The law requires that parental permission be obtained so that medical attention can be administered to minors. The following consent form should be signed by parent/guardian in order that such procedures as judged necessary may be conducted without undo delays. However, no major operations will be performed, except in extreme emergency, without parent/guardian being contacted. I give permission for such diagnostic and therapeutic procedures as may be deemed necessary for my son/daughter, and also to present information concerning his medical condition to responsible clinic officials when deemed desirable.

Mike Ninivaggi is the assistant coach at Wantagh High School. He is a former Assistant Coach for the 1997 New York State Champion, East Meadow High School Baseball team. In his 19 years at Wantagh, He has been part of 15 division championships, The 2016 State Championship, three Nassau County Championships, and three Long Island Championships. Mike has helped direct the camp since its inception.

Keith Sachs is presently, and has been for the past 32 years, a physical education teacher and coach in the Wantagh School District. He graduated from Calhoun High School where he was an All Long Island player. Keith graduated from Hofstra University

where he was captain of the baseball team and named to the All-East Coast team. He has directed and organized various baseball camps in the northeastern United States. Coach Sachs has spent the last 25 years at the helm of the Warriors baseball team. In that time, Wantagh has become a baseball powerhouse. In 2016, the Warriors won Wantagh’s second New York State Baseball Championship. In, 2018, Wantagh won back-to-back-to-back L.I. Championships and

made the New York State Baseball Finals. During his tenure as head coach, Wantagh has won Two State Championships, Five L.I. Championships, Five Nassau County Championships, Seventeen Division Championships, and has reached the playoffs for 25 straight seasons. Coach Sachs was recently recorded his 400th career win.
