JANUARY
January 1, 1104 67th St, Windsor
Heights: Engine 320 and Ambulance 323
assisted Windsor Heights on a structure
fire. Team 320 assisted with ventilation
needs from a down comforter that had
caught fire in a dryer.
January 12, 8449 Harbach Blvd: Clive
Fire was dispatched to Crest Manor
Apartments for a structure fire. Crews
arrived to find a cooking fire that was
had been extinguished by an occupant.
She sustained minor burns to her hand/
arm, face and lower leg when she
removed a burning pan from the stove.
EMS crews transported her for
evaluation and treatment. There was no
property damage other than burned
linoleum flooring.
January 19, 8327 Harbach Blvd:
Dispatched to an apartment fire. Upon
arrival, smoke was visible from the front
entryway. Team 320 entered to
investigate and extinguish a fire inside
the oven of a basement apartment. The
entire building was charged with thick
black smoke. Fire crews assisted with
ventilation.

for the members who completed their

Work on Potential Clive/Windsor Heights
Merger: Analysis and conceptual
planning continued, including logistical
meetings with the Iowa Bureau of EMS,
Insurance Services Offices (ISO),
WestCom, and the Medical Director.
Annual Banquet: The annual Clive
Firefighters Association Banquet was
held on January 17th at the Elk’s Lodge.
The evening included: Of the Year
Awards, Years of Service Awards, and the
annual video.
FEBRUARY
February 3, 14811 Brookview Dr.,
Urbandale: The Clive Fire Department
was dispatched with two engines and a
duty officer for mutual aid to Urbandale
on a working residential fire. This
structure was a 6,000 sf single family
home with substantial smoke on arrival.
The fire vented through the roof near the
front of the home. Engine 320 and
engine 220 were both assigned to fire
attack and overhaul. The Duty Officer
assisted Command with visual
observations of remote sides of
the structure.
February 13, 10201 University Avenue:
The Fire Department assists the Police
Department a few times each year with
access to buildings for their investigation
needs. On February 13, we provided
ladder access to the roof of the 10201
University Avenue shopping center for
CPD access.
Recognition of
Service:

Paramedic
Brandon Gray
resigned following
seven years of
service to the City of Clive. He was
presented a recognition plaque for
his service.
MARCH
March 5, 13850 Lake Shore Dr:
Brookview Dr., Urbandale: A carbon
monoxide alarm had activated.
Investigation revealed elevated CO levels
in a basement mechanical room. Crews
assisted with natural ventilation and
ensuring the occupants contacted an
HVAC company for immediate response.
March 5, 2071 NW 86th St.: An initial
report of a fire alarm activation was
upgraded to smoke filling a hallway.
Crews arrived to find a
cooking fire in one
apartment that was
extinguished prior to
our arrival. Crews
assisted with ventilating smoke. No
occupants were injured. Damage was
estimated at $250.
March 15-18: Dry conditions in March
made conditions favorable for grass
fires. Crews responded to three grass
fires during this week.

the Female Firefighter
Division in the Downtown
Des Moines Fight for Air
Climb held on March 22.
Recognition of Service:

Christina McFarland
resigned following six
years of service to the
City of Clive. She was
presented a recognition plaque for
her service.
Facility Needs Focus Group Meetings: In February and
March. Shive Hattery conducted two rounds of focus
group meetings with Fire
Department staff to
evaluate space needs of
different program areas.
Three separate meetings
were conducted to provide
various opportunities for
staff to participate.

