Even when appropriate clinical steps lead to a correct diagnosis, diagnostic errors may still occur due to failures in follow-up systems. Problems with follow-up and care coordination lead to poor patient outcomes, patient dissatisfaction, care team burnout, inefficiencies and redundant testing - all contributing to poor business performance. This presentation will provide you with resources and tools that will help to reduce the incidence of follow-up system failures in your organization. Following this session, the participant will: • Understand causes and contributing factors of breakdowns in diagnostic care processes
