2011 STEAM GENERATOR TUBE INSPECTION REPORT
Technical Specification (TS) 5.6.7 of Appendix A, to the Donald C. Cook Nuclear Plant (CNP) Unit 1and Unit 2 Operating License requires that following the completion of an inspection performed inaccordance with TS 5.5.7, Steam Generator (SG) Program, an inspection report be submittedwithin 180 days after initial entry into Mode 4. CNP Unit 1 entered Mode 4 on October 21, 2011.This report details specific attributes of the inspection in accordance with TS 5.6.7. Consistent withthese requirements, I&M, the licensee for CNP Unit 1, is submitting the Unit 1 2011 SG TubeInspection Report as an enclosure to this letter.

Unit 1 2011 Steam Generator Tube Inspection Report
Introduction
UNIT 1In October of 2011, steam generator (SG) inservice inspections were conducted onDonald C. Cook Nuclear Plant (CNP) Unit 1. In accordance with the reporting requirements ofCNP Technical Specification (TS) 5.6.7, the results of the inspection are provided herein.
Unit 1 SG Description
The four replacement Babcock & Wilcox model 51R steam generators were initially placed inservice in December of 2000.
Each of the replacement steam generators contains 3496 thermally treated alloy 690 tubes withan outside diameter of 0.875 inches and a nominal wall thickness of 0.049 inches. The tubesare arranged in a triangular pitch pattern of 85 rows and 107 columns. The U-bend of all tubeshaving up to a 12 inch centerline bend radius (Rows 1-13) are stress relieved. In addition, thebundle has an increased tube bend radius i.e. the row 1 bend radius in the original steamgenerators was increased to 4.750 inches in the replacement SGs, to reduce the stressconcentration in the U-bend area.
The tube support structure consists of eight stainless steel (SA-240-410S-modified) lattice gridassemblies and four sets of fan bar (also called flat bar) restraints. Each lattice grid consists ofinterlocking high and low bars that form a lattice pattern to provide lateral restraint in the straightportion of the tubes. The fan bar restraints are made of the same stainless steel material andoffer support in the U-bend area.
The tubesheet is a nominal 21.25 inches thick and is made of ASME SA-508 Class 3a low alloysteel forging material with Inconel cladding on the primary side. All tubes are hydraulicallyexpanded into the tubesheet holes and flush seal welded at the primary face. The expansionincludes the entire depth of the tubesheet for both the hot and cold leg sides.
The Unit 1 SGs have accumulated 100.2 effective full power months of operation since beingplaced in service and 85.6 effective full power months of operation in the first sequentialinspection period as defined in the CNP TS 5.5.7.d.2. The SGs operated 39.8 effective fullpower months since they were last inspected.
As detailed in item "A" which follows, the 100% inspection scope of the 2011 examinationfulfilled/exceeded the TS inspection requirements for the second half of the first inspectioninterval. The next planned inspection of these SGs will occur at the end of the current cycle(Spring 2013).

A. The scope ofinspections performed on each SG
Primary Side Eddy Current Inspection Scope* A 100% bobbin coil probe examination encompassing the full tube length of all in-service
tubes was completed in all four SGs.
* A 20% (nominal) hot and cold leg top of tubesheet sample inspection using a rotating three-coil probe to examine the expansion transition (+/-3 inches) at the top of tubesheet wasperformed in all four SGs. Note: Some extents were increased to up to +12/-3 inches to fullybound foreign object locations.
* Special interest rotating three-coil probe examination was performed on all four SGs for allreported bobbin coil indication 'T'-codes, possible loose part signals and a sample ofreported freespan history, manufacturing burnish mark history and ding indications.
The following table summarizes the above inspections and the number of tubes examined.

Visual Plugq Examination100% of the installed tube plugs were inspected using a remote camera to confirm plug locationand condition. No abnormal conditions were identified during this examination.
Secondary Side Inspection ScopeVisual inspections were completed on each SG at the top of the tubesheet and in the steamdrum in one SG. Top of the tubesheet areas examined included the divider i.e. no-tube lane,bundle annulus, and select inner bundle passes. No abnormal conditions were recorded.
The steam dome of SG 13 was visually inspected. The inspection consisted of the accessibleparts of the secondary deck components, primary and secondary separators, feedring/J-nozzlesand upper tube bundle area. All visible steam dome areas appeared in solid mechanicalcondition with a uniform light powdery coating of deposits. No erosion or degradation wasobserved on any of the primary or secondary separators inspected. A video probe was also

lowered through the primary separator flow arms to the top of the tube bundle. This inspectionrevealed shiny tube u-bend surfaces and intact fan bar structures.
B. Active degradation mechanisms found
The only degradation mechanism detected during the inspection was wear. The source of thedegradation was support structures (fan bars and lattice grids) and foreign objects.
The table below summarizes the number of wear indications recorded in each SG during theinspection for the various mechanisms. The specific indications and their associated percentthroughwall (%TW) depth are listed in Attachments 1-7 of this report.

