and to comply with all codes, laws, regulations, restrictions, or limitations governing the use of equipment or services employed by it in providing AMR.

8. As specified in Part 3, Section 1, the Utility shall be responsible for repair charges incurred for

each repair visit by the Company to subscriber's premises in connection with a service malfunction when it is determined that the malfunction is caused by or associated with the MRTE. The Utility shall maintain labels and markings on the MIU and, where practicable, on the remaining portion of the MRTE. Labeling must include the Utility's billing name, billing telephone number and contact telephone number.

9. Calls completed between the Utility and AMR central office common equipment via the Utility's Exchange line will be charged for at the rates specified in Part 4, Section 2. Calls between Market Service Areas (MSAs) will be transported by the Utility's designated interexchange carrier.

10. The Utility will be liable for all non-recoverable charges associated with service establishment in

the event there is a Utility initiated cancellation prior to installation. 11. Technological changes, made at the request of the Utility, may be provided at the option of the

Company. 12. The minimum initial service period for AMR central office common equipment is 36 months,

excluding service provided under the Limited Service Provision specified in D. following.

AUTOMATIC METER READING (cont’d) C. Term Payment Plan

AMR is offered under a Term Payment Plan (TPP) option that allows a Utility to pay a fixed rate for AMR service over a 36, 60 or 84 month payment period. During the effective term, monthly rates for services installed under this arrangement will not be subject to Company initiated rate changes. At the expiration of the TPP, the Utility may elect a new TPP at the then prevailing rates, continue the service on a month-to-month basis at the then prevailing rates, or terminate the service. At the expiration of the TPP, the Utility will be billed on a month-to-month basis at the then prevailing rates unless the Utility elects another option. At the option of the Utility, payment for all the remaining monthly charges associated with the AMR Central Office Controller may be made as a lump sum payment, as described in Part 2. The prepayment of the monthly charges in no way constitutes a purchase of the AMR central office Controller. The Company retains full ownership of all equipment covered. In the event that the service is terminated prior to completion of term, all remaining charges shall become immediately due and owed. A Utility may prepay all of the total outstanding charges during the selected TPP.

D. Limited Service Provision

The Limited Service Provision option allows a Utility the opportunity to test and integrate AMR equipment, methods and procedures. 1. Utilities not currently subscribing to AMR may participate in an optional Limited Service Provision. 2. A Limited Service Provision arrangement is restricted to one central office location per Utility. 3. An AMR Limited Service Provision is provided subject to the availability of Company facilities and

at the option of the Company. 4. The service period is 12 months for customers that elect the Limited Service Provision. 5. Upon conclusion of the Limited Service Provision period, the Utility will either terminate the

service or select a TPP. The Utility will notify the Company, in writing, of its intention 30 days prior to the expiration date of the Limited Service Provision Period or be terminated at the option of the Company.

6. A Per Month rate, or at the option of the Utility, a prepayment rate, as well as a non-recurring

charge will apply for the Limited Service Provision. Non-recurring charges associated with the installation of service shall be paid in full when service is established.

AUTOMATIC METER READING (cont’d) E. Rates and Charges

The following rates and charges are in addition to the rates and charges for any other services associated with AMR. 1. Meter Read Charges

AMR usage charges are applicable when a Utility successfully completes a meter read from a customer's premises. Two rate periods will apply: Rate Periods Charge Per Read Peak 8:00 a.m. - 10:59 p.m. $0.12 Off Peak 11:00 p.m. - 7:59 a.m. 0.07 Notes: - Time of day provisions apply to each day of the week. - A successful read will be determined by a billing record registered for each contact. - In addition, the Utility will be billed according to current voice usage rates. If applicable,

appropriate 800 usage service charges from the central office Controller to the Utility will also apply.

2. Limited Service Provision

AMERITECH SERVICE MANAGEMENT SYSTEM A. General

1. Ameritech Service Management System (ASMS), classified as neither local exchange nor interexchange telecommunications service, provides the customer with the capability for additional control and management of Centrex Service, Centrex Switching Service (CSS), Integrated Information Network (IIN), Ameritech Integrated Digital Network and other Company provided telecommunications services. ASMS provides a single point of mechanized entry into the Company for the following services: - Trouble Reporting - Measurement Reporting - Testing Services - Performance Monitoring - Customer Control

2. ASMS services will only be provided from central offices equipped for the services subject to technical limitations and availability of such equipment and facilities.

