How to start using COOL today from your iPhone or iPad 1. Open Safari as your web browser. Put in the web address www.cool.net. in the

web browser address line.

2. The COOL “Activate/Log-on” page will appear. 3. Enter your new COOL access code and tap on the “Activate/Log-on” button. 4. You’ll be directed to the COOL License page. Tap on “Accept” on the license

page. 5. The COOL Launch Pad will appear on the screen.

Adding the COOL App icon to your iPhone or iPad

1. At the top left of the iPad and bottom middle of the screen of the iPhone, tap on the box with an arrow (see screen shots below)

2. With the box open you will see function categories from which to choose. Click on the box that says, “Add to Home Screen.”

3. After you choose “Add to Home Screen”, a box will open showing you the COOL

app icon has been added to the screen of your ipad and or iPhone. 4. Complete the setup by simply clicking the “Add” button in the upper right hand

corner of the open box (see below). You now have the COOL App icon on your mobile device.

Add a COOL launch App to other OS Mobile Devices

1. Open the web browser and go to www.cool.net. 2. When the COOL page opens, enter your COOL Access Code into the box and

login 3. Once you are at the COOL App Launch Pad page, open the browser

“Bookmarks” 4. Bookmark this page and follow the mobile device system instructions to save to

your mobile home screen Depending on the mobile device, its settings, functions and user changes to the device, you may have to periodically re-enter your COOL access code. Please note: You can access everything in COOL with your smartphone. However, due to the fact that these phones have small screens they are not optimal devices for accessing certain online resources. Depending upon the specific smartphone, the full user experience and power of the COOL system may be less than optimal since some of the COOL interactive tools may be difficult to fully navigate on a smartphone. COOL is best utilized and offers the maximum user experience with a computer or tablet due to a larger screen size. The COOL Lpad From the COOL Lpad, with simply a click or a tap, you have access to all the unique information available on COOL. From the Lpad you can go directly to some of COOL’s most popular information and resources by simply clicking/tapping on the App icon of your choosing. In the upper left hand corner, you will see a gear icon. Tapping or clicking on it allows you to edit your personal information such as an e-mail address or phone number. You can also view the Apps you have installed.

Customize Your COOL Lpad There are several ways to customize your Lpad for the way you use COOL. You can:

• Change the order of the App icons (within their categories) by simply click/tapping and dragging the App icon to its preferred position

• Add or remove Apps from your User Selected Apps section at the bottom of the Lpad page by clicking/tapping on the COOL App Store link in the left hand column of your Lpad page. Remove or add Apps by clicking/tapping and dragging the App icons between your User Selected Apps section and the COOL App Store.

• Learn more about each App on the COOL Lpad by hovering your curser over a specific App. A window opens to your left with a more detailed description. You may turn the App description window off by checking the OFF radio button at the top right of the Lpad.

• From any touch screen device, to see the App descriptions, click on the App

Navigating within COOL and Site Organization Upon clicking/tapping on an App Icon, you’ll be taken directly to that specific area of COOL. You can then navigate within COOL by clicking on one of the blue tabs across the top of the page or by clicking/tapping on a related topic in the shaded side bar on the right side of some pages. You can return to the COOL Lpad by clicking on the multicolored tab at the far right side of the tabs row near the top of the page. You will find links appear at the very top of each COOL page where you can go to:

• The COOL Lpad (Main Menu) • Information about the organization that brings you COOL (Who We Are) • A glossary offering definitions of special terms used in the site (Glossary) • A form to contact a COOL representative by e-mail (Contact Us).

COOL is composed of interactive tools and a database of over one million records. The site is organized around Create-a-Care-Guide, an interactive tool that helps you create a written care plan similar to those created by professional care managers. A Care Guide is the written product of an evaluation of the needs of a person based on his or her physical ability, cognitive functioning and medical needs. However, if you do not need to create a care guide, you can simply bypass the Create-a-Care-Guide tool and access only the specific information and features you need. Please Note: To be able to save information, resources and cost calculations for later use or to print a report, you will need to go into the Create-a-Care-Guide section to register and create a file cabinet (see instructions under Site Registration below).

COOL is intuitively organized into several sections, including: • What is a Care Guide? - Learn what a Care Guide is and what the key

considerations are in developing one. • Create-a-Care-Guide - This section introduces the online tool for creating a

written plan to care for someone who is ill, disabled or elderly. • Assessment Tools - In this section you will find interactive tools to help you

assess physical or mental impairment and determine what services are needed to cope with short- or long-term periods of disability.

