Sports Medicine Excellence Group’s network is providing its partners professional support with regards to doc-tors’ day to day work with patients but also in the fi elds of studies, research projects and implementation of self-developed in-novative technics.

In order to achieve these goals, SME has developed a concept that focuses on a holistic treatment for patients ranging from the early stages of diagnosis through surgery to the fi nal plans of rehabilitation. Close cooperation and constant commu-nication with all the relevant partners are key to the success of achieving the highest standards.

An example of such a concept is the inte-grated health care contract (IV as German abbreviation) concluded by all parties in-volved. In 2011, the IV-contract with Bar-mer GEK was launched in Lower Saxony, and in South Tyrol a public mandate is being negotiated with the local medical service. These contracts stipulate all in-clusive prices that manage the treatments

of musculoskeletal injuries, from the dia-gnosis to the therapy, and which, through effi cient control and added value creation, help reduce costs.

A second example is HOSPITECTURE®, a joint venture between SME and Matteo Thun & Partners, which has newly defi ned a patient’s residence (hospital) during his/her rehabilitation. The concept combines the highest standards of medical care with luxurious but affordable comfort.

A third example of SME’s concept that focuses on a holistic added value creati-on within the musculoskeletal system is the close cooperation between SME and Salzmann AG. Salzmann AG is a compa-ny based in St. Gallen (Switzerland) that, through centralising and standardising, aims to reduce the costs for material and implants used during surgeries.

Scientifi c research allows for a controlled and reproductive quality. The OSM Re-search Foundation, which is a scientifi c foundation integrated in the network, is providing the necessary infrastructure. This allows for the patient to enjoy the re-sults of the applied clinical research. Training concepts are being used to inte-grate young talents into existing practices in order to expand the structure of the net-work and to ensure succession is guaran-teed.

It is the requirement to ourselves to pro-vide the highest possible level of patient care which allows for a fl exibility with the constant changes of the health care sys-tems, insurers and doctors.

PARTNER OF SPORTS MEDICINE EXCELLENCE GROUP

The Sportsclinic Austria (SCA) was founded in 2007 as the fi rst Sportsclinic of the network of Sports Medicine Excellence Group. Four surgeons and seven receptionists/medical assistants care for their patients. Ao. Univ. Prof. Dr. Christian Fink, Dr. Christian Hoser and Dr. Peter Gfoeller treat acute and chronic muscu-loskeletal disorders following the motto Movement – our expertise. In addition to sports and trauma injuries, their focus is on the treatment of osteoarthritis and chronic joint disease. Dr Verena Dollinger advises on all aspects of general and sports medicine. Manual and pain therapy and acupuncture are also part of their treatment fi elds as well as Sports medical examinations and consultations training. The four are supported by medical teaching interns. The Sportsclinic Austria is the only private Traumatology of Austria, which is

available to their patients 365 days a year.Looking after the patient’s welfare and the daily organisation of the practice are: Julia Hofherr, Isabella Reiter and Anna Marthe as assistants, Gabi Hofer-Stern and Angela Huebner in the practice management, Astrid Mayer as offi ce manager. The private hospital Hochrum is a partner of the Sportsclinic Austria. The orthopaedic technology Schaper, sports therapy Huber and the Institute of Sports, Alpine Medicine and Health Tourism are in the same building which allows for optimal com-munication between doctor, patient and therapist and guaran-tees fast care and recovery of patients.

Since 2009, following the motto Joints – our expertise, the team of surgeons at the Sportsclinic Germany (SCG) in Hannover, offers medical expertise in the fi eld of orthopaedics, trauma surgery and sports traumatology of the large joints, i.e. shoulder, knee, elbow, hip, ankle and foot. The concept of Sportsclinic Germany is to provide an in-depth treatment in-stead of a broad range of basic and standard care. Specifi -cally, those patients should fi nd help who do not achieve their goals with a conservative treatment. The preservation of the joint after an individual diagnosis and counselling is a priority. The team of doctors is: Prof Dr Philipp Lobenhoffer, PD Dr Jens Agneskirchner and Dr Markus Troeger as well as Kristian Kley. Dr Malte Holschen, in his last year of training, assists the team. The practice management team is: Sabine Deppe and Rita Grundmann, who have been part of the team since the ope-ning of the Sportsclinic Germany, Julia Ricke, Sonja Queisler

(parental leave) and Annika Brettner as well as Rebecca Meyer as assistant to Prof Lobenhoffer.In the back offi ce Reiner Behr is taking care of the accoun-ting once a week and has been part of the team since the be-ginning. Inga Beyer is responsible for marketing and scientifi c support within the network.In order to guarantee fast, optimal and individual care for pati-ents, the Sportsclinic Germany’s team cooperates with various partners. The treatment rooms as well as the clinic for anaes-thesia of Mr Alimy are in the building of the Eilenriede hospital at Uhlemeyerstrasse 16. For outpatient and inpatient surgery, there are operation theatres and beds at the Eilenriede hospital and at the DRK hospital Clementinenhaus. The physiothera-pists of the therapy centre Langenhagen look after inpatient patients at both hospitals.