3. The customer accesses ASMS by means of a customer provided terminal or work station

equipped with a communications package over either a customer provided private line or the public network.

4. All customer provided equipment used to interface with ASMS is required to conform with the

Technical Reference Specifications as used by the Company and found in Technical Reference AM-TR-MKT-000026. The technical reference documents are available from: APEx Support Team (734) 523-7348

5. ASMS services may be subscribed to individually or in combination with other ASMS services under conditions stated elsewhere in this guidebook.

6. ASMS customers are permitted to access only their own customer information.

7. ASMS customers may continue to use established, non-mechanized procedures for obtaining

information involving their Company provided services.

8. The Company reserves the right to temporarily restrict the customer from accessing any or all ASMS services when service-affecting conditions exist or during scheduled maintenance downtime as determined by the Company.

9. Initial training in system operation for up to ten customer personnel in no more than two sessions is provided at the time of system cutover at an agreed upon location.

Ameritech Service Management System (ASMS) will not be provided to new customers on or after June 1, 1992. Customers having this service may continue such service from their present location(s). In the event service at any location is terminated for the customer, such service will not be reestablished.

ASMS System Feature capabilities are provided to all ASMS subscribers. Viewing Feature This feature permits the customer on-line access from customer terminals to view ASMS administration tables (including customer specific information) and any reports processed by ASMS. Printing Feature This feature enables a customer to send selected ASMS reports to a customer designated printer for hardcopy output. Reports which can be printed include: - Measurement Reports (Scheduled) - Performance Monitoring Alarm and Alert Data

Notes Feature This feature, available from the main menu or sub menus, provides a simple, direct means for the customer or the Company to initiate and/or respond to service inquiries. This feature allows the user to: - Review notes sent by the Company - Save/delete notes sent by the Company Customer Information (Circuit ID Table) Access This optional feature is accessible from either the Trouble Reporting menu, Testing menu or Customer Information menu, and provides the customer on-line access to complete up-to-date listings of all Company provided lines and trunks loaded into ASMS which serve the customer. Number ID tables available to the customer are: - Telephone Numbers - Trunk Groups Due to technical limitations, on-line Customer Information Access may not be generally available to all customers.

1. Trouble Reporting

a. General

Basic Trouble Reporting Basic Trouble Reporting allows the customer to report troubles associated with non-engineered circuits (on a per circuit basis) to the Company. The customer may also add information to existing trouble reports.

b. Service Features - Trouble reporting on Company provided services - Confirmation of trouble reports received by the Company - Additions and changes to trouble reports

2. Measurement Reporting

a. General

1. Measurement Reporting provides for scheduled reports on traffic on Company provided

services. Scheduled summary reports are available on a 24 hour basis. Traffic reports may provide peg count, usage and overflow outputs. Reports and usage may be viewed on a customer terminal and/or printed at customer designated locations.

2. Traffic reports may be provided in standard or non-standard formats. Standard traffic

reports are described in b. following. Non-standard traffic reports may also be available to meet specific customer requests.

- Centrex Universal Call Distribution Traffic Report: traffic measurements by

Centrex/IIN/Ameritech Integrated Digital Network group including, but not limited to, offered, answered and abandoned counts.

- Trunk Group Traffic Report: traffic measurements, where available, by trunk group

including, but not limited to, peg count, usage and overflow counts. - Console Traffic Report: traffic measurements by Centrex/IIN/Ameritech Integrated

Digital Network console group including, but not limited to, usage, busy and overflow counts.

- System Traffic Report: traffic measurements by Centrex/IIN/Ameritech Integrated

Digital Network system including, but not limited to, total originating, direct in and out dial and intragroup counts.

2. Formats:

- Scheduled reports of 24 hours of traffic data 3. Reports may be viewed and/or printed at customer locations on customer provided

equipment.

