Introduction
Cochrane Germany (CG) is the representative of the international Cochrane
Collaboration (CC) in Germany. Furthermore, it is the reference centre for the
German speaking part of Switzerland, for Austria, Liechtenstein, the Czech Republic,
Slovakia and Hungary (for branches s. page 19). The CC is an international not-for-
profit organization whose “vision is a world of improved health where decisions about
health and health care are informed by high-quality, relevant and up-to-date
synthesized research evidence” (www.cochrane.org). To achieve this, more than
37,000 people from over 130 countries work together in preparing, updating and
promoting systematic reviews about health care interventions. To date, over 9,200
Cochrane Reviews are published in the Cochrane Library
(www.cochranelibrary.com). The national centres are tasked to deliver the following
‘core functions’ in their countries:
1. To promote and represent The Cochrane Collaboration.
2. To serve as a source of information about The Cochrane Collaboration. 3. To provide or facilitate training and support for review authors, editors,
handsearchers and other contributors to The Cochrane Collaboration.
4. To support regional editorial bases of Review Groups, Methods Groups and Fields by: assistance to find funding and mediating conflicts, either between Cochrane
entities or between individuals and entities.
5. To contribute to improving the quality of Cochrane reviews by performing,
supporting or promoting methodological research. 6. To promote accessibility to The Cochrane Library to healthcare professionals,
patients and others, e.g. by pursuing national subscriptions and translations
where necessary. 7. To handsearch general healthcare journals in the linguistic area of the Centre and
to submit the search results to the Collaboration’s trial database. *

In addition to the core functions as a centre of the CC, the German centre acts as a
national ‘hub’ for the implementation of international scientific and strategic
developments related to knowledge transfer, the concept of evidence-based
medicine and the role systematic reviews can play for both. It supports the
implementation of methods related to the concepts of evidence-based health
research and the grading of evidence according to the GRADE approach. Another
scientific focus is the investigation of the extent of dissemination bias in medical
research and how it affects the reliability of the results of research syntheses.

CG was formally registered within the Cochrane Collaboration in 1997. Up to 2004 it
received funding from the German Federal Ministry of Education and Research,
which was complemented by the Medical Center of the University of Freiburg and the
Medical Faculty of the University of Freiburg. Since January 2005 the German Federal
Ministry of Health has sponsored the CG and will continue until the end of 2016.
In 2014, the Medical Center of the University of Freiburg established the Center as a
stand-alone department (Centre for Evidence in Medicine /Cochrane Germany) and
committed to funding Cochrane Germany with a funding perspective for the next five
years. The possibility of establishing a ´German Cochrane Institute` with a full
professorship for systematic reviews is being explored.
A discussion and a parliamentary hearing about the importance of the Cochrane
entities in Germany were stimulated by a comprehensive review of Cochrane
Germany. The review was commissioned by the German Ministry of Health and
conducted by PROGNOS AG1. The final report of this review contains structured
planning methods used to evaluate the strengths, weaknesses, opportunities and
threats involved (SWOT Analysis) for CG. As a result there are ongoing negotiations
about opportunities for an institutional funding of CG by the German Federal
government.

Funding
In 2015 Cochrane Germany received institutional funding from the University Medical
Center Freiburg and the Medical Faculty of the University Freiburg, complemented by funds from the Baden-Württemberg Ministry of of Science, Research and Arts.
Additionally Cochrane Germany raised third party research funds (project funds) to
the amount of Euros 477.000.
CG objectives in 2015
The objectives of CG in 2015 were:
1. Fulfilling the Core Functions of a Cochrane Centre
2. Supporting the activities of the GRADE Working Group and fulfilling the
function of a GRADE Centre in Germany
3. Recruiting and supporting reviewers / review groups
4. Participating in systematic review activities, evidence assessments and HTA
reports
5. Developing and conducting education and training, workshops and courses
related to evidence based medicine, systematic reviews and the GRADE
methodology
6. Improving the quality of publications of clinical trials in German journals by
promoting publication guidelines
7. Information for users of evidence for different target groups (science
journalists, health system professionals, consumers and the public)
8. Maintaining information structures
9. Searching German printed publications for unpublished clinical trials
(Handsearching)
10. Consolidation of the funding for CG
The objectives were accomplished by the following projects, workshops and events, publications and presentations, and further related activities:
Projects in 2015 (selection)
In 2015 CG conducted a total of 6 funded projects with an overall funding volume of