The Sports Medicine Innovations AG (SMI) offers the group’s surgeons to implement their ideas from their clinical practice into reality. The SMI develops and testes such ideas in close cooperating with the surgeons but also with external specialists by using innovative products and techniques for surgical treatment of injuries of the musculoskeletal system. Dr Wolfgang Schwaiger is coordinating and managing this fi eld and generates new ideas along with the doctors.

During 2013, the Sportsclinic Switzerland has revi-sed its positioning strategy in Switzerland and with this has ter-minated the cooperation with the Hospital Gut in St Moritz. The SME will continue - and in particular – to implement proactively the goals of medical excellence, the science and optimise the cost processes set within the group. Thereby anchoring in the public and not just in the private sector is an important prere-quisite for the positioning of Sportsclinic Switzerland.

As integral part of the network the OSM Research Foundation (OSM) covers the area of research. In order to achieve consistent standards for documentation and quality control, the foundation is providing scientifi c personnel and the latest technical equipment. Other priorities of the foundation are the further education for surgeons, in the context of surgery training and further education as well as the development of young talents in the fi eld of orthopaedics and sports traumto-logy. All Sportsclinic locations offer such further education for young surgeons. Mag Caroline Hepperger is head of scientifi c

Since May 2012, the Sportsclinic Italy in Bozen (SCI) is open. Here, the renowned orthopaedic trauma surgery group practice Ortho+ was won as partner. Following the motto Activity – our expertise the surgeons treat patients with dis-comforts on shoulder, hip, spine, knee, foot and hand but also cover accident surgery: Dr Christoph Frontull, Dr Peter Moro-der, Dr Thomas Oberhofer, Dr Markus Mayr, Dr Robert Gruber and Dr Florian Perwanger. The practice management team is: Angelika Rabanser, Linda Pezzela, Margrit Innerebner, Renate Brunner and Sabine Ebner.

In January 2013, Sportsclinic Germany opened a second facility in Munich. In Munich Dr Manfred Achten, Dr Volker Braunstein and Prof Dr Stefan Hinterwimmer offer, as specialists in sports medicine, orthopaedics and surgery with a holistic treatment approach. Following the motto Mobility - our expertise it is their primary aim to shorten the course of treatment in order to help their patients to become mobile and active again. The surgeons offer their patients a full range of prevention, diagnosis, conservative and surgical therapy and rehabilitation of disorders and sports injuries of the shoulder, elbow, spine, hip, knee, ankle and foot. Looking after the pati-ents are Antonia Moser and Doris Losbichler, as practice ma-nager respectively medical assistant. The partners of Sportscli-

As in the previous two years, the activities of the group in Switzerland have been characterised by the esta-blishment of cost control and its negot-iations with the industry. This process deems to be quite slow as, even with the introduction of DRG (possibly because of it), the focus to optimise the value crea-tion standards has reduced. It proves to be more challenging for the industry in Switzerland to divide product and service costs and with that to bring the product costs to a similar level as the largest Eu-ropean market in Germany.

Sports Medicine Innovations AG (SMI) allows for sur-geons to implement their ideas into reality. SMI is developing, in close cooperation with surgeons as well as external specialists, innovative methods and products for surgical and conservative treatments of musculoskeletal inju-ries. A bottom-up approach is being used by closely working with the network’s surgeons in order for day to day problems to be understood, evaluated and solved.By evaluating and developing the ideas of the surgeons, SMI is able to accelerate innovation through fl at hierarchies as well as through direct interaction with the inventor. The aim is for the surgeon to be accompanied throughout the whole process i.e. from the initial idea through the scientifi c evaluation and taking out a patent to the commercialisation. The competitive advan-tage of an effi cient innovation process in this fi eld will lead to new and cost effi cient products for the patient. In 2013, various projects were further pursued and intellectual property in the orthopaedic and sports medicine was acquired. Dr Wolfgang Schwaiger coordinates the various projects in his capacity as Chief Technology Offi cer (CTO) at Sports Medicine Innovations AG.