• Search for Resources - COOL maintains a database of more than one million records. In this section you can search for the following services by any location you choose:

o Physicians o Hospitals and clinics o Professional Care Managers o Certified Elder Care Attorneys o Facilities, such as assisted living and nursing facilities, that care

for disabled or elderly individuals o Home health care agencies and homemaker/chore services o Adult day care and senior centers o Medical equipment and home security alert providers o Governmental agencies that provide advocacy, education and

support for seniors and people with disabilities, by location o Pet boarding and sitting o House sitting and property care o Relocation assistance o New mother assistance o Travel assistance o Homeland security

• Cost Calculators - COOL includes cost calculators to help you: o Predict the cost of care in your geographic area by service type o Compute your loss of wealth if you are forced to reduce your work

unable to work • Medical Resource Center – COOL provides you with a comprehensive

medical resource center that allows you to research an extensive number of medical topics. Spanish translations and illustrations are available for some topics. Categories of topics include:

• Advance Directives - COOL provides you information about Advance Directives and helps you create one of your own to guide family members and medical professionals in the event you or a loved one are unable to give consent for treatments or procedures.

• Browse Our Library - The COOL Library includes a vast array of information of interest to individuals with disabilities or chronic health conditions, seniors, long-term care patients, caregivers and family members. Many of the articles were written exclusively for COOL based on research conducted by CareQuest University (www.CareQuestU.org) and cannot be found elsewhere. In addition to articles, the Library holds:

o Links to websites offering information on health-related topics o Publications on Medicare and Medigap health plans o Forms and checklists related to health care and caring for

individuals who are sick, disabled or elderly • Care-Alert System® - The Care-Alert section allows you to create

detailed emergency contact and medical information forms for you and your entire family.

• Return Main App Menu - Click/tapping on this tab returns you to the COOL Lpad.

Site User Registration – Creating Your File Cabinet To use the interactive Create-a-Care-Guide or save information, resources, cost calculations or other pertinent information for later use or printing a report, you will need to register as a user of the site and create your File Cabinet. You can register and create your File Cabinet in one of two ways. You can click/tap the Care Guide tab, scroll to the bottom of the page and click/tap on Begin a new Care Guide – or you can click/tap on the What is a Care Guide tab and at the top of the page, click/tap on the link in the shaded box, scroll down and click/tap on the Create File Cabinet link. In both instances, you will be taken to the Your CareOptions OnLine File Cabinet page. Once on the page, scroll down to the File Cabinet Setup section and:

• Enter your email address. • Create a password and password hint, enter your name and click/tap on

Continue • Answer the questions to create a file folder for the person you are creating a

Care Guide for and click/tap on Continue. The answers to these questions will enable COOL to identify resources in your geographic location and to suggest articles, resources and services selected especially for you.

When you are logged into your File Cabinet you can add articles, provider information or other information to your folder by clicking/tapping on the Add to Folder icon on the right sidebar of the page. Once an article has been added to the current folder, this image changes to say Added to Folder, so you always know whether you have saved the information you are currently reading. (Assessment and Cost Calculator results are automatically added to your current file folder. You need to register only once. Then, each time you return to COOL, you will sign in with your e-mail address and password to gain access to your private File Cabinet.

Note: The information you enter when you register is completely confidential. NavGate Technologies will never share it with any other person or organization. This information is used only to help you identify services, providers and costs in your geographic area, prepare a personalized Care Guide and give you faster, more accurate results when you search for information on the site.

Links to the COOL Glossary Whether you use Create-a-Care-Guide to help you create a comprehensive Care Guide or decide to search for resources, browse the library and use the various tools more selectively, you will find blue, underlined words or phrases. Click/tap on any of these words or phrases that are unfamiliar to you, to see their definitions in the Glossary.

Then use your browser’s Back button to return to the page you were reading. To browse the Glossary, click/tap on the word Glossary, which appears above the tabs at the top of each page.

Using Create-a-Care-Guide Create-a-Care-Guide directs you through the process of creating a customized Care Guide to meet your unique needs or those of a loved one. As you move through the Create-a-Care-Guide process, you can save the information you’ve collected in your personal file cabinet. If you want to create more than one Care Guide - for example, one for yourself and one for an elderly parent - you can create multiple file folders within your file cabinet. At the end of the process you will be able to print your customized Care Guide(s) to share with family, friends and medical and service providers.

Important Note: NavGateTM cannot be responsible for any lost data or Care Guides. Therefore, it is recommended you print and/or save, to your hard drive or disk, your complete or partially completed Care Guide.

