“ALL STAR” HOMEOWNERS PROGRAM

PROPERTY COVERAGES will automatically be periodically updated based on changing dwelling construction cost factors. A statement to this effect is contained in the application and the application must be signed by the applicant. The residence must be insured to 100% of Replacement Cost, the application must be completed and a photograph of the residence attached.

The “ALL STAR” Homeowners Program is designed for risks of a better class in all respects and basically for dwellings that are 30 years old or less. This will require careful original risk selection and continuous monitoring by the agent.

COVERAGES

The “ALL STAR” Homeowners policy provides property and liability coverages as described below.

PROPERTY COVERAGE SECTION

The NS-5 “ALL STAR” Homeowners Form provides the following property coverages:

1. Covers the residence, private structures, personal property and additional living costs against risks of direct physical loss unless the loss is excluded.

2. Provides expanded coverage replacement cost coverage to the dwelling not to exceed 125 percent of the Coverage A limit.

3. Replacement Value on personal property is an automatic coverage, subject to the limit shown on the Declarations for Coverage C.

4. Refrigerated Property coverage is automatically included. This provides coverage, without special limit, for refrigerated property lost due to interruption of electrical service or by mechanical or electrical breakdown of the refrigeration system. No deductible applies.

5. Debris removal coverage is broadened to include $1,000 coverage ($500 per tree) for the cost of the removal of tree debris caused by Windstorm or Hail (need not have damage to “insured property”).

6. Lock And Garage Door Transmitter Replacement Coverage is automatically included. This provides coverage for the cost to replace locks on exterior doors of the insured dwelling when the keys to those locks are a part of a covered theft loss. This also provides coverage for the cost to replace a portable transmitter that is part of an automatic garage door opening system if such transmitter is lost or stolen. No deductible applies.

LIABILITY COVERAGE SECTION

1. Personal Liability - Covers payment on behalf of the insured of all sums which he/she shall become legally obligated to pay as damages because of bodily injury or property damage caused by an occurrence arising out of his/her premises or personal activities.

2. Medical Payments - Covers medical expenses incurred by persons, other than the insured, who sustain bodily injury caused by an accident arising out of the insured’s premises or personal activities.

3. Personal Injury coverage is automatically included up to the Personal Liability limit. 4. Watercraft with outboard motors of 50 HP or less are automatically included for liability and medical payments.

STANDARD LIMITS

Following are the standard limits under the “ALL STAR” Homeowners Policy:

MANDATORY COVERAGES

It is mandatory that insurance be written for all coverages provided under both the Property Coverage and the Liability Coverage Sections of the Homeowners Policy.

The Liability Coverage Section of the policy requires coverage for the following exposures and the additional premium developed from the Premium Charts must be charged when such exposures exist:

1. ALL additional residence premises where the Named Insured or spouse maintain a residence other than business or farm property;

2. ALL residence employees of the Named Insured or spouse not covered or not required to be covered by Workers Compensation Insurance (charges required for residence employees in excess of 2); and

3. Incidental office, professional, private school or studio use by the insured on residential premises of the insured.

ELIGIBILITY AND UNDERWRITING REQUIREMENTS

1. A completed Homeowners application must be submitted along with at least two recent photos of the property (front and rear). Photos of all structures on the premises should be submitted for review. Use the MSB/RCT Software program and attach the RCT printout.

2. Only 1 family owner-occupied dwellings (single unit dwelling only – no townhouse/duplex) used only for private residential purposes and with no more than 2 boarders or roomers are eligible.

3. A policy may be issued to cover a dwelling in the course of construction provided the policy is issued only in the name of the intended owner-occupant of the dwelling and will be completed within approximately 6 months. Coverage must be written at 100% of expected completed value.

4. The minimum limit available shall be $200,000. 5. Maximum limits available for dwellings are $1,000,000 if Protected, $800,000 if Partially Protected, and

$750,000 if Unprotected. Maximum limit available for dwellings that qualify for the Protected Subdivision Discount is $800,000.

6. The dwelling must be insured for 100% of current replacement cost. Contents will also be on a replacement cost basis and cannot be written for less than 75% of Coverage A - Residence.