Cooperation with Barmer GEK for an integrated health careIn 2011, the Sportsclinic Germany in Hannover agreed to an integrated health care (IV) contract with Barmer GEK. The goal of this cooperation is to offer a wide range of services for shoulder, hip and knee injuries from the diagnosis through to surgery to rehabilitation. By adhering to effi cient processes, costs can be red-uced without compromising on the high quality of doctors. Additional partners that offer such high quality of standards are DRK hospital Clementinenhaus in Han-nover and the Landgrafen Klinik in Bad Nenndorf (rehabilitation).

New removal method for re-construction of the cruciate ligamentProf Dr Christian Fink and Dr Christian Hoser have developed a removal me-thod of the quadriceps tendon. This im-provement is specifi cally used for the reconstruction of the cruciate ligament. Disadvantages, such as cosmetically poorer results, can be signifi cantly redu-ced with the enhancement of a standar-dised, minimal-invasive removal method. With this, the interest for quadriceps ten-don to be used as the primary transplant for the ACL reconstruction is increasing.

New surgery technique to stabilise the patellaProf Hinterwimmer as co-developer of new technique for the reconstruction of the medial patellofemoral ligament (MPFL) to stabilise the patella. The tech-nique is anatomically and avoids any ar-tifi cial material.

PD Dr Jens Agneskirchner and Dr Mar-kus Troeger have developed an equip-ment to reconstruct the cruciate liga-ment as well as its necessary surgery technique for years. The equipment was developed together with DePuy-Synthes. On Friday, 15th March 2013, the so called market preference trial (MPT) began at which the equipment will become accessible on the world-wide market place. With this, PD Dr Ag-neskirchner and Dr Troeger have recei-ved the certifi cate of the United States Patent and Trademark Offi ce. Surgeons from all over the world were invited to trial the new tool at a work-shop. Participants received tips and valuable information from the inventors.

Within the last 4 years, Sportsclinic Aus-tria, the Institute for Sports science at the University of Innsbruck and Sports Therapy Huber have developed a new computer based test procedure that al-lows the evaluation of returning into the day to day work and sports life after a cruciate ligament injury, a knee-check called “back in action”.

Patients will go through seven different tests, including a one- and two leg sta-bilising test on a MFT surface, jump- , speed- and coordination tests. All the data will be entered into a computer and will be evaluated with the help of a spe-cial software. For comparison reasons, the standard values of over 400 heal-thy test people were taken. An athlete would need to be well above the ave-rage values, should he wish to return to the exercise regime.

The researchers conducted two test units for their studies. The fi rst analy-sis occurred four to fi ve months after a reconstruction of the cruciate ligament. All of the people within the test showed

defi cits. After a re-test, after six to se-ven months, they still showed defi cits. Depending on the results, returning to day to day activities would be allowed or delayed. The test shows that many test people wrongly appraise themselves. In future, physiotherapists should be able to obtain the new test procedures.

osteoarthritis of the shoulder and damaged rotator cuff bene-fi t from this arthroplasty for a long term period. The speakers at the course talked about the indications of such implant, its composition and function, possible complications and study results. Further presentations compared the inverse shoulder arthroplasty with other surgical techniques, explained its use for certain injuries or revision surgeries and presented innova-tive materials. Agneskirchner demonstrated, at a model as well as in a live surgery, the surgical technique which then allowed the participants to practise during workshops. Subsequently, special cases from the practice were discussed.

7. International Oxford Course – 14th & 15th June 2013Prof Dr Philipp Lobenhoffer initiated the course along with Chris Dodd, surgeon and specialist for operational technolo-gy of the Oxford partial prosthesis, from Oxford. This course is the only German speaking course out of two authorised Oxford courses in Germany. Approximately 55 participants followed the lectures and presentations by Prof Lobenhoffer, Chris Dodd, Dr Markus Troeger, Kristian Kley and Dr Sanjay Weber-Spickschen. The topics ranged from the introduction of the new Oxford-prosthesis, the indication and correct choice of patients, demonstration of surgery techniques as well as counter indications of the comparison between Osteotomy and full prosthesis to study results, after treatments and what to do when complications occur. Participants could practise the

Knee Course Hannover – Osteotomy and partial joint replacement – 22nd & 23rd February 2013.Prof Dr Philipp Lobenhoffer and PD Dr Jens Agneskirchner were the scientifi c directors at the further education course for surgeons which took place at the Hotel Courtyard by Marriott at Maschsee.Approximately 80 participants followed the live surgeries and lectures on the three topics: Osteotomy around the knee, un-dicondylar joint replacement and selective femoral patellar replacement. Participants had the opportunity to practise and enhance their skills during workshops as well as ask questions during a discussion round.