Returning to your Care Guide You can leave your Care Guide and come back to it as often as you wish. Each time you return to the COOL website (and after entering your access code) you can:

• Access your file cabinet by entering your e-mail address and password from either the What is a Care Guide? tab or the Create-a-Care-Guide tab.

• Add to or edit your existing folders; delete folders; and/or switch between folders in your file cabinet.

Navigating Create-a-Care-Guide Here are some helpful navigation tips that apply only to the Create-a-Care-Guide section.

• To use the Create-a-Care-Guide process, you must be signed in to your File Cabinet. You will be able to proceed past the first step only after you create a new File Cabinet or sign in to an existing one.

• After signing in to a File Cabinet, you may navigate through Create-a-Care-Guide by:

1. Click/taping on one of the buttons in the Create-a-Care-Guide navigation box that appears in the upper right-hand corner of each page: the NEXT STEP button to move forward in the planning process; the PREV button to return to the previous step; or the EXIT button to stop the Create-a-Care-Guide process and return to the home page.

OR 2. Using the Jump to menu list to skip ahead or back to any Create-a-Care-Guide section. This is useful if you wish to re-visit any section that you’ve previously completed, or want to re-start the planning process when returning to COOL.

• You can go to your File Cabinet at any time by click/taping on the My File Cabinet link at the top of the page. When in this screen you can switch between file folders, if you are maintaining more than one folder.

Using COOL Without Using Create-a-Care-Guide If you don’t feel you need or want to use Create-a-Care-Guide to create a comprehensive Care Guide, you may still want to use the information and resources available on the COOL website. You can do this by simply ignoring the Create-a-Care-Guide tab. If at any time you decide you want to create a Care Guide, click/tap on the tab, Create-a-Care-Guide, and follow the instructions to create your own, individually tailored Care Guide. Creating and Printing Documents COOL allows you to create and print the unique documents you have developed for yourself and your loved ones by following these steps:

1. Tap/Click on My File Cabinet – found in the upper right hand corner of any COOL page. You will be taken the My File Cabinet page.

2. From here you can create and print the Care Guide information you have

assembled, print your Care-Alert documents and your Advance Directive documents.

3. Tap/Click on the on the documents you want to create/print and follow the prompts.

APPENDIX Case Studies The following three case studies are included to help you understand how to use several of the features of COOL. Case Study #1 “Creating a Care Guide for a Family Member” Background: Alice, an 75-year-old widow:

• Resides in Little Rock, AR • Lives alone • May be exhibiting early signs of Alzheimer’s Disease • Has had deteriorating physical health over the past couple of years as well • Takes four different medications a day

Emma, Alice’s 40-year-old daughter:

• Lives in Chicago, IL • Works full-time • Has two siblings who live in California

Situation Emma has become concerned about her mother’s mental functioning and her inability to perform the activities of daily living (bathing, continence, dressing, eating, toileting, and transferring) because of her deteriorating health. What does Emma do?

Using COOL Emma determines she needs to assess her mother’s situation very carefully and decides to travel to Arkansas for an extended visit. She hopes to answer the following questions and gain relevant information by using COOL:

• What is a Care Guide and how can it help me make decisions about my mother’s care?

• Does my mother need help with activities of daily living? • If my mother needs in-home assistance, how much will it cost and what

agencies provide it near her home? • How can I get an initial assessment of my mother’s cognitive abilities • Where can I learn more about Alzheimer’s Disease and the medications my

mother is taking COOL File Folder: After logging onto COOL using her access code, Emma arrives at the COOL Launch Pad™ (Lpad™) and click/taps on the Create a Care Guide App icon. She reads through the information about what is a care guide and then creates a file cabinet and folder for Alice. Emma will use the file folder to store and retrieve information pertinent to the Care Guide for Alice. Emma responds to several background questions while setting up her file cabinet, enabling COOL to supply information pertinent to Alice’s situation automatically from other areas of COOL. Emma selects the Create-a-Care-Guide tab, follows the easy-to-use navigational features and begins using COOL to obtain answers to her questions. By clicking/tapping on the directional arrows in the green box on the right side of each page she can start the Create-a- Care-Guide process and move through the various steps in the care planning process. What is a Care Guide? Emma reviews the basic information about Care Guides; the purpose of COOL and important things to consider when developing a Care Guide. Assessment Tools: Any good Care Guide must begin with a careful assessment of the person’s needs. Emma does the Quick Assessment which indicates that some follow-up assessments should be done. Emma believes she should conduct two assessments on her mother - the Functional Assessment to determine how her mother is coping with

activities of daily living; and the Cognitive Assessment to see if she should schedule an appointment with a physician for a complete professional examination and evaluation. Through a combination of observations and asking questions of her mother, Emma completes the two assessments and enters the responses online. The results of the Functional Assessment indicate that Alice is still living somewhat independently, but could use some in-home assistance such as home health care and perhaps a meal service. The results of the Cognitive Assessment indicate that Alice may be exhibiting signs mild cognitive impairment. Emma believes it would be prudent to have a professional assessment of her mother’s cognitive functioning.