7. A new building valued at $5,000 or more must be reported within 90 days. An addition to or remodeling of a covered building which increases its value by $5,000 or more must be reported within 90 days. 8. A smoke detector should be placed on each floor. For dwellings with 3 levels, a minimum requirement is that

smoke detectors be placed on at least 2 levels. 9. Only dwellings that are 30 years old or less are eligible for new business or for renewal policies which are being

converted to this plan. Once written on this policy, renewals under this plan will be considered for dwellings exceeding 30 years, subject to proper maintenance, updating and good loss experience.

10. The roof needs to have been reshingled in the last 20 years. (Roof surfaces that are 15 years old or older may not be insured for replacement cost coverage.)

11. Dwellings are not acceptable if they have older wood roofs, clay tile roofs, or if they have asbestos slate roofs or siding.

12. Seasonal dwellings are not eligible for the “All Star” Homeowners policy and are to be written as a separate policy under our Standard Homeowners program.

13. Dwellings with masonry fireplaces that have a masonry/brick chimney and clay liner will be acceptable. Dwellings with UL approved fireplaces and UL approved metal chimneys are acceptable if they have been installed after January 1, 1986. Dwellings and other structures with any other type of woodburning equipment are not acceptable.

14. Farm Liability is required when: a. there are more than 40 acres; b. the insured has farming operations; or c. there are horses or other livestock. 15. Following is a list of Homeowners risks not to be bound and generally will not be written:

a. Property that is in generally below average physical condition. This program is designed for risks of a better class in all respects. This will require careful original risk selection and continuous monitoring by the agent.

b. Risks cancelled or nonrenewed by other carriers. c. Risks which have sustained 2 OR MORE losses during the past 3 year period. d. Dwelling risks which were not originally designed for dwelling purposes. (e.g., a mercantile building

converted to a dwelling.)

e. TRAILER HOMES, MOBILE HOMES, DOUBLE WIDE MOBILE HOMES, CONVERTED MOBILE HOMES OR HOUSE TRAILERS whether permanently located with attached leans or not. THESE ARE SPECIFICALLY EXCLUDED FOR HOMEOWNERS COVERAGE.

f. Hobby Farms or any risk formerly occupied as FARM PROPERTY on which any former farm buildings still exist.

g. Homes with more than 2 related private structures or with a related private structure larger than 1,600 square feet.

h. Dwellings without a central heating system (other than all-electric homes). i. Unusual risks including basement homes, log homes, earth homes, dome homes, and homes with flat roofs.

j. Risks where insureds have a history of financial instability within the last 5 years. k. Risks with wild animals or pets that show aggressive or vicious tendencies. l. Risks with Dobermans, Rottweilers, Presa Canarios (also known as Canary Island Cattledog), Pit Bulls,

including, but not limited to, American Pit Bull Terriers, American Staffordshire Terriers, or Staffordshire Bull Terriers, Chows, or dogs with aggressive tendencies.

m. Do not submit risks with swimming pools, unless prior approval has been received from a North Star Underwriter.

POLICY TERM

Policies are written on a 1 year term basis. The policy will automatically be renewed each year with an updated declarations page. The policy will be subject to the premiums, rules and forms in effect at the time of renewal. Policyholders will initially receive copies of all policy forms. Upon renewal they will receive only an updated declarations and forms that are new or revised. Policyholders may receive all policy forms upon request – a statement notifying the insured of this will be on each annual declarations.

FIRE PROTECTION DEFINITIONS

The following 3 fire protections will apply:

1. Protected: Residence is located within 1,000 feet of a fire hydrant and within 5 road miles of a responding fire department.

2. Partially Protected: Residence is located over 1,000 feet from a fire hydrant, but is within 5 road miles of a responding fire department.

3. Unprotected: All others.

Dwelling normally rated as partially protected may be eligible for a Protected Subdivision Discount - see requirements on page NSASHO-12.

The General Policy Terms, a perils section and liability coverage section are issued with every policy. When assembled, the policy must be made up of the following parts:

BILLING PROCEDURES

See the Billing Information section of this Agents Manual.

CALCULATION OF PREMIUM

Step 1 - Determine the Base Premium based on Protection Class, and the amount of Coverage A.