3rd Reverse Shoulder Arthroplasty Course – 3rd & 4th May 2013PD Dr. Jens Agneskirchner was the scientifi c director. The course offered the approximately 80 participants from 12 coun-tries in depth theoretical and practical aspects as well as impro-vements for the inverse shoulder arthroplasty. Agneskirchner was supported by a well-known faculty: Prof Dr Philipp Loben-hoffer and Dr Markus Troeger from the Sportsclinic Germany, Prof Dr Helmut Lill (Hannover), PD Dr Joern Kircher, Dr Hans Rudolf Bloch and the current president of the “European Soci-ety for Shoulder and Elbow Surgery” (SECEC/ESSE), Dr Ales-sandro Castagna (Italy). Over the past fi ve to ten years, the inverse shoulder arthro-plasty was further distributed and developed. Patients with

AGA academy course ‘Update: arthroscopic operati-on techniques on the shoulder’ – 6th & 7th September 2013The AGA course, which was held in Innsbruck, was run by Prof Dr Christian Fink and Dr Christian Hoser as well as PD Dr Jens Agneskirchner. Participants were informed about the latest and best practice of various surgical techniques by numerous presentations. The practical part was a priority and was much appreciated by the participants. The speakers demonstrated various surgical tech-niques and were happy to answer any questions.

Hip meeting in Bozen – 11th October 2013Get together for surgeons, physiotherapists and trainersSurgeons and physiotherapists from all over the South Tyrol ga-thered at the SCI’s meeting room, in order to discuss the diagno-sis and treatment of hip joint injuries. High level presentations on the cause, diagnosis, conservative and surgical treatment possibilities were enhanced by the infl uence a diet could have on osteoarthritis.

Hannover Arthroscopy- and Joints Course – 8th & 9th November 2013Prof Dr Philipp Lobenhoffer, PD Dr Jens Agneskirchner and Prof Dr Helmut Lill (Friederikenstift Hannover) were the scienti-fi c directors. The course was held, as previously, at the Medical School Hannover. The operations were transmitted live in HD quality from the Friederikenstift Hannover. Core facts: The course included 14 highly complex live ope-rations, discussions and workshops and is therefore unique within the German speaking area. Apart from the arthrosco-pic surgeries on shoulder, knee and hip, joint replacement was another main topic of the annual course. Apart from the 11 sur-geons, there were 35 physicians, 10 students, 10 nurses, 6 video technicians, various staff from Intercongress as well as 10 members of the teams of Friederikenstift and Sportsclinic Germany to work behind the scenes. The eight shoulder surgeries on Friday as well as the six hip respectively knee surgeries on Saturday were enhanced by panel discussions and industry related workshops. So the 312 participants were enabled to further educate themselves on a theoretical as well as on a practical level. For the fi rst time, si-multaneous translation was offered for non-German speakers during the live surgeries.

1. Munich Forum Sports medicine – 13th November 2013Dr Manfred Achten from Munich was, along with PD Dr Bernd Wolfarth, the scientifi c director of the fi rst sports medicine fo-rum. After a welcome, Dr Volker Braunstein gave a lecture on stress manifestation of the shoulder during sport activi-ties. During the second lecture, Dr Achten talked about back pain in day to day life and demonstrated possible preventa-tive treatments. Afterwards, Prof Stefan Hinterwimmer talked about knee injuries during sports activities. After a break, during which the participants had the opportuni-ty to visit the exhibition and discuss any issues with the spea-kers, PD Dr Wolfarth explained sports medical aptitude and performance examinations from an internistic point of view. Guest speakers, Dr Klaus Kenn and Dr Roland Schmidt, tal-ked about ‘Pneumology pressure induced respiratory distress in the fi eld’ and ‘Sport cardiology – what is new?’.

Further education of young surgeons is part of the network’s philosophy. Interested surgeons have the possibility to sit in on certain surgical techniques for one or more days, weeks or months. During a year, this would be approximately 70 visitors from all over the world. Of course, surgeons are also using the network in order to participate in fellowships or lectures of other peers (see year in review OSM).