APRIL
April 9, 2250 Woodlands Parkway
(Casey’s): An overheated refrigeration
compressor was the cause of a smoke
scare. Fire crews were able to quickly
isolate the smoke to the compressor and
refer the problem to maintenance staff.
April 24, 3417 Melanie, Urbandale: Clive
assisted with a structure fire. The
structure was a two-story home. Fire had
extended from a chimney chase in the
walls and attic, with flames through the
roof upon fire department arrival. Clive
assisted with rapid intervention team
stand-by and salvage.
Indian Hills Career Day:
Fire Department staff
assisted with the Indian
Hills Junior High Career
Day on April 1.
Clive Firefighters Association Annual
Easter Egg Hunt:
The 48th Annual
Easter Hunt was
held at Linnan Park
on April 4.
Assistant Chief Dave Lyons Retirement:
Assistant Chief Dave Lyons was
recognized for 25 years of service to the
City of Clive at a reception held on April
16. Dave served in many leadership
capacities, including Lieutenant, Captain,
and Assistant Chief.
Continued Work on Potential Clive/
Windsor Heights Merger: Both City
Councils approved a Memorandum of
Understanding agreeing to “collaborate
on a comprehensive investigation and
analysis to enter into a possible
consolidation of the two cities’ fire &
EMS departments.” A timeline of 180
days was established to develop a final
agreement for consideration.
MAY
May 1, 1860 NW 80th Street, Windsor
Heights: Crews arrived to fire coming
from a cut in the concrete curb where a
cutting saw hit a
natural gas line.
A spark from the
saw ignited the
natural gas.
Flames were
extending
approximately five feet high with an
audible hissing sound.
May 3, 14461 Summit Drive: Fire started
near the exterior chimney chase of a two-
story residence. Engine 220 extinguished
the fire with damage limited to the first
floor level of the chimney chase. The
cause of the fire is undetermined,
however occupants had been using a
Chiminea earlier in the evening. Damage
was estimated at $6,000.
May 3, 14407 Forest Avenue (Mi
Mexico): Kitchen cooking fire in . The
Ansul system extinguished the fire
(saving the building). The front of the
hood was blackened and the plastic lens
on the light fixtures were melted.
Damage was estimated at $1,000.
May 5, 12000 Hickman Road (Menards):
Responded to a report of a semi on fire
at Menards. Crews arrived to find staff
members applying dry-chemical
extinguishers to the engine
compartment. Engine 220 applied water
to cool the compartment to prevent any
flare-ups. Damage estimated at $6,000.
May 11, 8434 Harbach Blvd: Paged out
for an investigation of an explosion then
upgraded to a structure fire. Arrived to a
multi-family structure with occupants
reporting light smoke and an electrical
burning odor. In an upstairs bathroom
there was smoke damage around an
exhaust fan and a burned rug on the
floor. Ladder 325 removed the exhaust
fan and about two cubic feet of material.
Damage was estimated at $500.
Update to Fire Code: In May, Clive City
Council adopted an update from the
2003 International Fire Code to the
2012 International Fire Code.
JUNE
June 5, 15723 Hawthorn Dr: Fully
engulfed attached garage with flames
extending to the
attic. Occupants
were awakened by
smoke detectors
and able to exit the
back door. No
injuries were reported. Damage
estimated at $175,000.
June 10, 8355 Alice Avenue: Smoke on
2nd floor of an apartment building. Fire
personnel forced entry and extinguished
a kitchen fire. No injuries were reported.
Damage estimated at $25,000. Cause
was unattended cooking.
June 11, 15500 Hickman Road: Two-car
“T-Bone” style accident.
Hydraulic extrication
tools were used to
remove the door and
allow the patient to be
removed. Two patients
were transported .
June 25: Record
flooding occurred in
Clive. Fire Department
responses included:
Swanson Blvd – Assist two motorists
stranded in flooded street
Drake St – Assist four persons by boat and one disabled adult by ambulance
75th St – Assist five persons by boat
2015 Mission Lifeline EMS Recognition
Award! The Clive Fire Department
achieved a Bronze Level Recognition for
achievements in cardiac related care. We
actually met the criteria for Gold Level
recognition, but our call volume is too
low (according to
their current
rules), so the
“best” we could
achieve is
Bronze. The
American Heart
Association made
a presentation at the City Council
meeting on June 25.