Step 2 - Apply applicable Location of Risk Modifier from page NSASHO-4 to the rate determined in Step 1.

Step 3 - Apply Masonry Construction Discount, if applicable, to premium determined in Step 2.

Step 4 Apply Roof Surface Replacement Cost Factor, if applicable, to the premium determined in Step 3.

Step 5 When Metal Roof Discount is desired, apply the appropriate factor to the premium determined in Step 4.

Step 6 - If writing Actual Cash Value Loss Settlement - Windstorm or Hail Losses to Roofs or Actual Cash Value Loss Settlement - Windstorm or Hail Losses to All Exterior Surfaces, apply the appropriate factor to the premium determined in Step. 5.

Step 7 - To increase Coverage C, add the appropriate amount to the premium determined in Step 6.

Step 8 - Modify the amount determined in Step 7 to reflect the selection of a deductible option. (The Base Deductible is $1,000).

Step 9 - Modify the amount determined in Step 8 for all other percentage discounts or charges. Discounts or charges should be multiplied consecutively in the following order: Protected Subdivision Discount, Ordinance or Law, New Home Discount, Protective Device Discount, Mature Homeowners Discount, Companion Policy Discount, Wood Shingle Surcharge. Discounts or charges should not be added together.

Step 10 - Add the additional premium for all other mandatory or optional Property Coverages and all other mandatory or optional Liability Coverages.

Step 11 - Apply the applicable Valued Customer Discount factor to the total policy premium.

Step 12 - Apply the applicable Insurance Score factor to the total policy premium.

In the event that a covered loss occurs as a result of one occurrence to property covered under more than one policy insured with North Star Mutual, only the highest applicable deductible will apply, regardless of the number of covered property items damaged or destroyed.

Application of the Insurance Score

1. The insurance score will be applied to all new business and to existing policies at renewal. 2. A statement to indicate an insurance score has been used will be shown on the policy Declaration when an insurance score is used. 3. The insurance score used, including No Hits and No Score, will apply for a minimum of 12 months except in cases where corrections

have been made for errors in the insured’s credit history file with the national credit bureaus. A higher score obtained in this process will be applied back to the beginning of the current policy term.

4. The insurance score will be ordered every 3 years, unless an insured requests it at the time of a renewal. However, we do not need to obtain the updated insurance score more frequently than once in a 12-month period.

5. A No Hits or No Score will continue to apply to a policy except in cases where an insurance score is obtained from a national credit bureau. If an insurance score is returned, it will apply.

6. In order to comply with statute, a Neutral score from LexisNexis indicates that the insured’s credit report has been influenced negatively by an extenuating life circumstance such as catastrophic injury or illness, temporary loss of employment or the death of an immediate family member.

7. On or after May 15, 2015, if the insurance score drops at the 3-year review, the amount of increase to the insurance score factor is limited to 0.04 per policy period.

ORDINANCE OR LAW

The policy provides coverage for the increased cost which results from the enforcement of a code, ordinance, or law which regulates the use, construction, repair, demolition, or removal of property following a covered loss. If the covered loss plus the increased cost is more than the applicable limit, an extra 10% of the Coverage A limit is available to cover the increased cost. This 10% limit may be increased by applying the following factors to Coverage A rating:

ROOF SURFACE REPLACEMENT COST FACTOR

As roof surfaces age they become more susceptible to damage by windstorm and hail. To maintain replacement cost coverage, a charge will be made on dwellings with asphalt shingles or Class 4 impact resistant roof surfaces 16 years old and older. The charge also applies to increased limits of Coverage B (HO 3048). For steel or asphalt roofs under 5 years old or Class 4 impact resistant roof surfaces under 14 years old, a credit will be applied. No combinations of discounts will be allowed. Asphalt or steel roofs cannot also qualify for the Class 4 discount.

Impact resistant roof surfaces include any non-wood material earning an impact rating of Class 4 per Underwriters Laboratory (UL) Standard 2218. To qualify for the discount, the impact resistant roof surface must cover the entire roof surface, be installed according to the manufacturer’s specifications, and not be overlaid onto an existing roof. The insured must provide documentation showing the type of roof surface, the date of installation, and the UL Class Rating.

