¨ Select How you are applying: Common App/ Directly to the institution/ Other

Do not leave the selection “I don’t know”.

¨ Request transcripts & letters through Naviance if and as your schools require them.

If you have Common App schools:

¨ Match the two accounts.

¨ Email your counselor to notify them of your deadlines.

¨ Send your counselor your brag sheet.

All students:

¨ If you see a red stamp icon by a school in your Applying To list, provide a stamped envelope addressed to the college so we can mail your transcript.

¨ Read the statement below and sign.

By signing this form, I give permission to Shorewood High School to provide colleges with requested records. I authorize school officials to communicate with the above schools in the event that there are any significant changes between the time of recommendation and graduation. I hereby authorize the release of all requested student records and understand that my student records may contain any disciplinary records.