3. Testing Services

a. General Testing Services allow the customer to request tests on Company provided circuits equipped for testing capability. Test results are returned automatically for customer viewing.

b. Service Features

- Allow the customer to request testing on non-engineered circuits - Provide capability to view a summary of test results on non-engineered circuits - Provide capability to view detailed test results on non-engineered circuits - Allow test requests to be viewed in a status list summary

4. Performance Monitoring

a. General

The customer may obtain scheduled "Line/Trunk Out of Service" reports on Company provided services. Performance Monitoring may also provide the customer with capability to receive alarm and alert data generated by the serving central office which may affect Company services subscribed to by the customer.

b. Service Features 1. Switch Performance Reports

Capability to view scheduled "Line Out of Service" and/or "Trunk Out of Service" reports for lines and trunks providing Company services to the customer

2. Switch Performance Monitoring Alarm and alert data concerning Company provided services delivered in near real time to customer designated printers and/or computer systems

D. Regulations

1. Customer Responsibility The customer will be responsible for obtaining the terminal hardware, communications software, and network connections required for access to ASMS. Each customer will be provided with a copy of the user guide for ASMS.

2. Minimum Contract Period (MCP)

a. ASMS is offered under 12, 36 or 60-month MCPs. This allows a customer to pay a fixed rate

for ASMS services over the payment period selected. During the effective term, monthly rates for services installed under this arrangement will not be subject to Company-initiated rate changes.

b. Minimum Contract Period Extensions 1. Once during the term of a 12-month, 36-month or 60-month MCP, a customer may

extend the MCP subject to the following conditions. The term remaining on the original MCP plus the term of the extension ("term of the new MCP") may not be less than 12 months and may not exceed 60 months.

2. Minimum Contract Period (MCP)

b. Minimum Contract Period Extensions (cont’d)

2. The monthly rates for the existing MCP will continue during any MCP extension, unless a

lower rate would apply to the term of the new MCP based on the then current guidebook rates for a 12-month, 36-month or 60-month MCP. If the term of the new MCP is up to 35 months, the 12-month MCP rates would apply; if the term of the new MCP is from 36 through 59 months, the 36-month MCP rates would apply; if the term of the new MCP is 60 months, the 60-month MCP rates would apply. The lower rates, if applicable, will become effective on the date the extension is signed. Such rates will not be retroactive.

3. Regulations applicable to the 12-month, 36-month and 60-month MCPs apply to any extensions.

c. Month-to-Month Continuation

At the conclusion of any 12-month, 36-month or 60-month MCP, the customer may also elect to continue ASMS service on a month-to-month basis. Month-to-month rates are available only at the conclusion of a 12-month, 36-month or 60-month MCP. ASMS regulations other than those which pertain to MCP terms shall apply to any month-to-month continuation of service.

d. Rates and charges are due and payable on a per customer basis.

3. Termination Liability The customer requesting termination of any or all ASMS services prior to the expiration of the MCP will be liable for payment of all applicable Service Subscription Charges and I.N.C. installments under the deferred payment plan for the remainder of the term as a lump sum at the time of termination, adjusted for the time value of money.

4. Suspension of Service Suspension of Service is not offered with ASMS.

5. Responsibility of the Company ASMS processes commands in the sequence received. Consequently, service access delays may be experienced by the customer during peak load hours. Immediate access is not guaranteed. Moreover, the completeness and accuracy of the information provided by ASMS is not guaranteed.

6. Security

ASMS employs a multi-level security system to promote customer network privacy, but privacy is not guaranteed. To access ASMS, a customer must utilize log-in and user ID codes. It is the customer's responsibility to protect the log-in and user ID codes. The customer shall indemnify and hold the Company harmless from any and all liability or claim of loss or damage directly or indirectly arising out of or caused by, in whole or substantial part, the customer's use or misuse of, or failure to protect, the log-in and/or user ID. The Company reserves the right to temporarily restrict users from accessing ASMS if unauthorized usage is suspected. This reservation, however, does not diminish the customer's responsibilities set forth in this paragraph.

7. ASMS Availability ASMS System Features (front end functions), described in B. preceding, are designed to be available 24 hours per day, seven days per week. ASMS Services, described in C. preceding, will be generated by existing host systems which have historically been designed with varying availability profiles. Subject to the limitations of host system architecture, ASMS Services will be made available to the customer to the maximum extent practicable. The availability of both ASMS System Features and ASMS Services is subject to service affecting conditions and Company scheduled hours of maintenance downtime.

