Covid-19 context

Annual Report retrospectively covers the period from April to

March each year so final submission does not focus on

Covid-19 pandemic in any detail.

CCG published updates each month on website as part of Board

papers - including decisions and actions taken in the management

of pandemic and to provide assurance.

CCG involved in emergency response to virus. Had to adapt quickly to a new way of living, working and commissioning healthcare in Bolton.

In due course will provide information on how pandemic will influence how we operate as a health and care system throughout 2020.

Achievements – Partnership working

• Bolton Partnership Board continues to provide integrated health and care leadership for our town

• Great progress has been made with the Strategic Commissioning Function(SCF) and the Integrated Care Partnership (ICP) over the past year

• Both have moved to more formal arrangements

Achievements – Strategic commissioning Function (SCF)

• Director of Strategic Commissioning appointed (Melissa Maguinness) and responsible for a single commissioning team on behalf of Bolton Council and Bolton CCG

• Working on a joint commissioning plan for Bolton that is aligned to the Bolton Vision

• Plan will take into account effects of Covid,including on health outcomes and inequalities as well as how technology and other innovations have been used to maintain services

Achievements – Integrated Care Partnership (ICP)

• Managing Director appointed (Rachel Tanner) and senior leadership team now being formed to oversee integrated services across community health, adult social care, primary care development and some mental health services

• Already working together in multi-disciplinary teams and pulled together even more during Covid to provide support to care homes and operate integrated admission avoidance and discharge pathway

• Partnership involves Primary Care Networks, voluntary & community sector, housing, as well as the ‘parent organisations’ of Bolton Council, Bolton CCG, Bolton FT, Greater Manchester Mental Health

Achievements – Primary Care Networks (PCNs)

• Established in July 2019 - Bolton has nine PCNs, aligned to the neighbourhoods already identified as part of our plans for integrating health and care

• Neighbourhood working critical to supporting resilience of general practice and delivering more joined-up, proactive support to local communities.

• Each network has a Clinical Director who is expected to be a direct link between general practice and the wider integrated system.

• Bolton’s PCNs are: Westhoughton; Breightmet and Little Lever; Halliwell; Turton; Farnworth and Kearsley; Horwich; Rumworth; Bolton Central; Chorley Roads.

Achievements Patient and Public Involvement

Be Kind to My Mind website (designed by young people,for young people) has been visited 2,000 times since going live in April 2019

Extended Primary Care services – Winter awareness-raising campaign of evening and weekend appointments: ‘If you’re not offered it, ask for it!’

Flo the Flu Fairy – helping families understand the importance of the flu nasal spray for 2 & 3 year olds with targeted resource packs delivered to nurseries and childminders

Achievements – Improvements to IT systems

• Four separate networks reduced to one, covering the CCG, FT and all 49 GP practices

• Telephone system replaced and integrated with FT – free calls to each other!

• Measures support integrated working –invaluable during Covid

Achievements – Financial Responsibilities

• Operated within approved revenue resource allocation and delivered a breakeven position

• Delivered QIPP of £9.2m

• Supported our suppliers by paying 95% of invoices within 30 days

• Operated within the CCG Running Cost Allowance of £6.6m

Our financial challenge

We need to protect the NHS & Social Care for everyone who needs it… but budgets are already under pressure this year.

• Pressures caused by Covid-19 and the additional work required to restart and recover• Increasing cost of workforce• Meeting the needs of most complex cases• Costs of investing in new treatments

We must spend our Bolton £ wisely by working as one system of health and care in Bolton

Our health outcomes challenge

The Covid-19 pandemic has widened inequalities in health nationally, and in Bolton.With the direct impacts of Covid-19, the delays caused to treatment and the health impacts expected to be caused by isolation in lockdown, growing unemployment, etc, we now have an even greater challenge to:

• Improve life expectancy• Reduce health inequalities• Provide an enhanced quality of care.• Help people to get the support they need, wherever possible within their community• Deliver a sustainable system for the future