Search for Resources: Emma and her mother agree that Alice could benefit from home health care. Using the comprehensive resources available on COOL, Emma can search for these services in Alice’s geographic area using the Search for Resources tab. Entering Alice’s zip code, a search radius and types of providers, Emma can obtain a complete listing of services in the area. The results of the search indicate there are numerous organizations providing these services. Emma click/taps-on Add to Folder those organizations she would like to save to her folder. COOL automatically adds a checklist to her Care Guide to help her assess the competency and quality of potential home health services. Emma can print the checklist before interviewing potential home care providers. COOL will also automatically add to her Care Guide articles from the Library appropriate to mother’s needs based on the questionnaire Emma filled out when creating a file folder for her mother.

Because she lives several hundred miles from her mother, Emma thinks it may be beneficial, at some point, to employ the services of a Professional Care Manager that can be a resource and advocate and coordinate services for her and her mom in the event Emma can’t be there. Emma enters her mom’s zip code in the search function and is presented with a list of professional care managers in the area. She adds the information to her folder for future reference. Cost Calculators: Based on the results of Assessments, it appears that fairly frequent home health care visits may help Alice continue to live in her own home, at least for the time being. However, the cost of care is a concern to Emma. Emma and Alice feel that a home health aide coming for 3-hour visits, 6 days a week should be sufficient. After entering this information into the Cost Calculator, Emma can determine the annual cost for such a service in Little Rock is approximately $16,000.

Medical Resource Center Emma wants to learn more about the medications that have been prescribed for her mom. She click/taps on the Medical Resource Center tab and then on the Medication Advisor link. Here she learns about her mother’s medications and their potential side effects. Browse our Library: In the library, Emma can find a variety of articles specific to her mother’s health issues. In addition to articles and information that will be recommended automatically based on the questionnaire she completed when setting up her mother’s file folder. Emma can also find information pertinent to her mom and add these articles to her file folder. She selects articles about Alzheimer’s Disease, home health care and the inability to perform activities of daily living.

Generate a Care Guide: Emma wants to pull together all the information she added to the folder in her filing cabinet as she worked her way through COOL. By click/taping on Care Guides and then the Access Your Care Guides link, Emma may now print Alice’s detailed Care Guide. She also saves the Care Guide as a PDF file onto her computer and e-mails it to her siblings in California and to Alice’s care providers.

Helpful Links: While COOL can provide Emma with virtually everything she needs to create a Care Guide for her mother, it also has established links with more than 60 other websites. These will allow Emma to find a wealth of additional information about virtually any aspect of long-term care, short-term care, disability care or eldercare.

Summary of Case Study #1 Using COOL, Emma has been able to assess her mother’s condition, identify resources and their costs, download articles of interest to her mother’s situation and generate a Care Guide. The Care Guide will be invaluable to share with medical and service providers responsible for Alice’s care and with her siblings. Emma leaves Arkansas with the peace of mind that her mother is receiving appropriate care and services.

Case Study #2 “Advance Directives – caring for those that may have to care for you” Background: Bill:

• Is 65 years old • Retired • Lives with his wife Betty, who is also 65 years old • Has two grown daughters • Is physically active and in good health

Situation Bill had a good friend, John, who had a severe stroke a couple of years ago, which left him in a vegetative state. Because John had not provided his family with clear instructions on his wishes, should be unable to communicate, his family struggled to make decisions about his care. Some family members thought John would have wanted no heroic measures taken to keep him alive while others thought differently. John’s situation was a real eye-opener for Bill. He vowed he would give his family clear instructions of his desires if he was in a similar situation and would give the authority to act on his behalf to one person. What does Bill do? Using COOL After logging onto COOL using his access code, Bill arrives at the COOL Launch Pad™ (Lpad™) and click/taps or taps on the Create a Living Will App icon.