The roof surface age is determined by the difference between the current calendar year and the year the roof surface was installed. For example, calendar year is 2012 and the roof surface was installed in 1995, the roof surface age would be 17 years. When a dwelling has multiple roof surface ages the oldest roof surface age will be used to determine the level of charge.

This factor is not applicable when Actual Cash Value Loss Settlement – Windstorm or Hail Losses to Roofs (CF-1545), Actual Cash Value Loss Settlement – Windstorm or Hail Losses to All Exterior Surfaces (CF-1738), or Cosmetic Damage Exclusion (CF-2051) applies.

METAL ROOF DISCOUNT (Cosmetic Damage Exclusion applies)

Dwellings

Dwellings are eligible for a 10% discount when the dwelling has a metal roof surface. The discount also applies to increased limits of Coverage B (HO 3048). This discount is optional and when taken, form CF-2051 Cosmetic Damage Exclusion is added to the dwelling and related private structures insured under Coverage B (HO 3048).

Related Private Structures – NS-248, NS-3540

A metal roof discount is available on a Related Private Structure, specifically insured using Form NS-248 or NS-3540 if it has a metal roof surface. This discount is optional and when taken, form CF-2051 Cosmetic Damage Exclusion is added to the insured structure. Rating information for each form is shown in the Related Private Structures section of this manual.

NEW HOME DISCOUNT

A new dwelling may be eligible for a premium discount as shown in the tables below.

PREMIUM DISCOUNTS FOR PROTECTIVE DEVICES - HO 6516

The following premium discounts will be allowed for the installation of a burglar alarm, fire alarm or automatic sprinkler system in the dwelling. Systems must be UL approved and maintained in good working condition.

1. Central Station Burglary and/or Fire Alarm System or Smart Home System that alerts central station via internet enabled monitoring system of fire, burglar and automatic water leak shut off - a discount of 10%.

2. Alarm System that alerts the Fire Department or Police Department or Smart Home System that alerts owner of fire, burglar or water leak - a discount of 3%.

3. Automatic Sprinkler System with sprinklers in all areas including bathrooms, attics and attached structures - a discount of 3%. 4. Automatic Sprinkler System with sprinklers not in all areas - a discount of 2%.

PROTECTED SUBDIVISION DISCOUNT

Dwellings normally rated as partially protected are eligible for a Protected Subdivision Discount if the following conditions are met:

1. The dwelling is located within a platted subdivision consisting of five or more homes; 2. The dwelling is within clear view of other occupied dwellings; 3. The subdivision, or a portion thereof, is located within 5 road miles of a responding fire department; 4. The responding fire department has the ability to transport at least 2,500 gallons of water in a single trip; 5. The dwelling is the primary or principal residence of the named insured; 6. The dwelling is located on a good access road; and 7. The dwelling is not located in a severe brush or forested area.

Apply a 10% discount to the partially protected Homeowners Policy base premium rates.

VALUED CUSTOMER DISCOUNT

A discount, for customers of continuous Homeowners policies written with North Star Mutual for 3 or more consecutive years, will be applied starting with the 4th policy period. The discount, as shown in the table below, will be applied to the total policy premium for each policy at each yearly policy anniversary. The maximum discount is 12%, as shown below. IDENTITY THEFT PROTECTION

This protection consists of:

1. Identity Theft Recovery Service

An advocacy service provided through our partnership with CyberScout. This fully case-managed expert service takes care of victims’ problems by leading them through the restoration process, including notifying the appropriate authorities, dealing with governmental agencies, credit collection agencies, credit bureaus, and credit grantors until the crisis is resolved. They even follow-up months later to ensure success.

2. Disaster Document Recovery

CyberScout also assists disaster victims rebuild their original means of identification, as well as recreate other crucial personal and financial documents or help them with emergency authentication if they are impacted by disasters, such as hurricanes, tornados, earthquakes, including acts of terrorism, as well as home fires and floods - regardless of whether an identity theft occurred. Should an identity theft occur as a consequence of such a disaster, their advocate is there to protect and rebuild their identity.