ACQuire – funded by the Federal Ministry of Health

This project (German title: Aktivitäten des Deutschen Cochrane Zentrums mit Fokus
auf der Stärkung der Methoden-Kompetenz in der Leitlinienerstellung sowie in den
Gesundheitsfachberufen zur Verbesserung der Qualität und Evidenzbasierung in der
Gesundheitsversorgung) aims to strengthen the methodical expertise in the
development of clinical guidelines and in the health professions to improve the
quality and evidence base in the healthcare provision.
The project started in January 2015 and ended in December 2015.
DECIDE – funded by the European Commission
CG is a partner in the international consortium of the EU-project DECIDE (Developing
and Evaluating Communication Strategies to Support Informed Decisions and
Practice Based on Evidence) which aims to improve the dissemination of evidence-
based recommendations by building on the work of the GRADE Working Group to
develop and evaluate methods that address the targeted dissemination of
guidelines. The project ran from June 2011 until the end of 2015.
The DECIDE consortium which is a composition of members of the GRADE Working
Group, will further develop the GRADE approach to ensure effective dissemination of
evidence-based recommendations targeted at the key stakeholders (healthcare
professionals; policymakers and managers; patients and the general public) who
determine activities/decisions in clinical practice. The consortium adapts
stakeholder input from advisory groups, consultations and user testing. This is done
across a wide range of health systems in Europe. The targeted dissemination
strategies that are developed, will be evaluated in randomised trials, refined and
used and evaluated with real guidelines, developed by the DECIDE partners and
other guideline developers, that the project partners support.

INKA – funded by the German Ministry of Education and Research
This project (German title: “Ein Forschungsprojekt über die Veränderung der
Informationsqualität in der Kommunikationskaskade (INKA) von der medizinischen
Fachpublikation bis zur Rezeption der Medizinberichterstattung durch Ärzte und
Laien”) analyses scientific information on its way from the medical abstract to mass
media contributions and is funded from 2013 until the end of 2016. While the project
partner at the Technical University Dortmund analyses the loss of correct
information from initial scientific reports down to the diffusion into public media, CG
investigates the information behavior and resources of German Physicians in an
exploratory study.

KSA Thalassemia Guideline – funded by the Saudi Center for Evidence Based Health
Care (EBHC), Ministry of Health, Saudi Arabia
This project aims to develop a national, evidence-based guideline based on
systematic reviews for the treatment of Saudi Arabian patients with thalassemia in
cooperation with the Saudi Center for Evidence Based Health Care (Saudi Arabia),
the McMaster University (Canada) and local experts.
The project started in September 2014 and ended in May 2015.
RARE-Bestpractice – funded by the European Commission
CG is a partner in an international consortium which received EU funding for the
project “Platform for sharing best practices for management of rare diseases: RARE-
Bestpractices” in 2012. The project started in January 2013 and will last until
December 2016. The platform will be used for a comprehensive understanding of the
renewed challenges which are generated by actions and incentives of the European
Commission. It intends to address the following critical questions:
A) How to take better advantage of scientific progress in biomedical research in
ways to translate research results into tangible benefits for rare-disease
patients in a timely manner and to offer high quality information to these
patients and health professionals?
B) How to promote and support a consistent level of healthcare services for rare
disease patients, thus implementing the EU Directive on the application of
patients’ rights in cross-border healthcare?

As part of its 2020 Strategy, Cochrane International introduced a rebrand with new
Cochrane logo, templates for all Cochrane entities around the world and a new look
of Cochrane.org. A set of detailed brand guidelines set out our new brand: who we
are and how we express our identity in the way we look and sound.
At the same time, names of Cochrane Centres were unified and the German Cochrane
Centre was renamed to Cochrane Germany.