Reviewing the Introductory Information In reviewing the introductory information, Bill learns that Advance Directives are documents that are better known as either a Living Will or a Power of Attorney for Health Care. He decides he wants to create both documents. The Living Will will give his health care professionals instructions about the type of medical treatments he wishes to have should he not regain consciousness or an acceptable state of health. The Power of Attorney for Health Care will allow Bill to designate someone to make decisions about his medical care in the event that his medical condition leaves him unable to communicate his wishes. Talking with his family As difficult a discussion it is to have, Bill sits down with Betty and his two daughters to discuss his wishes should he find himself in a situation where he has a terminal illness and/or is unable to communicate his wishes to his health care providers. Bill’s family understands and respects his wishes. Further, Betty agrees to be designated as his Health Care Power of Attorney. Creating the documents While an attorney can be helpful, Bill decides to create his own Living Will and Health Care Power of Attorney. He is confident that by carefully following the steps laid out on COOL, he can create documents that are within the guidelines of the laws of his state. (Please note: Each state has different requirements for creating Advance Directives. Please check with your state to make sure the documents you create meet state requirements.)

Bill returns to COOL and click/taps on the Begin button to create his Advance Directives. After entering some basic information (name, address, phone number, date of birth), Bill answers the questions in the Advance Directives Questionnaire including designating a Power of Attorney for Health Care. He adds and modifies some of the wording to best describe his wishes.

After click/taping on Finish, a PDF document is produced. Bill prints the document, signs it and has a witness sign it as well. The witness cannot be a health care provider, or family member. (Check with your state to determine whether one or two witnesses are required.) After executing the document, Bill provides copies to his immediate family members and his health care providers. The PDF document will also be automatically saved to his file cabinet on the COOL site.

Please Note: We recommend that you plan to complete the advance directive document in one sitting. If you need to stop in the middle of creating your document we cannot save it for you. You will need to start over.

Summary of Case Study #2 While it was a sometimes difficult discussion to have with his family, Bill feels better knowing that he has carefully communicated his wishes, should he be facing a terminal illness and/or is unable to communicate his wishes concerning his medical care. He also feels good knowing that his family is not placed in the terrible position of having to make literally life and death without adequate information about his wishes.

also lives with them Situation Joanne realizes her family’s various emergency contact and medical information is scattered in numerous places, in both electronic and paper formats. As a mother to two school-age children and a caregiver to her mother, Joanne want s make sure that in the event of an emergency, the people that would need access to this information would have access to it in a timely manner. She wants all the information about her family and their pet in one location where it would be accessible and easily shared with other family members, neighbors and health care providers. What does Joanne do? Using COOL After logging onto COOL using her access code, Joanne arrives at the COOL Launch Pad™ (Lpad™) and click/taps or taps on the Create a Care Alert App icon.

Joanne is automatically taken to the Care-Alert section of COOL. The Care-Alert section will allow Joanne to create detailed emergency contact and medical information forms and cards for her family, including their dog, Lenny. Creating Your File Cabinet Site User Registration To use the Care-Alert System, Joanne registers as a user of the site and creates a File Cabinet. The Fie Cabinet will allow Joanne to store all the information on each family member necessary to print Care-Alert Forms as well as (wallet-size) Cards for everyone. As a first timer visitor to COOL, Joanne click/taps on the Create File Cabinet button and enters her email address, creates a password and click/taps on Continue. Creating Your Care-Alert Forms and Cards Now that she has created a File Cabinet, Joanne can save information in it and return to it at any time, simply by logging on to COOL and then accessing her File Cabinet. Creating Care-Alert Forms and Cards is a simple-to-follow five step process. Joanne click/taps on Start a New Care-Alert® to get started and works her way through the five steps. Joanne:

1. Enters basic demographic information about herself 2. Adds information about their dog, Lenny 3. Adds information about her medical history, primary care physician and

lists her designated power of attorney for health care 4. Enters emergency contacts information 5. Reviews all the information and edits it as necessary; saves it to her File

Joanne then follows the same steps for each family member, creating Care-Alert Forms and Cards for each member. Joanne prints several copies of the forms and places them in their home, cars and other easily accessible places in the event of an emergency. Joanne also provides a copy of the Care-Alert Form to those she listed as contacts on it, and others who "need to know" should an emergency occur.

Summary of Case Study #3 In a matter of less than hour, Joanne is able to enter information on her family members and pet and print Care-Alert Forms and Cards. She has now ensured that vital information is communicated should an emergency occur. In addition, Joanne now has easy access to all her family’s information in one place and can easily make changes and create new Care-Alert Cards and Forms should information change.
