Apply for a new ACC in HPOS Use this guide to apply for an Approved Collection Centre (ACC) in HPOS. Read about Approved Collection Centres on our website before you apply. You’ll find details about what you need to do and how to access HPOS. Most pathology registration fields are mandatory in HPOS. You must complete these. Apply for a new ACC in HPOS Use this guide to apply for an Approved Collection Centre (ACC) in HPOS.

Read about Approved Collection Centres on our website before you apply. You’ll find details about what you need to do and how to access HPOS.

Most pathology registration fields are mandatory in HPOS. You must complete these. If you miss a mandatory field, you’ll get an error message when you submit the application.

Step 1: get startedLog on to HPOS. Go to humanservices.gov.au/hposSelect My programs.

The My programs page will display. Select Pathology Registration.

Step 2: view your APA The Approved Pathology Authority Selection page shows a list of Approved Pathology Authorities (APAs). Select View the APA you’d like to create an ACC for.

Step 3: start your applicationThe Approved Collection Centre Selection page will display a list of existing ACCs.

Select Apply for New Collection Centre.

Step 4: complete your applicationThe New Pathology Collection Centre application form will display.

Complete the declarationSelect Declaration and read all the information. Select all the check boxes to confirm you agree.

Complete your collection centre details

Select Collection Centre Details and complete all the fields.

A premises within or adjacent to a facility means a premises next to, nearby or across the road from a requestor or a person connected to the requestor of pathology services.

To enter the locality or postcode, select the suburb from the list that appears when you enter the postcode or suburb.

Additional address lines will display once you enter the postcode.

Complete all address lines. Select the correct details from the list that appears you when type in the street name.

Complete your contact details

Select Contact Details and complete all fields.

Complete your lease detailsSelect Lease Details and complete all fields.

What we mean by these terms:

Lease details - These fields are mandatory, except for additional area used by ACC. Additional area means any other areas included in the lease such as bathroom facilities or storage areas. If you take up a lease extension or month to month lease, tell us in writing.

Person connected to a requestor - Check the Department of Health’s Red Book on their website to determine if you’re a person connected to the requestor.

Outgoings - Net outgoings are additional operating expenses paid as part of the rent such as carpark or electricity. The lease or sub-lease may list these under ‘services’. You must provide details of all outgoings and list each item with an amount or percentage. You can only enter digits in this field.

Rent - You’ll need to provide the amount of rent in dollars per year at the time the lease was agreed. If you don’t know the exact amount for future years, make an estimate. You must tell us when you do know the exact amount.

Reportable period for outgoings and rent - You must include details of rent and outgoings for the current and future years of the lease or sub-lease, for up to 5 years.

Monthly leases - If the premises has a month-to-month lease and you don’t know when it will finish, record the end date as 1 year after the current lease ends. If lease has been on month-to-month agreement for a long time, enter the end date 1 year from the date you apply.

Premises is for a Hospital or Community Centre. Tick the lessee option if:

the premises you’re in is a hospital or community centre

there’s no lease agreement or ownership

there is an agreement for occupancy.

If you don’t have a lease end date, enter the end date as 100 years from the lease start date.

If you don’t pay rent for the premises, enter $1.00 in the rent amount.

You need to complete the additional questions if there are other areas the ACC uses which are included in the lease, such as bathroom facilities or storage areas.

If you mark the APA as a lessee or sub-lessee of the premises, you’ll need to answer extra questions.

To enter the address, use the drop down boxes that appear when entering the postcode and address details. There is a 30 character limit for each of the following:

‘Building or Department’ field

‘Property’ field

Remaining address fields

If you select Yes to more options to extend the lease or rent per annum net of outgoings, you’ll have to complete extra fields.

You must complete the date fields for rent information.

Upload your supporting documentsSelect Supporting Documents.

Uploading documentsThe document files must be 10 mb or less in size. Split up larger documents and name them clearly, such as ACC (No.) - Part 1, Part 2.

The file name can only include these characters: A-Z, a-z, 0-9, space, hyphen, full stop and brackets ().

Floor plansYou can upload a hand drawn floor plan.

If you’re a new ACC or you have a new lease agreement, the floor plan must show the:

ACC name

full street address

area occupied by the ACC in square metres

size of each common area used by the ACC and other occupants of the facility

total size of the facility.

If you’re renewing and don’t have a new lease, you still need to supply a floor plan with the:

ACC address

location shown on the floor plan

size of the ACC in square meters or room measurements.

Select the type of document you want to upload from the drop down box in Description.

Select Choose and add your document.

Select Upload.

If the APA is a lessee or sub-lessee of the premises, you must include both the floor plan and lease documents.

Select Submit.

A Confirmation page will display. Check the details and select Confirm to submit your application.

The Approved Collection Centre Selection page will display. The ACC you applied for will be in the Pending Collection Centres section.

We’ll send a message to your HPOS mailbox to confirm we’ve got your application.

Step 5: create an invoice for your applicationYou need to create an invoice and pay for your ACC application. Select Payments.

The Payments screen will show a list of any raised invoices. Select Create New Invoice.

The New Invoice page will display a list of ACC applications without an invoice.

Select the Pay check box for the ACC application you want to create an invoice for. You can include multiple applications on one invoice.

Select Create Invoice to submit your selection.

The New Invoice Confirmation page will display. Check the details are correct then select Confirm.

The View Invoice page will display the invoice details and payments steps. Select Close.

The Payment page will display. To go back to the Approved Collection Centre selection, click on the back key.

Step 6: pay for your applicationWe’ll send you a message in your HPOS mailbox with details of how to pay for application.

You must pay the invoice before we can process your application. Read more about how to use HPOS messages on our website.

When we’ve approved your application, we’ll send you an email with your approval letter and ACC certificate to your HPOS mailbox.

Step 7: check your receiptTo view your receipt, go to the Approved Collection Selection pageSelect Payments.

The payments screen will display. Select Get Receipt.

The receipt will display as a PDF.

Step 8: log out For your privacy and security, log out when you’ve finished using your HPOS account.
