NAVIGATE FROM HOSPITAL TO COMMUNITY SETTINGS

Temiskaming Hospital and CMHA-CT have partnered to implement an Integrated Mental Health and Addictions Client Navigator role, located within the hospital . The position commenced December 2, 2019 with a focus on improving transitions of care for patients with mental health and addictions from hospital to the community setting .

Key roles and responsibilities of the client navigator position:

• Support for patients with mental health and addiction conditions presenting in the emergency department (ED) to redirect and connect (where appropriate) to community-based resources/supports as an alternative to hospital admission

• Support assessment and discharge planning for admitted patients thereby improving transitions of care

• Develop common care/communication/intervention plans for high risk/high use individuals

• Provide follow-up phone calls for patients discharged from hospital and high-risk patients presenting to ED

• Promote community-based services and access• Further Rapid Access to Addiction Medicine

strategies and pathways for those most in need of rapid access to addiction medicine both at the hospital and in the community

• Make recommendations and assist in defining/developing proactive joint response with OPP as a second phase

The system navigator will focus on the most vulnerable, high risk and repeat ED users . It’s hoped this approach and partnership will reduce the need for ED visits while supporting a more community-based recovery model .

BRINGS SECURE VIRTUAL SERVICES TO CLIENTS

In a blink of an eye, our business world changed dramatically . Information technology (IT) professionals across the world are busier than ever as they support business continuity efforts during a time of physical isolation and social distancing . For CMHA-CT, this means helping people with the tools and secure solutions to enable communication with peers, clients and other professionals .

From the onset of COVID-19 and with little time for project planning, we pro-actively implemented solutions that helped employees deliver needed secure services in the most user-friendly way . The IT team immediately deployed the necessary equipment to 130 employees to enable them to work

remotely . Rolling out MS Teams also allowed people to meet virtually through chat, audio and video communication, providing business in a not-so-usual but effective way . Additionally, we forced multi factor authentication for remote desktop service and Office 365 to secure our information when working remotely .

We are now part of the CMHA IT Community of Practice which is private to the CMHA IT leads in Ontario . Through MS Team, we share discussions and files to help support the work of the CMHAs across the province . Our work is being shared as a valuable resource for other CMHAs for which we are grateful and proud . Employees have worked hard, been flexible and have adapted to many changes with technology .

Hats off to everyone and thank you for helping to make our IT experience as pleasant as possible .

This year, CMHA-CT began the multi-year process of organizational accreditation through Accreditation Canada . Accreditation is an evaluation of an organization against a set of standard requirements including leadership, client safety, human resources, information technology, quality improvement, infection prevention, medication management, service delivery, and more . It is a challenging and comprehensive process that requires the involvement of many internal contributors . Ultimately, accreditation will ensure that we continue to deliver high quality mental health and addiction services to our communities .

To facilitate accreditation, we created the Accreditation Steering Committee . This regionally represented group, composed of staff members with a variety of clinical and non-clinical roles, is responsible for the management and advancement of the accreditation process . Accreditation Canada has commended our strategy of collaboration and has suggested that our method may be included in future recommendations . Despite the COVID-19 pandemic, we are confident that we will be prepared for our on-site accreditation survey in March 2021 .

Mental Health Week (MHW) is an annual awareness week that occurs during the first week of May . MHW raises awareness and helps decrease stigma about the variety and prevalence of mental health issues .

The Mental Health Week Committee (MHWC), led by frontline staff is comprised of nine individuals: five in Timmins, two in Kirkland Lake, and two in Timiskaming Shores . The turnout for MHW 2019 was 3-5 times larger than last year . This is the MHWC’s signature event, demonstrating the importance of #GetLoud .