8. Allowance for Interruptions

a. When ASMS service is interrupted due to a failure or malfunction in any of its components, subscribed services may be unavailable and a prorated adjustment of the monthly subscription charges will be allowed in accordance with the regulations specified in this guidebook. An interruption period starts when an inoperative service is reported to the Company and ends when the service is operative. The credit adjustment is based on b. and c. following.

b. No credit shall be allowed for an interruption of less than 24 hours. The customer shall be

credited for an interruption of 24 hours or more at a prorated amount of the monthly charges for the unavailable service/feature for each 24 hour period, or major fraction thereof, that the interruption continues.

c. No allowances will be granted for interruptions required to perform preventive or routine

maintenance, or when service is temporarily restricted due to service affecting conditions. Scheduled maintenance downtimes will be determined by the Company. No allowances will be granted to perform software or hardware updates when the customer is notified at least 24 hours prior to such occurrences.

9. Liability of the Company

The liability of the Company is described in Part 2, Section 2.

10. Definition of a Business Day Business day hours are defined as Monday through Friday, 7:00 a.m. to 6:00 p.m. All day Christmas Day (December 25), New Year’s Day (January 1), Independence Day (July 4), Labor Day, and Thanksgiving Day, or the resultant legal holiday, will be considered non-business day hours.

E. Rates and Charges

1. Method of determining usage charges:

a. Connect Time charges are determined using Steps 1. through 3. following:

1. Obtain total minutes accumulated over the billing period for each rating period and round up to the nearest minute

2. Divide the total number of minutes by 60 and round to two decimal places (chargeable hours).

3. Multiply the chargeable hours determined in Step 2. by the hourly rate.

b. When ASMS chargeable usage time begins in one rate period and ends in another, the usage charge will be compiled in two parts.

LOBBY INTERPHONE SERVICE FOR MULTIPLE APARTMENT BUILDINGS Push Button Type Service A. Regulations

1. This service provides for the origination of telephone calls from a building lobby to apartment telephones through the use of lobby telephones equipped with push-buttons. The lobby interphone system can process only one call at a time.

2. Lobby interphone service can be associated with only one exchange line in a tenant's apartment.

Each apartment telephone connected to the service is arranged to pick up and hold one central office line and one lobby interphone line and is furnished with a lobby door unlocking feature. The service provides a distinctive ring for each type of incoming call, central office or lobby interphone, when the tenant's line is not in use. A tone signal indicates to the tenant when a lobby interphone call is waiting if his line is in use on an exchange call or vice versa.

3. Where tenants do not have residence service or have residence service, but prefer that their

lobby door answering service not be associated with their residence service, lobby door answering service to a tenant may be provided through the use of a Type 2001 Channel, as specified in Part 15, Section 2.

4. One lobby interphone system with a maximum of three digits and without apartment number

correspondence may serve several buildings when: - all buildings are served by the central office where the common equipment is located. - the lobby telephone provides communication only to apartments served by that lobby.

5. The building is responsible for all Lobby Interphone Service furnished except the auxiliary

HOTEL SERVICE/1/ A. General

1. Hotel Service is a P.B.X. type service furnished to hotels (including marinas and motels), apartment houses, clubs having sleeping rooms for members and guests, and hospitals. The service is for the use of the customer's guests, tenants, patrons, members or patients and for use of the customer in the administration of his business.

2. Hotel Service is furnished on a Hotel Trunk basis. In lieu of Hotel Trunks, customers may

subscribe to any other type of business P.B.X. trunk service offered in their exchange, as shown in Part 4, Section 2.

3. Stations of the hotel system located in rooms of guests, tenants, patrons, members, patients and

operating quarters of the customer may be used for originating and receiving calls over the telecommunications network.

4. Stations of the hotel system located in stores, shops and other businesses in the same premises

may be used only for intercommunication with other stations of the system. Such stores, shops and other businesses must subscribe to their own telephone exchange service for originating and receiving calls over the telecommunications network. Stations of the hotel system are not subject to mileage charges.

5. Types 2001A and 2001B channels to the premises of other customers outside the hotel

(apartment house, club or hospital) may be furnished for use of the hotel customer in the administration of the customer's business.