3. Identity Fraud Expense Coverage

Coverage of $5,000 is provided, at no additional cost, for expenses incurred by an insured as the direct result of identity fraud. Identity fraud occurs when an individual’s personal identifying information is used without his or her permission to purchase goods or services, obtain credit, borrow money, or otherwise violate federal, state, or local law. A $100 deductible applies to Identity Fraud Expense coverage.

Coverage is provided for expenses resulting from identity fraud that occurs at any time prior to the end of the policy period and is discovered by the insured during the policy period or during the one-year period that follows. In the event that the insured obtains replacement identity fraud expense coverage, the one-year extended discovery period terminates as of the date the replacement coverage takes effect.

Covered expenses include:

• Costs for obtaining, reproducing, notarizing, or sending affidavits or other pertinent documents; • Costs for telephone calls; • Research fees; • Lost earnings, not to exceed a maximum of $250 per day, per insured, or a total of $5,000 for lost earnings

of all insureds; • Loan application fees for reapplying for a loan; and • Attorney fees to defend lawsuits, remove criminal or civil judgments, or challenge information provided in a

consumer credit report.

For an additional premium, Installed Equipment Breakdown Coverage can be added to the Homeowners. Installed Equipment Breakdown covers mechanical and electrical breakdown of installed equipment in a dwelling. Covered equipment includes: equipment designed and built to operate under internal pressure or vacuum, other than weight of contents, communication and computer equipment and other electrical or mechanical equipment. UNDERGROUND SERVICE LINE COVERAGE – CF-2158

The property coverage form will be extended to provide coverage for direct physical loss to service lines that are owned by the insured or for which the insured is legally liable to repair. This will include such things as underground pipes, including their valves and equipment and also underground wires; that convey a utility service such as water, sewage, electricity, gas, steam or communications to or from the residence on the described location. The loss must be caused by or result from a mechanical breakdown, artificially generated electric current, wear and tear, freezing, including frost heave and thaw or weight of equipment, animals, or people. The limit of coverage is $10,000. The most we will pay for any tree, plant, or shrub is $500. A $500 deductible applies to this coverage

This is an automatic coverage on all Homeowner policies. The insured may request to have this coverage removed.

Additional Premium: $20

The Insurance Score does not apply to this coverage.

RELATED PRIVATE STRUCTURES

Occupied by Insured - HO 3048

Coverage B is intended to provide coverage for structures that are pertinent to the household and not used or designed for business purposes. Typically these are detached garages or small backyard storage sheds for lawn mowers, snow blowers, garden equipment, etc.

When an additional amount of insurance is written on a specific private structure, the following premium shall apply separately to each such structure:

$6.00 per $1,000 coverage (all forms)

This rate must be adjusted by the same Roof Surface Replacement Cost Charge or Metal Roof Discount that is applied to the Dwelling. Form CF-2051, Cosmetic Damage Exclusion is added to the insured structure when the Metal Roof Discount is given.

On Premises - Specifically Scheduled Structures - NS-248

When an acceptable private structure is over 1,600 square feet or there is more than 1 private structure on the premises then the private structure must be specifically scheduled for its full replacement cost. The limit that applies to Coverage B does not apply to specifically scheduled structures. With underwriting approval, a building is acceptable when the following are met:

1. Building is not used or designed for business purposes. 2. Building must be in good condition and qualify for replacement cost. 3. Building must be written at 100% of replacement cost.

THEFT COVERAGE - RESIDENCE UNDER CONSTRUCTION - NS-413

All Homeowners forms exclude loss by theft or attempted theft in or to a residence being built or theft of materials or supplies for use in construction of the residence until the residence is occupied. The North Star has available this optional endorsement to provide theft coverage from a residence while it is being built and before it is occupied by the insured.

The Homeowners rules permit the writing of a Homeowners policy on a residence under construction for occupancy by the insured. This endorsement may then be attached to provide theft coverage in those situations.

This endorsement gives the theft coverage for 180 days at a flat premium charge of $20 per $1,000 of coverage. You must specify the amount of coverage wanted on this endorsement. The coverage provided by this endorsement is subject to the policy deductible. The premium charged for this endorsement is a retained premium.

