En la insuficiencia renal (especialmente en el
síndrome nefrótico), la antitrombina se pierde
en la orina, lo cual lleva a una mayor
actividad del Factor II y del Factor X, y a una
marcada propensión a la trombosis.

Cardiac Ejection Fraction (EF) is a good index of
left ventricular dysfunction and low EF

Among patients with reduced LVEF who
were in sinus rhythm, there was no
significant overall difference in the primary
outcome between treatment with warfarin
and treatment with aspirin. A reduced risk
of ischemic stroke with warfarin was offset by
an increased risk of major hemorrhage. The
choice between warfarin and aspirin should be

The selection of an anticoagulant agent (warfarin, dabigatran,
apixaban, or rivaroxaban) for permanent/persistent/paroxysmal
AF should be individualized on the basis of risk factors, cost,
tolerability, patient preference, potential for drug interactions,
and other clinical characteristics, including time in the
international normalized rate therapeutic ration if the patient has
been taking warfarin.
Chronic anticoagulation is reasonable for patients with chronic
HF who have permanent/persistent/paroxysmal AF but are
without an additional risk factor for cardioembolic stroke (in the
absence of contraindications to anticoagulation).

Pharmacological Treatment for
Stage C HFrEF (cont.)
Anticoagulation is not recommended in patients
with chronic HFrEF without AF, a prior
thromboembolic event, or a cardioembolic source.
Statins are not beneficial as adjunctive therapy when prescribed
solely for the diagnosis of HF in the absence of other indications
for their use.
Omega-3 polyunsaturated fatty acid (PUFA) supplementation is
reasonable to use as adjunctive therapy in patients with NYHA
class II-IV symptoms and HFrEF or HFpEF, unless
contraindicated, to reduce mortality and cardiovascular
hospitalizations.

Heparinas de bajo peso molecular (HBPM)
Tienen algunas ventajas importantes:
1) la posibilidad de su administración en una sola

HNF, especialmente en los enfermos de mayor

Heparina sódica
Puede utilizarse como anticoagulante en
todos los pacientes, independientemente de
la estrategia elegida (clase: I/NE:A)2.

estrategia farmacológica es preferible
enoxaparina a heparinaNa52 (clase: IIa/NE:B)1,2.
Enoxaparina:dosisde1mg/kg por vıa
subcutánea (s.c.)cada 12 h.
En caso de insuficiencia renal ajustar dosis
según ClCr.
En caso de ClCr inferior a 30ml/min, esta´
contraindicado su empleo.
Enpersonas de edad superiora 75 años se
deben utilizar dosis menores(se recomienda
0,75mg/kg).

Si se elige la estrategia intervencionista , puede
elegirse como tratamiento anticoagulante inicial previo
a intervencionismo coronario como monoterapia2

Si tras el cateterismo se indica angioplastia o stent,
puede usarse en el momento de realizar la
intervencion, en vez de IIb/IIIa, si esta medicación no
se había prescrito inicialmente (clase: IIa/NE:B).
Si tras el cateterismo el paciente debe recibir
tratamiento quirúrgico , debe suspenderse 3 h antes
de la cirugía.

Conclusión
Aunque tanto la heparina Na como la

una evidencia clase: IIa/NE:A, excepto
si se ha de realizar cirugía de
revascularización en las primeras 24
hs y en pacientes con insuficiencia
renal grave.

Patients received one of the standard lytics
(tenecteplase, tissue plasminogen activator,
reteplase, or streptokinase) per the treating
physician’s choice. All patients received aspirin.
In a double-blind, double-dummy fashion,

La dosis se ajusta al peso del paciente, de 80 U/kg como bolo
inicial, y continúa con una infusión a razón de 18 U/kg/h. La
dosis debe modificarse de acuerdo con el tiempo parcial de
tromboplastina (TPT); el objetivo es mantener entre 1.5 y 2.5
veces el control. El monitoreodel TPT debe realizarse cada 4-
6 horas,

estrategia contra reloj para su diagnóstico, caracterización
y tratamiento resulte apropiadamente desarrollada. Casos

aspirin (75 mg to 325 mg once daily) rather than oral
anticoagulation (Grade 2B) or combination therapy with aspirin

Remarks: Patients who place an exceptionally high value on stroke reduction and
a low value on avoiding bleeding and the burden associated with antithrombotic
therapy are likely to choose antithrombotic therapy rather than no antithrombotic
therapy.

reasons other than concerns about major bleeding), we suggest

rivaroxaban experienced increases in
bleeding compared with those receiving
warfarin. These included patients aged
≥ 75 years, and those with prior MI or PAD.
It is important that these factors are
considered when selecting the optimal
therapy for individual patients, but not at the
