Instructions
To get the most out of this training, you are encouraged to participate in all activities and assignments.
For those of you who wish to be credentialed as a certified hypnotherapist, the requirement by the governing bodies is a minimum of 125 hours, and some organizations require more. In order to receive 125 of those hours from this course, you are required to attend all 40 hours of the live training, plus the home study portion, for which we are crediting you with 85 additional hours.
For those of you who wish to receive CEUs for your credentials, you will receive 40 CEUs for attending all the hours of the classroom portion. In order to receive more CEUs, you will need to accomplish a proportionate amount of home study assignments. Keep in mind that you will receive 45 hours by completing all of the listed assignments.
For the full 125 CEUs, In addition to the classroom hours, you will need to also have completed the assignments listed above, the online HIV Awareness and Ethics courses, as well as complete and document 40 hours of non-supervised practice sessions
If you are attending this course to simply learn and enjoy the hypnotherapy experience, you can complete as many of the home study assignments as you would like. You will be given a certificate for all hours completed.
In all cases, in compliance with the policies relating to continuing education, the number of hours that will appear on your certificate will reflect the number of hours that you attend the live training, plus a proportionate number of home study hours according to your checklist.
As you accomplish the tasks, you may check them off your list. It is suggested that you move through the assignments in the order given in the workbook and on the list, to build your knowledge and understanding of the techniques and their applications.
If you already have a practice, and have intake forms previously created, you can check that off your list. You do not have to create a new one for this course.
NOTE: Your Assignment Checklist will be submitted to the instructor not later than THREE WEEKS AFTER the final day of class. The instructor will review each checklist, and reserves the right to randomly request to view your work or ask questions about the reading assignments as proof of completion.

Practice Records
Note: To complete this course, you are required to conduct forty hours of unsupervised practice sessions. Each session can be shorter or longer than one hour. The important element is that you accumulate a total of forty (40) hours of practical experience, using techniques as they have been introduced. Enter your hours of practice in 10-minute increments.
You can be the “therapist” in all of the practice sessions, or be the “client” in up to half of the sessions. After each session, please complete a section of this form electronically (type in the fields). All homework assignments and practice records must be submitted not more than FOUR - SIX WEEKS** AFTER the last day of class. Simply fill in this record and attach it to an email. Submit it to [email protected]. Submit as a single WORD document, NOT as a pdf! Please keep a copy for your records.
** Make every attempt to complete practice sessions within four weeks. You may take up to six weeks. If you need additional time, confirm your specific needs with Susan Fields.