For an additional premium, coverage for direct loss to covered property caused by water or matter present in the water that enters the residence through sewers, drains and sump systems may be added. This coverage does not apply to coverage B and D. The policy deductible amount and terms apply to this coverage.

The following underwriting eligibility criteria must be met when writing sewer back-up limits of $10,000 or more:

1. No previous sewer back-up claims in the past 5 years. 2. There must be a sump pump with a battery back-up and/or a generator on site if the home is not a patio home

ENHANCEMENT ENDORSEMENTS

The Homeowner Enhancement Endorsements increase the internal Limits on Certain Property which are built in to the Homeowner base policy form. Two options are available - the HO Extra or the HO Premier. The Arson Reward coverage is an added coverage under either option. Special Limits under Coverage C are increased to the amounts specified for each form. If an increase is requested on individual items, coverage then can be obtained either by not attaching these endorsements and using the previous method or by adding scheduled Inland Marine coverage where available.

EARTHQUAKE COVERAGE - HO 2754

For an additional premium, we insure for direct physical loss to property covered under Coverage A, B or C caused by earthquake. When the Earthquake peril is written, it must be written for the same coverages and the same limits as the other coverages written in the policy.

The deductible is 5% of the limit for each coverage and is subject to a $250 minimum. If the limit on property is increased by endorsement, the total limit is used to determine the deductible.

LOSS ASSESSMENT/ASSOCIATION DEDUCTIBLE – DESCRIBED LOCATION

Property coverage for loss assessment/association deductible, and liability coverage for loss assessment, arising out of the described location is included at $1,500 for loss assessment/association deductible under incidental property coverages and at $1,500 for loss assessment under incidental liability coverages. The limit under incidental property coverages may be increased.

Loss Assessment coverage (under Incidental Property Coverages) applies when the insured’s financial obligation arises out of an assessment levied by the association and that results from a loss to property that is owned collectively by all association members. Loss Assessment coverage (under Incidental Liability Coverages) applies when the insured’s financial obligation arises out of an assessment levied by the association and that results from “bodily injury” or “property damage” to which Coverage L applies.

Association Deductible coverage applies when a master policy deductible is charged against the insured during the policy period and results from a direct loss to property that:

• would be eligible for coverage under the Homeowners policy if it were owned by an insured or was an insured’s insurance responsibility;

• is covered under the association’s master policy; and • is caused by a peril insured against, other than earthquake, that is covered by the Homeowners policy.

GUNS AND GUN ACCESSORIES - HIGHER LIMIT

The policy limits coverage for guns and gun accessories to $2,500 for loss by theft, misplacing or losing. Coverage may be increased as needed.

HORSES

UNDERWRITING RULES FOR RECREATIONAL VEHICLES & WATERCRAFT 1. Coverage may not be bound for collision coverage on snowmobiles. A visual inspection and photo that verify

no existing damage are required before collision coverage may be bound.

2. Physical damage coverage must be for 100% of present value. Liability and medical payments must be

concurrent with Homeowners Policy to which attached.

3. Coverage can be included under the Homeowners for either liability or physical damage. It is not mandatory to

include both as long as the applicant fully understands the coverage he/she has purchased.

4. Snowmobiles, ATV’s, personal watercraft, golf carts, boats and motors are not acceptable:

a. If licensed for road use (Does not apply to CF-1849).

b. If used for business purposes.

c. If used for racing, competition or similar contest purposes or any stunting activity.

d. If rented to others.

e. If the engine or gear ratio has been modified for speed or acceleration purposes.

f. If there are 2 or more chargeable accidents or traffic violations in the past 3 years or if there have been any

accidents or violations when written on a Tenant's Contents policy.

g. Where there has been a conviction for driving under the influence, open bottle violation, careless or reckless

driving, hit and run, or leaving the scene of an accident in the past 3 years.

h. Where the principal operator is under 18 years old.

5. Dirt bikes, mini bikes, mopeds, cabin cruisers, yachts, inboards or houseboats are not acceptable.

6. Submit any other type of recreational vehicles to Home Office before binding.

7. Submit the drivers license number and the date of birth of all operators.