Fan Bar WearThe large amount of fan bar indications was not anticipated based upon previous inspectionresults which identified only 51 indications spread among the four steam generators during a50% inspection conducted in 2006.
Based upon the step change in indications, a root cause evaluation was performed whichdetermined that the tube wear was a result of a resonance situation which developed in thetubing. Prior to steam generator replacement, CNP Unit 1 instigated a reduced temperatureand pressure (RTP) program in the reactor coolant system (RCS). The intention of this programwas to prolong the life of the steam generators. The RTP had the affect of lowering RCStemperature by approximately 20°F and reducing secondary side steam pressure fromapproximately 805 psig to 670 psig. Following replacement of the steam generators in 2000,with a similar but different designed unit, RTP operation was continued.
As part of the root cause investigation, computer modeling was performed which suggests thelower steam pressure conditions associated with RTP, stimulate oscillations and create aresonance situation in the steam generator tubing. This in turn created slow growth wear at thefan. bar/tube intersection. The wear was initially below the level of detectability. However, assteam generator operating time increased, the wear continued to occur and increase in depth.Extended inspection intervals served to mask the discovery of the indications until the 2011inspection.
The maximum depth of the reported fan bar wear was 28% TW. The following tablesummarizes the fan bar wear depths recorded.

Initial project planning is now being performed to restore the unit to pre-RTP conditions in anexpeditious manner. Conservative repair decisions and degradation projections were maderegarding the fan bar indications to ensure tube integrity throughout an extended inspectioninterval of up to 36 months. However, as noted previously, inspections are conservativelyplanned for the end of the current cycle to monitor the condition and validate model projections.
Lattice Grid WearLattice grid wear showed a slight increase in the number of indications during Unit 1 Cycle 24over the previous inspection. During the 50% examination in 2006, 14 lattice grid wearindications were reported. The 2011 examination reported 33 lattice grid wear indications. Themaximum depth of lattice grid wear was 30%TW. The remaining lattice grid wear indicationswere all below 20%TW.
Foreign Obiect Induced WearFour indications of foreign object related wear were detected during the inspection.
SG 11 had a probable foreign object induced wear indication 1.72 inches below the sixth hot legsupport in tubes R58/C54 and adjacent tube R59/C53. No foreign object was detected at thislocation. The wear indications were sized at 25%TW and 39 %TVV, respectively.
SG 13 had a foreign object lodged between tubes R65/C25, R67/C25 and R66/C26approximately 10.5 inches above the hot leg top of tubesheet. Tube R65/C25 was the onlytube with a wear indication. The part was documented by secondary side visual inspections butcould not be removed. The associated wear was sized at 7%TW.
SG 14 had a single indication of foreign object wear contained within the span of the seventhhot leg lattice grid on tube R33/C57. The depth of the wear was 10%TW. The tube was notaccessible for visual inspection; however, the indication was indicative of the object still being inplace.

G. Results of condition monitoring, including the results of tube pulls and in-situ testing
Evaluation of the indications found during the 2011 inspection indicated that the conditionmonitoring requirements for structural and leakage integrity, as specified in TS 5.5.7, weresatisfied.
Degradation specific condition monitoring limits were applied to each degradation mode. Asshown in the figures below, no degradation exceeded the technical specification repair limit of40%TW or the condition monitoring limits. No detectable primary-to secondary leakage wasdetected during the previous cycle or is postulated to occur during current operating interval.

* Two condition monitoring limits were established for foreign object wear based upon whether ornot the foreign object was still present. Based on the shape of the flaws EPRI ETSS 27901.1was used in two instances when wear was evident but no foreign object was present. EPRIETSS 10908.4 was extended to size the wear on two additional indications in which the objectwas still present.
The inspection found no indications that met the criteria for in-situ pressure testing and no tubeswere required to be pulled.