The Cochrane strapline is the core idea that lies at the heart of everything we do. It
expresses the essential reason why Cochrane exists.
By June 2015, we moved our website to the new look and at the same time
streamlined the contents and improved functionality. Cochrane International
introduced their new website Cochrane.org for their main target users, which now
also provides a second, more comprehensive Community website for Cochrane
insiders and staff. By June, we adapted our website to the corporate identity design
and layout as proposed by Cochrane Central. This entailed streamlined contents and
functionality. Furthermore, Cochrane Central introduced their new website
cochrane.org which is designed to address their target audiences.

Joerg Meerpohl established and organized a basic review course for authors together
with the Austrian and Swiss Cochrane branches. This course was hosted for the first

Contribution to the “Cochrane Library”: The Cochrane
Central Register of Controlled Trials
Handsearching Project
Handsearching means searching manually for trial publications in printed scientific
medical journals. Each Cochrane Centre searches medical journals, published in
national languages, to identify these reports which otherwise would not be
accessible, because the printed journals are not digitally available or not listed e.g. in
the great biomedical literature databases. This effort increases our knowledge base
of data from clinical trials and tries to overcome barriers leading to publication bias
(by source, medium or language) in systematic reviews. The results of this process
are submitted to the Cochrane Central Register of Controlled Trials (CENTRAL) which
meanwhile contains over 920,000 citations of controlled clinical trials – far more than
accessible via PubMed or EMBASE (Excerpta Medica Database). In the last 15 years
CG identified more than 20.000 study publications, of which nearly 50 % are not
listed in MEDLINE and therefore not easily detectable for scientific research.

Handsearching has been on hold in 2013 and 2014 due to lack of resources and has
been taken up again in 2015. One or two journals from five medical disciplines (allergology, sport medicine,
nursing science, paediatrics and ophthalmology) were chosen for the handsearching

project. These journals are relevant for German physicians and are not indexed in
MEDLINE.
Table 1 shows the number of original articles and abstracts (separated into CCTs and
RCTs) which are not listed in MEDLINE and which were found by handsearching in the five selected medical journals.

EMBASE Search Project
In 2015 CG contributed to the EMBASE-Screening-Project of the Cochrane Collaboration. EMBASE is a database that covers worldwide literature, especially
from Europe, in the field of human medicine and related fields with special emphasis
given to drug research, pharmacology, pharmacy, pharmacoeconomics, toxicology, basic biological research, health policy and management, public, occupational and
environmental health, and substance dependence and abuse.
The project's purpose is to identify reports of RCTs and quasi-RCTs from EMBASE for publication in CENTRAL. At this time (as of January 2016), 15 % of all citations in
CENTRAL are from EMBASE (compared to 60 % from MEDLINE, 25 % from other
databases). In 2015 CG has screened in total 1,040 studies.

Published protocols and reviews
The efforts of CG, the German Review Groups (see below) and the Germany-based
Cochrane working groups (http://www.cochrane.de/de/cochrane-deutschland) as
well as the contributing people in Germany lead to the fact that the number of
corresponding authors in Germany, contributing to Cochrane Reviews, increased
from 542 in 2010, to 691 in 2013, and to 932 in 2016 (as of April 2016) (Search filter:
Country: Germany; Reference centre: CG; Role: Author). Therefore the number of
corresponding authors in Germany has doubled since 2010. That fact highlights the
increased support of German Cochrane personnel in the conduction of reviews.
At the same point of time CG was given as reference centre by 2,368 persons (Search
filter: Reference centre: CG). 588 (2013: 532) come from Switzerland, 213 (2013: 186)
from Austria, 34 (2013: 31) from the Czech Republic 7 (2013: 0) from Slovakia and 37
(2013: 35) from Hungary, 1 (2013: 1) from Liechtenstein (Source: ARCHIE, Cochrane
author’s database, search from 21.04.2016).2
Figure 1 shows the number of reviews and protocols with Germany as the country of origin. Until April 2016 there exist 165 reviews and protocols from Germany.