This year’s highlights included:

• Illuminating Hollinger Headframe in green to support the CMHA Shine Green campaign to raise awareness about mental health

• The successful Walk for Wellness at Gillies Lake, Timmins Ontario supported by our amazing multiple community partners: Tai Chi Society, Project Love, Timmins Native Friendship Centre, Misiway Community Health Centre, NEOFACS and Porcupine Health Unit

• Holding a series of mindfulness activity workshops for clientele at Intrepid Place and a Smoothie/Health Bar hosted by the FUN team

• Performing random acts of kindness by staff• Recognizing Diversity Living, a supportive living

environment, for their work in promoting mental health

• Free public film night sponsored by Timiskaming Shores with street teams providing promotional items and information

• Free cardio dance class and public swim sponsored by the Kirkland Lake group and a local fitness studio, plus Random Acts of Kindness with free promo bags containing $5 Tim Hortons card, contact information and other promotional items

Overall, we saw an increase in community partner participation with our Walk for Wellness, expanded media coverage, and garnered significant positive results from our staff feedback which topped a 6 .83 average response out of seven . Well done CMHA-CT!

PERSEVERE DESPITE CHALLENGES

Referred through the primary care clinic, I worked with a client who experienced extensive cognitive impairment due to an acquired brain injury . Previously, they were fully independent, gainfully employed and living alone . Following the injury, they were consigned to retirement home living . I was asked to help optimize the client's safety in their apartment, but upon meeting the client, it became clear they were struggling with a loss of meaningful activity and independence . In particular, they relied on friends for transportation and their goal was to use public transit to increase independence and community engagement .

This turned out to be a struggle . The client's memory was severely impaired and retaining information from one appointment to the next was difficult . Medical

complications delayed the process, and throughout the experience, missed appointments and setbacks were commonplace . Together, the client and I persevered . Neither of us gave up despite the barriers to progress . Eventually we started to see success . At first, we met at the apartment and I travelled with them . Eventually, we met at the bus station and later at the mall (a trip which had many complicated bus numbers and a transfer) . Months later, the client successfully navigated their first medical appointment alone . The courage and perseverance resulted in a greater level of independence initially thought impossible . Although it is a small step, claiming the skill of public transit has allowed this individual to reclaim some of their life again .

That's a good news story we can all get behind!

This year, the CMHA-CT began developing the 2020-2024 Strategic Plan . A strategic plan is a roadmap that helps us make decisions and points us in the right direction . It outlines our vision for the future and defines us as an organization .

One of the key components in the development of a strategic plan is the engagement and consultation with members of our community . We gathered diverse feedback from our board of directors, staff, community partners, and community members . These engagement

opportunities were conducted through an equity lens, deliberately taking into consideration the perspectives of especially vulnerable populations such as members of Indigenous, LGBT2SQ+, Francophone, and seniors’ communities .

Although the COVID-19 pandemic has put a slight delay in our consultations, we continue to look for creative ways to engage community members so all voices can be included . As a result, the strategic direction of our new plan will be representative, meaningful, and deliberate .

I live with serious and persistent mental health issues . It’s been a journey to get to where I am today, from living in a recovery home to working with people who are in our housing program, and the agency as a whole . I use my story and the challenges I have overcome to help encourage my clients . I relate and connect with clients on different levels and empower my clients to take control over their own lives and not let their illness take control of them . Letting them know that recovery is possible .

As a peer support, I’m able to be involved in different ways by sharing my experience:

• Providing one-on-one support to clients meeting them where they are at . This includes taking calls, doing check-ins, linking appropriate services, collaborating with community agencies, partaking in team meetings and attending to crisis when needed .

• Developing and leading a peer chat group based on topics that help clients build life skills such as self-esteem, setting boundaries, and mindfulness . I facilitate Wellness Recovery Action Plan (WRAP) sessions . WRAP is an eight-week program that supports people so they can keep themselves well . I developed an alternate reality group that helps individuals understand their diagnosis and symptoms and develop coping mechanisms .

Facilitating a family information support group,I am able to share my personal experiences with mental illness, provide information on mental health issues, share community supports, and advise ways to work through challenges to better understand and help their loved ones .

I love my job at CMHA-CT . Some days can be tough, but I get great satisfaction in helping others . I would like to thank CMHA for not only giving me the opportunity to do what I do but everyone in the agency for their support .