B. Rates

1. Network access line charges for P.B.X. trunks as described in Part 4, Section 2, are applicable.

2. Each customer dialed call within the flat rate calling area for Local Area Service, as specified in Part 4, Section 2.

/1/ Hotel Service may continue to be provided to present Hotel Service customers in MSAs 3 and 6

served by non-conforming offices, until such time as their serving central office has been upgraded. As facilities become available these customers will be converted to the standard rate structure and Hotel Service will be discontinued. ENSEMBLE

Effective September 1, 2011, no further installations, Term Payment Plan renewals, moves, rearrangements, or changes of any type may be made to Ensemble Service. Customers of record on September 1, 2011 may continue their service as long as such service remains at the location at which it was being furnished on the aforementioned date. In the event that these services are discontinued at their present location for any reason, they will not be reestablished. The Company will not impose termination liability to those customers discontinuing service. The preceding supersedes all of the rules and regulations that follow. A. Description

ENSEMBLE service will provide customers access to end users through an integrated hub in specific LATAs. ENSEMBLE is comprised of specific directory numbers and the Switched Redirect Network; a dedicated network of data optimized high usage one-way trunk groups to a centralized dedicated hub central office. ENSEMBLE customers can offer their end users, specific local directory numbers, which are routed over the Switched Redirect Network (SRN), to the centralized dedicated hub. The ENSEMBLE customers will then receive their end users hubbed traffic via existing ISDN Prime Service.

B. Definitions Query Access A specific directory number (DN) which in concert with the SRN recognizes calls placed to the DN and routes them over the SRN. Switched Redirect Network (SRN) An overlay network of data optimized high usage one-way trunk groups utilizing Intelligent Network (AIN) capabilities which link end offices in a selected LATA to a centralized dedicated hub central office.

C. Terms and Conditions

In addition to regulations set forth elsewhere in this guidebook, the following terms and conditions apply to ENSEMBLE:

1. Service is available in the following selected LATAs:

2. Prime ISDN Service is necessary to retrieve the hubbed traffic.

3. Payment Plans

Term Payment Plans ENSEMBLE service is available under the Term Payment Plan (TPP) whereby customers may elect to pay a fixed price for a 36 or 60-month period. After the selected TPP is satisfied, the monthly extension price will apply unless a new TPP is selected. Month-to-Month Month-to-month rates are available for Query Access to Integrated Hub, Additional. Single Payment Option (SPO) The monthly recurring price may be prepaid for periods that coincide with any term payment plan. The prepayment charges in no way constitute a purchase and the Company retains ownership of all equipment covered by prepayment. The following conditions apply:

- Customers who prepay will have an allowance applied. The allowance will be based on the number of months of the prepayment period. The prepayment amount (principal) is the total outstanding recurring charges less the payment allowance. The prepayment will be adjusted for the time value of money. Recurring charges will cease for the rest of the term and start up again at the end of the contract period if services have not been disconnected. If prepayment has been elected and the service is discontinued prior to the end of the contract period, a credit for the unused portion of the contract period, adjusted for the time value of money, will be given to the customer.

- Customers who prematurely disconnect the service will have termination charges deducted from the prepayment amount and any balances credited to their bill.

4. Termination Charges

Termination liability will apply to ENSEMBLE service disconnected prior to the expiration of the contracted Term Payment Plan period. The charge will be calculated from the remaining fixed monthly recurring charges or any minimum billing guarantees that would have been incurred by the customer if the service had been in service for the entire contract period.

5. Credit Allowance Credit allowances will be granted when service is interrupted due to Company failure. The credit allowance will be based upon the ratio of the duration of the service interruption measured from the time the interruption is reported to or detected by the Company, whichever occurs first, and expressed in multiples of 30 minutes to the total time in a 30-day month. That ratio, multiplied by the monthly charge for the service affected, shall determine the amount of the credit allowance. No other liability shall attach to the Company in consideration of such interruption to service.