The SME group has agreed to a colla-boration with the Swiss company Salz-mann Group which will reorganise and standardise the procurement and logisti-cs of materials and implants which are needed in medicine of the musculoskel-etal system. With the help of the logistics service of Salzmann Group, the supply to partner hospitals is unifi ed and simpli-fi ed. These logistics solutions as well as projects that also aim to standardise indi-vidual operations (DRG‘s), have the goal to reduce the material costs of the group, while maintaining the highest quality of products in order to offer a high quality, but affordable patient care in the future.

HOSPITECTURE® MATERIAL COSTS, LOGISTICS

We are working on many sites of the SME group together with our sur-geons on projects that refl ect changes in the health care bill and allow, in prepara-tion of outpatient surgery, lean medical processes.

The Sports Medicine Excellence Group, in collaboration with architect Matteo Thun from Milan, has founded the Joint Venture HOSPITECTURE ® in order to come up with innovative struc-tures which allow for a new treatment and care after surgery.

HOSPITECTURE® newly defi nes the hospital for patients with injuries of the musculoskeletal system and in a way that allows for high-quality medical care in a luxurious environment at affordable prices. Sustainable and effi ciently-sca-led architecture is therefore associated with processes that can greatly enhance the medical effectiveness, accommoda-tion and rehabilitation of the patient.

HOSPITECTURE® combines the medical care with the concept of hospi-tality in a pioneering and simple archi-tecture, so that the rehabilitation can be fully optimised. The words “hospital” and “hospitality” are both originating from the Latin radical “hospes”, “guest”. HOS-PITECTURE ® is therefore seeking to achieve physical as well as mental wel-fare. We accompany our patients – our guests – in their rehabilitation in order for them to get back to their day to day acti-vities as soon as possible.

HOSPITECTURE® is changing the time period between surgery and rehabilitation towards an ambulatory care and / or operation with very short residence time for the patient. The con-cept of costs that arise from injuries, are being newly defi ned by combining surgi-cal rehabilitation in a hotel-style, recove-ry friendly ambience.

HOSPITECTURE® is fi rst incor-porated into new structures that are planned in Innsbruck and - as a satellite location of Innsbruck - in Kitzbuehl.A prestigious project which will be built along with the Austrian Ski Federation (ÖSV) on the Olympic site in Innsbruck, will combine elite sport and cutting-edge medicine and will equip exclusive part-ners such as the ISAG (Tilak), sports therapy Huber and Schaper Bandagist and orthopaedics. The Sportsclinic Aus-tria will take over the patient care for the region from the existing hospital Kitz-buehl and thereby, in cooperating with the city of Kitzbuehl, awaken the life of the hospital.

2013 was shaped by a strong change in the health system in various countries. Switzerland has woken up slowly from its shock after the DRG-introduction two years ago. Ma-terial and implant costs are a frequently surfaced topic. The Neue Zuercher Zeitung as well as the renowned “Schweizer Handelsblatt” address the different, partly grotesque price diffe-rences in various European countries. Same products can cost up to 3 times more in Switzerland than in Germany.

The SME continues its focus to work hard on concepts that, through standardisation and collaboration of elected and im-portant partners from the industry, reduce costs and provide the highest level of patient care but in a competitive price ran-ge.

In the future, patient care will be shaped by inter divisional treatment packages which will defi ne patient care - from the diagnosis to rehabilitation - as part of an all-inclusive package. These price packages, which will have been negotiated with in-surers and “payers”, shift the business risk (but also the chan-

ces) back to the surgeons, who are in a position to infl uence and control the total added value creation of the musculoske-letal system.

These concepts which, due to cost pressure in the individual countries, will be the only solution and the only way to a more cost conscious but high quality health policy, require even fur-ther standardisation of medical products within the European countries. It is important to therefore separate the product costs - varies in different countries - and service costs. With this, the individual claimable service will be depending on the capability for innovation and effectiveness of the supplier, whilst the pro-duct price will and must be consolidated.

The yearly Medtech-Report by Ernst & Young1 features a chan-ging picture of the medical technology industry and the health systems in Europe to which the “perfect storm” is awaiting. The trend is moving towards value based health systems which reward all supplier that can offer medical quality at affordable prices.

Taking this into account, each part of the value added creation will need to focus its strategy on these three parameters: high-est quality in patient care, most competitive price range within a short time frame, i.e. a timely return to activities.

In order to be prepared for the perfect storm, one must adhere to these three parameters.
