INTRODUCTION

Patients, their families and friends commonly require support and assistance inorder to cope with the broad range of needs that arise when approaching thefinal months of life. Likewise, health care providers require referenceinformation to enable them to assist patients and their families. Thispublication seeks to address these needs by providing a compilation ofcommunity resources, including hospices and other organizations and agenciesthat are especially attuned to end-of-life concerns and the commitment toimprove the care of the dying.

In this document, we strive to share resources that enable and empower us tocounter the pain, isolation, and loneliness, and to help with other aspects ofadvance planning in life’s final months.

This directory is a beginning step in bringing together in one publication alisting of support services available in Colorado. Inclusion in the directorydoes not reflect endorsement of a program, or of any specific resource orpractitioner over another. No doubt, many available resources have beeninadvertently omitted. We encourage users of this directory to supplement andupdate our listings by use of the form appearing at the back of this publication. Because changes to telephone numbers do occur frequently, some of thesenumbers will become outdated after publication. The “800" numbers can beverified by calling 1-800-555-1212.

COMFORT CARE

Comfort care or palliative care interventions alleviate distressing physicalsymptoms and address psychological, social, emotional and spiritual needsassociated with the disease process. The goal is a more comfortable existencewithout prolonging the dying process or aggressively sustaining a quality of lifethat would be unacceptable to the patient. Comfort care uses less invasivemethods to support the patient’s physical and emotional well-being, such asmedication, physical therapy and relaxation techniques, counseling andspiritual guidance.

The “Comfort Care Resources” section of this guide includes an alphabeticlisting, by county, of names and telephone numbers for the following:

Adult Day Programs: Health and social services, individual therapeutic andpsychological activities furnished on a regularly scheduled basis in an adult dayservices center, as an alternative to long term nursing facility care.

Bereavement Support Resources: Bereavement support services, includingeducation, counseling, and grief support groups, are offered to families andfriends following the death of an individual and to communities followingtraumatic events. Bereavement services can be obtained through localcommunities or hospice programs.

Community Mental Health Clinics: Mental health programs offeringassistance to determine decision making capability, suggested medications foragitation and mental health assessment and treatment planning.

County Health Departments: Agencies available through local countygovernment offices that may provide direction to additional resources andservices in a particular area.

Funeral Homes/Mortuaries: A location that provides funeral directing,embalming and/or cremation services to the public. Only those funeral homesand mortuaries belonging to the Colorado Funeral Directors Association areincluded in this directory..Home Health Agencies: Part-time or intermittent skilled nursing services,other therapeutic services (physical therapy, occupational therapy and speech

therapy), and home health aide services made available on a visiting basis in aplace of residence used as the patient’s home. Only those home healthagencies that are certified for Medicare reimbursement are included in thisdirectory.

Hospice Programs: Coordinated programs of intermittent home and inpatientcare, available 24 hours a day, using an interdisciplinary team to providepalliative and supportive services to a patient-family unit experiencing a lifethreatening disease with a limited prognosis. Hospice addresses the medical,psychosocial, spiritual, and practical needs of the individual and the relatedneeds of the family and loved ones throughout the periods of illness andbereavement. Hospice usually includes medications and biologicals related tothe terminal illness. Although normally provided in an individual’s privateresidence, hospice care may also be available in long term care facilities or inhospice in-patient units.

Hospitals (General): A health facility that offers and provides twenty-fourhours per day, seven days per week, inpatient services, emergency medical andsurgical care, continuous nursing services, and necessary ancillary services toindividuals for the diagnosis or treatment of injury, illness, pregnancy, ordisability.

Medical Society: The county medical societies that are listed are staffed and try to assist patients with questions regarding medical issues within theircommunities.

Senior and Disabled Services: County supported agencies that provide a rangeof services for senior and disabled persons.