6. Changes

With the written permission of the Company, consistent with other regulations of this guidebook, the obligation to pay the TPP charges may be assumed by another customer, if the service has not been terminated, and if the other customer intends to continue using the service at the present location and actually continues such use. Such assumption of service does not relieve or discharge the original customer from remaining jointly or severally liable with the transferee for any and all obligations existing at the time of the transfer. During a customer’s TPP term, conversion may be made to a new TPP period of the same or different length, if the expiration date for the new service or TPP period is the same as or greater than the original TPP period. The new TPP becomes effective upon execution. In all situations described in this paragraph 6. the customer incurs no liability for the remaining months of the original TPP, since the change is not considered a termination of service. Changes to ENSEMBLE service will create a subsequent change charge as reflected in the ISDN Prime Service offering.

EMERGENCY REPORTING SERVICES FOR MUNICIPALITIES A. General

1. In the interest of public safety, the Company will make available to municipalities telephone channels and facilities for a public reporting system which will permit direct conversation between telephones on city streets or in other locations accessible to the public and telephones generally located at fire and police headquarters for the purpose of transmitting reports of fires and requests for police assistance.

2. The Company will furnish all lines and equipment required for such a reporting system, consisting

generally of reporting telephones at locations designated by the municipality connected by lines to switchboard or current recording positions located in the municipal, fire or police headquarters. The municipality will furnish suitable mounting space for the reporting telephones and provide any conduit required between such telephones and the point of connection with the facilities of the Company.

3. Other reporting arrangements may be furnished to municipalities by means of equipment located

in the central office through which a telephone at an emergency reporting location may make announcements to several exchange lines simultaneously.

Type I permits a special control station located at an emergency reporting location to signal a particular group of individual line service telephones and give an announcement to the person who answer on these lines. Type II permits firemen upon hearing an alarm in a dial exchange to call a telephone at an emergency reporting location and be connected simultaneously to receive an announcement. See "Note" under Rates for this type. Type III permits up to 13 incoming central office lines to be simultaneously connected to one or more (maximum 10) emergency reporting stations. The number of reporting stations determines the number of possible exchange line connections. An alarm circuit is provided in the central office to indicate a “receiver off hook” at any one of the reporting stations. This type also permits the operation of up to four remotely located siren controls.

4. All operating on the customer’s premises will be performed by the customer. Suitable commercial power, including, outlets, which may be required for the operation of the service shall be furnished by the customer.

HIGH VOLTAGE PROTECTION SERVICE A. Description

High Voltage Protection Service is Special Protection Against Hazardous Voltages. Company services provided at or near electric power generating switching and distributing locations may require special protection equipment or services to isolate or neutralize the hazardous voltages that could appear due to GPR and/or induction resulting from faults in the electric power system. The protection objectives for Company services at these locations are: 1. To minimize electrical hazards to personnel engaged in construction, operation, maintenance or

use of the telecommunications systems.

2. To limit electrical damage to communications equipment and cable facilities in the event of a fault in the electric power system.

3. To provide the required reliability and integrity of telecommunications transmission as specified by the customer.

B. Terms and Conditions

In addition to the regulations set forth elsewhere in the Guidebook, the following regulations apply to High Voltage Protection Service:

1. Special protection equipment may be necessary at the customer's premises, at the Company Central Office and/or on the right-of-way at remote locations.

2. Protective requirements will be applied equally to exchange and private line services and the level of protection will be determined by the circuit requiring the highest level of protection.

3. Minimum protection standards as specified by the Company will be established in accordance with the objectives stated above. The customer may select a higher degree of protection depending on the need for minimum service interruptions.

4. The customer shall be responsible for providing to the Company the following:

- The probable magnitude of the GPR (rms) at each such location where Company-provided telecommunications services are requested.

- A forecast of the type and quantity of each telecommunications service required at a given

location for a period mutually agreed upon by the Company and the customer. - Other technical data required by the Company to determine the method and amount of

protection required at each location. - Revisions in requirements of the customer must be provided in writing to the Company with

sufficient lead time to permit incorporating such changes into the design of the protection system.

5. All special protection equipment must be provided entirely by the Company. Special high voltage

protection equipment furnished by an electric public utility ("Power Utility") prior to October 8, 1984 may continue to be utilized at its present location on the power utility's premises.

