• Sudden cardiac death (SCD) in young athletes is rare.
• Exercise is a trigger for SCD in predisposed athletes.
• Syncope is relatively common in athletes. Most cases are benign but may represent serious cardiac disease. The history (including witness reports) is an important discriminator.
• Pre-participation screening with ECG identifies athletes with electrical disease and cardiomyopathy.
• The ECG cannot identify all athletes at risk of SCD.
• Early CPR and AEDs save lives in sport.
