Complete MV1, Wisconsin Title and License Plate Application, including fees for tax on the purchase price, title and loan filing if there is a loan on the vehicle. o If you wish to obtain license plates/registration, vehicle must meet all applicable equipment standards in Chapter 347 Wisconsin Statutes and Wisconsin Administrative Rule Trans 305 prior to it being operated on the road. Submit any previously issued title or registration documentation, or bill of sale if available Submit pictures of the vehicle, including front, rear, side views and Vehicle Identification Number (VIN) o If you do not have a clear picture of the VIN download MV3623 Public Vehicle Identification Number Certification form to be completed by law enforcement. Complete the written statement on page 3 of this application with the following information: o How you obtained the vehicle o Whom you obtained the vehicle from (provide names of any known previous owners) o How you acquired the vehicle without a title and the steps taken to obtain a title o How long the vehicle has been in your possession o Include any history you are aware of for this vehicle, such as if it was previously salvaged, rebuilt/ salvage, flood damaged, used as a taxi or police vehicle in this or any other state. Submit the completed application to: Vehicle Research Unit Wisconsin Department of Transportation P.O. Box 8070 Madison, WI 53708-8070 STEP 2 If the application is complete, the Wisconsin Department of Transportation (WisDOT) will: Determine whether there is an active title record on file for the vehicle in the state of Wisconsin or any other state. o If no record is found, WisDOT will continue processing the application for a bonded certificate of title. o If a record is found in Wisconsin records you will be notified and required to complete form MV2896, Vehicle/Driver Record Information Request to receive information regarding previous owners. o If a record is found in other state(s)’s records, you will be notified and required to seek previous owner information in that state(s). Determine if the vehicle meets minimum vehicle requirements. If the vehicle does not meet minimum requirements the Department will request the vehicle be brought to those standards before proceeding with the Bond process. Review any bill(s) of sale and consult with price guides. Send the applicant a letter to advise of vehicle valuation and request the following items: o A surety bond describing the vehicle for one and one half times the value of the vehicle. o Any items initially not received with the application. INSTRUCTIONS STEP 1

• Complete the Application for Bonded Certificate of Title including the statement section.

• Complete MV1, Wisconsin Title and License Plate Application, including fees for tax on the purchase price, title and loan filing if there is a loan on the vehicle.

o Include any history you are aware of for this vehicle, such as if it was previously salvaged, rebuilt/ salvage, flood damaged, used as a taxi or police vehicle in this or any other state.

STEP 2

If the application is complete, the Wisconsin Department of Transportation (WisDOT) will:

• Determine whether there is an active title record on file for the vehicle in the state of Wisconsin or any other state.

o If no record is found, WisDOT will continue processing the application for a bonded certificate of title.

o If a record is found in Wisconsin records you will be notified and required to complete form MV2896, Vehicle/Driver Record Information Request to receive information regarding previous owners.

o If a record is found in other state(s)’s records, you will be notified and required to seek previous owner information in that state(s).

• Determine if the vehicle meets minimum vehicle requirements. If the vehicle does not meet minimum requirements the Department will request the vehicle be brought to those standards before proceeding with the Bond process.

• Review any bill(s) of sale and consult with price guides.

o How long the vehicle has been in your possession.

o Include any history you are aware of for this vehicle, such as if it was previously salvaged, rebuilt/salvage, flood damaged, used as a taxi or police vehicle in this or any other state.

Submit any previously issued title or registration documentation, or bill of sale if available. The bill of sale should list the vehicle year, make and vehicle identification number (VIN). The bill of sale should also contain signature(s) of seller and purchaser(s).

Pictures from all 4 sides of the vehicle.

Picture of the Vehicle Identification Number (VIN) tag or form MV3623 Public Vehicle Identification Number Certification, completed by law enforcement.

Check or money order made payable to Registration Fee Trust for the total due listed on the MV1. Keep copies of all application items.

DO NOT obtain a surety bond until you have received a written request for the Wisconsin Department of Transportation listing the amount of bond required to issue a Wisconsin title.