6. Equipment cabinets or mounting arrangements may be provided by the customer or by the

Company at additional cost. 7. All costs incurred due to rearrangement of circuits or equipment associated with special

protection when requested by the customer, will be billed to the customer. 8. Except when specifically provided otherwise, special protection services are also subject to all

charges and regulations as provided in another Part or Section of this guidebook. 9. Complete or partial temporary suspension of service provisions as described in Part 2, Section 4

do not apply to special high voltage protection equipment. 10. High Voltage protection equipment and/or associated transmission facilities as may be required

for protection or at the request of a customer and not provided specifically in this guidebook, may be provided at rates and charges determined for each location dependent on the cost of the protection facilities provided.

11. A customer may request to have high voltage protection equipment moved within the same

premises or to different premises within the area covered by this guidebook. The charges for such a move will be as provided in Part 3, Section 1. Applicable monthly rates will continue for the duration of the move.

AMERITECH NETWORK SWITCH ALTERNATE ROUTING A. Description

Network Switch Alternate Routing (NSAR) is an Advanced Intelligent Network (AIN) based service that allows the customer to safeguard against the loss of incoming calls resulting from the complete failure of the customer’s serving central office switch. NSAR allows the customer to develop and maintain a contingency plan that can be activated at their command to reroute incoming calls around their failed central office switch and back to their location or an alternate location. NSAR allows the customer the ability to develop and test solutions to potential problems before they occur. NSAR supports up to ten alternate routing plans with a maximum of 10,000 telephone numbers. NSAR operates across the public network or private facilities. The customer may activate NSAR 24 hours a day, seven days a week in coordination with the Company. Upon local central office recovery, conditions permitting, the Company will deactivate network controls in coordination with the customer. A NSAR plan is subject to review by the Company in order to determine effects on network capability, capacity and control. All telecommunications service required for rerouting must be in place or subscribed to at the same time as NSAR and will be reviewed by the Company. NSAR allows the customer to exercise their back-up facilities during normal conditions.

1. NSAR service is available where central office facilities and equipment permit. 2. The NSAR customer is responsible for the payment of any applicable station-to-station guidebook

charges for each call between the central office where the NSAR protected numbers reside and the telephone to which the call is being rerouted.

3. Upon request of the customer, the Company will coordinate an out-of-hours test of the customer’s

alternate routing plan which will be subject to a charge as found in the Prices section following. 4. NSAR is available to PBX, Centrex, ISDN and basic exchange business line customers where

facilities permit. 5. Due to network capacity and capability limitations during a long central office outage, some

blocking of calls in the network may occur, and therefore, all incoming calls may not be rerouted and terminated to the alternate site. In all cases, the first priority will be to stabilize the public network.

6. Customer requested changes to an existing NSAR plan are subject to a charge as found in the

Prices section following. 7. Because of the unavoidableness of errors incident to the services and to the use of the

Company’s facilities, the services and facilities furnished by the Company are subject to the terms, conditions and limitations specified herein. With respect to a claim or suit by a customer, or by any others, for damages associated with the installation, provision, termination, maintenance, repair or restoration of service, the Company’s liability, if any, shall not exceed an amount equal to the proportionate charge for the service for the period during which the service was affected.

C. Features

Customer Access NSAR provides two customer interface options: Internet Access and Touch Tone Access. NSAR with Internet Access provides direct access to the Service Management System. Under this method, the customer’s service administrator may define an entire contingency plan, change the “forward-to” destination for protected telephone numbers, activate or deactivate a contingency plan, modify an existing plan and retrieve information about existing plans. NSAR with Touch Tone Access provides the customer’s service administrator the ability to activate or deactivate a contingency plan.

NAME AND NUMBER DELIVERY A. Description

Name and Number Delivery is a pay per use feature which is activated when a customer makes an outgoing call and a no answer condition occurs. The calling party will be asked by a recorded announcement if they would like to record their name and number for future delivery to the called party. The service will then prompt the caller to say their name and number. Name and Number Delivery will record and store the name and number and will attempt to deliver it on a preset schedule to the called party.

B. Terms and Conditions Name and Number Delivery will be available to residence and business customers only from equipped central offices where facilities are available. Name and Number Delivery will not be available to Centrex or PBX customers. This feature will be offered only on calls which originate from and terminate within Chicago. At the request of the customer, access to Name and Number Delivery may be blocked. The receipt of Name and Number Delivery recordings may also be blocked. Either or both types of blocking will be provided at no charge to customers on a per line basis.
