(b) the type of asbestos containing materials;
(c) the form of the asbestos containing materials;
(d) an estimate of the amount of each of the ACM's found; and

* "asbestos" is a fibrous mineral capable of disintegrating into airborne, respirable fibers that
present a hazard to human health if inhaled. It is the fibrous form of the mineral silicates belonging
to the serpentine and amphibole groups of rock-forming minerals. These include:
a. actinolite, amosite (brown or gray asbestos), anthophyllite, crocidolite (blue
asbestos), chrysotile (white asbestos), tremolite; and
b. any mixture containing one or more of the minerals mentioned; and
c. any of these materials that have been chemically treated and/or altered.
* (cont) ACM's are classified in two broad categories:
Bonded materials in which the asbestos is tightly bound into a cementitious or resinous
matrix. Examples are asbestos-cement (AC) sheets and pipes, fire resistant insulation
boards, brake discs and pads, vinyl floor coverings. Bonded products do not constitute a
hazard unless asbestos fibers are released and become airborne.
Non-bound or fibrous materials are where asbestos is not firmly encapsulated in a matrix
and therefore more prone than bonded products to release airborne fibers. Examples are
sprayed asbestos ceilings, thermal lagging, gaskets/seals and fire/heat protective clothing.
