Make PopHealth more useful for provider practices. – Get EHR data into PopHealth– Provide functionality to practices that leverages that data.– Get the data out of PopHealth, with push to our Texas HIEs the
priority.• Providers will configure the software to periodically reach out to
their EHR and extract the information necessary to build a CCDA summary of care document for every Medicaid client.
• Alternately, CCDA docs would be procured via vendor-supported means, when available. PopHealth can then be configured by the provider to periodically push these CCDA docs to the HIE of their choice.
• HIEs will be able to offer profiles, downloadable from their website or pre-loaded in a PopHealth download.