The production of reviews with corresponding authors in Germany shows a
continuous increase from 1998 until the beginning of 2016. Figure 2 shows the
number of published systematic reviews and protocols from Germany, Switzerland,
Austria, Liechtenstein, Hungary and the Czech Republic in 2015. From the last report

Austria, 108 to 110 in Switzerland. No changes in comparison to the last year can be
seen in the number of reviews and protocols for Liechtenstein, Hungary, Slovakia
and the Czech Republic.

The CHMG deals with the evaluation of the diagnosis and treatment of adults with
haematological diseases. The main focus is on leukaemia and lymphoma and the
care of those suffering from other haematological diseases like aplastic anaemia is
also included in the wider scope of the group. In addition, the group evaluates
interventions affecting the haematological cell system like stem cell transplantation
and haemotopoetic growth factors. Finally, supportive care and the management of
haematological diseases also are tasks of the Cochrane Haematological Malignancies
Group.

Since 2000, the CMED has been preparing systematic reviews on the benefits and
harms of healthcare interventions for metabolic and endocrine disorders,
particularly diabetes mellitus and obesity. The CMED's main area of work is the
evaluation of the effects of health care interventions relevant to the prevention,
treatment or management, and rehabilitation of metabolic, nutritional and
endocrine disorders. The CMED expanded its scope to systematic reviews of
diagnostic test accuracy within the same topic areas. The CMED primarily focuses on
systematic reviews which evaluate patient-orientated outcomes measures. Authors
who wish to analyse surrogate outcomes like laboratory parameters may do so but
always have to investigate a standard set of patient-orientated endpoints. Exclusive
evaluation of surrogate indicators only will not be possible.

Austrian Cochrane Branch The Austrian Cochrane Branch opened in December 2010 as part of the Department
for Evidence based Medicine and Clinical Epidemiology of the Danube University
Krems. Main tasks are:
It provides the basis for evidence-based decisions in the Austrian health
system.
It is supported by Austrian scientists, who provide the means to create
science-based and independent medical information in the form of Cochrane
Reviews that are available to the Austrian health care and the public.
It promotes the publication of the Cochrane Collaboration and its importance
in the public, aiming at transferring research findings into health care and thereby improving patient information.
It organises workshops for those interested, such as (future, potential)
authors of Cochrane reviews as well as employees of the Cochrane
Collaboration.
It is the first point of contact for people interested as well as for employees of
the Cochrane Collaboration in Austria.
It functions as a contact for employees of the Cochrane Collaboration
worldwide, and thus facilitates the communication within the Cochrane Collaboration.
The Austrian Cochrane Branch is funded by a grant from the Lower Austrian Health and Social Fund (NÖGUS) and supported by the Danube University Krems.

Swiss Cochrane Branch The Swiss Cochrane Centre is the branch that is supported by the German, the French, and the Italian Cochrane centres. It is a national network which is located in

Hungarian Cochrane Branch The negotiations of establishing a Hungarian Cochrane Branch resulted successfully in the official opening of the Branch in October 2014 at the Medical Center of the
University of Pécs. Director of the branch is Prof. Dr. Tamás Decsi and the scientific
coordinator is Dr. Szimonetta Lohner. The goals of the branch are:
to provide information about Cochrane and participate in its international
activities
to train and build capacity for the preparation of systematic reviews, including the preparation and updating of high-quality Cochrane systematic
reviews
to provide language-specific services: e.g. extracting data from Hungarian
papers for Cochrane reviews and translating review summaries into
Hungarian
to disseminate the results of Cochrane reviews in Hungary and to support the
use of Cochrane reviews by specific user groups
to facilitate knowledge transfer of high quality healthcare evidence through formal and informal partnerships with relevant stakeholders in Hungary
to promote access to the Cochrane Library and to Cochrane systematic