JULY July 4, 16400 Douglas Pkwy (Shuler Elementary): A ten year old child had his head wedged between a slide tube and second floor wall of a playground structure. Clive Police and Engine 220 extricated the child. July 6 and July 28: Significant rains caused minor flooding concerns.
July 6 – Fire Department staff assisted with contacting businesses on the south side of University Blvd to alert them of minor anticipated flooding.
July 28 – Fire Department staff assisted with monitoring North Walnut Creek and Maddox Parkway for flash flooding concerns .
Lt. Schut Assists at Miracle Burn Camp of Iowa: Lt. Josh Schut volunteered for the third year in a row, assisting with the Miracle Burn Camp of Iowa, July 11-18th.
House Fire Prop: Special recognition to Firefighter/Paramedic Joel Otte for the idea and design on a House Fire Prop! This was constructed with the assistance of other Fire Department staff. This is a great tool for public education events, station tours, and block parties. Clive Festival Slip-n-Slide: Clive Fire Department staff organized and operated the Clive Festival Slip-n-Slide.
AUGUST
August 3, 9060 Hickman Road (Billion
Auto): Vehicle fire with the engine
compartment fully involved. Fire
extended into the dash. Owner stated he
was driving eastbound on Hickman when
he noticed heavy smoke coming from the
hood and jumped the curb and drove
into Billion Auto's lot. No damage to
other vehicles or property. Damage was
estimated at $1,700.
August 9, 3524 NW 169th Circle: A small
fire had been extinguished by a
homeowner prior to fire department
arrival. The fire appeared to have started
from a small portable grill and extended
to a deck support column. The
homeowner
stated he had been cooking some
"gourmet burgers" on the LP grill. A
neighbor noticed the fire and alerted
them. Damage was estimated at $500.
August 26, 1400 Block of 100th: A car
versus utility pole
accident was reported.
Crews initially staged as
the driver had left the
scene and was being
pursued by Clive Police. Following
apprehension, Clive EMS personnel
evaluated the driver for minor injuries.
August 27, 9104 Summit Dr: Structure
fire at an occupied duplex. Significant
fire involvement on the
back of the structure
noted on arrival. Fire
appears to have started
on the exterior of the
building around a
wooden deck and
extended into the attic. A passer-by is
credited with alerting the occupants,
allowing them to safely escape. The
cause of the fire was carelessly
discarded smoking materials. Damage
was estimated at $40,000.
Special Extrication Training: Special
extrication training was held at Roy’s
Towing in West Des Moines. This offsite
location allowed a
vehicle under a
semi-trailer
practical scenario.
SEPTEMBER
September 9, 120th and Lincoln: A
vehicle was found down a steep hill in
the woods with the driver inside. Several
small trees were cut that were pinning
the driver’s door closed. The patient was
extricated and taken to the hospital.
September 14, I80 Eastbound: A single
occupant was involved in
a rollover accident. The
patient was out of the
vehicle upon FD arrival
and was transported to
the hospital with minor injuries.
September 15, 10371 Clark: A
commercial lawnmower was on fire.
L325 quickly extinguished
the mower fire and
surrounding grass.
September 16, University
and 142nd: Medical waste was strewn
along University. Fire crews assisted in
isolating the waste. SERVPRO assisted
with cleanup and disposal. The hauler
was charged for the response.
September 2: A badge pinning ceremony
was held for members who completed
their probationary period. Mayor
Cirksena administered the oath of office.

OCTOBER October 11, 8435 Alice Avenue: Fire
crews arrived to a light haze in an
apartment building hallway. One
apartment had smoke banked halfway to
the floor. Occupants used a fire
extinguisher to extinguish a cooking fire
inside the oven. Fire department
personnel assisted with ventilation. No
injuries and no structural damage.
October 15, 11800 Hickman Road:
Motor vehicle crash with subject trapped
at the entrance to
Loves. A minivan T-
boned the right rear of
a semi-trailer. The
trailer’s rear wheel
carriage was knocked
completely off the trailer. Hydraulic
rescue tools were used to open the
driver’s door. The patient was
transported to the hospital.
October 21, 1445 85th Street: A fire was
reported at Clive Children's Center.
Crews arrived to find a light haze inside
of the building. A box was on a stove
burner and was accidentally ignited.
Staff had extinguished with an
extinguisher. No structural damage was
found. One staff member was evaluated
for inhaling extinguisher powder, but did
not require transport to the hospital.
October 22, 7500 University: Crews
arrived to a reported car fire. On arrival,
the car owner was attempting to put out
the fire with an extinguisher. Fire crews
completed extinguishment. Damage
estimated at $5,100
Fire Prevention Week Activities: Fire
Prevention Week
included numerous
educational activities
with pre-schools and
elementary schools. In
addition, an open
house was held at Station 32 on
Saturday morning.
Operational Change to Single Engine
Response: After an internal review of
data and experience, our response
protocol was changed on October 22
from a two engine response to a single
engine response on automatic
fire alarms.
NOVEMBER November 4, 8755 University
(Hardees): An SUV accelerated
over the sidewalk and struck a
corner of the building causing
significant damage. No injuries
were reported. Mechanical
issues were noted as causing
the car to unexpectedly accelerate.
November 11, 7300
University: Single
vehicle was
reported to have
rolled-over on its
side. E320
stabilized the vehicle. The roof was cut
and folded down for patient removal. The
single occupant was transported with
non-life threatening injuries.
November 21, 1601 NW 114th Street
(Mercy West): A light fixture under an
awning was on fire. A light ballast failed
and ignited the plastic light cover. A
water can was utilized to extinguish/cool
the plastic until no further smoldering/
melting was taking place. Damage was
estimated at $500.
November 29, 7119 Jefferson St
(Windsor Heights): Clive provided an
engine, ambulance, and chief officer to
assist with a structure fire in Windsor
Heights. Clive crews assisted with
ventilation and checking for extension. A
melted dehumidifier was found in the
basement and the outlet was charred.
Merger Discussions Fail with Windsor
Heights: On November 2, the Windsor
Heights City Council chose to remain with
an independent fire department. This
decision followed two years of close work
between the communities to develop a
merger plan that provided mutual
financial and operational benefits.
Repaired Steps and Sidewalks: Clive
Public Works crews rebuilt the stairs and
ramp around the NW corner of Station
32. CPW graciously
took on the project
and did a very
professional job.
DECEMBER December 14, 12695 Woodlands
Parkway: A search for an elderly person
with dementia was completed by police
and fire personnel. Following 1 1/2 hours
in rain and 40 degree temps, she was
found. She was evaluated for
hypothermia before being returned home.
December 17, 9888 NW 119th Court -
Granger: Clive responded with a Mass
Casualty Incident trailer for a search in
progress . The tent was set-up with
heating to protect the missing person’s
family from the cold and elements.
December 27, 13100 Block of
Cedarwood Avenue: An residential fire
alarm response revealed smoke and
flames towards the back of the first floor.
The front door was forced and crews
extinguished a fire on a dining room table.
Damage was estimated at $1,000.
December 31, 2800 Block of 167th
Street: A carbon monoxide alarm
activating at a single family home
registered a reading of 13 parts per
million in a basement utility room. The
engine crew remained on scene and
allowed the occupants to shelter in the
fire engine away from the cold until Mid-
American arrived.
Lt. Eric Kallem Promoted to Assistant
Chief - Training: Eric was promoted to
Asst. Chief of Training
following an assessment
center promotional process.
He was originally hired as a
career Firefighter/Paramedic
in 2001 and promoted to
Lieutenant in 2006.
Santa Day: On December 12, the CFFA
brought Santa to Clive for the 53rd
consecutive year. Due to the size of Clive,
2015 brought a new format with seven
pre-designated Santa Stops.
Replacement E220:
A replacement engine
was received for Station
22. This engine was
manufactured by
Pierce. It replaces a
rescue engine that served Clive and
West Des Moines for over 16-years.

PHOTOS
