1 • Director’s Message

Assamualaikum w.b.t. & Greetings to all, Salam 1 Malaysia

In 2010, the Traditional and Complementary Medicine Division (T&CMD), Ministry of Health Malaysia has made many inroad achievements, as the human resource capability was improved through additional staff and training. One of the achievements was with the Key Performance Indicators (KPIs) in which all achieved the targets set. T&CMD had also completed four (4) researches in the year 2010 and one of the research was published in the international Journal of Alternative and Complementary Medicine (JACM), November 2010 edition.

T&CMD continued playing an active role in promoting Traditional and Complementary Medicine (T&CM) in Malaysia. T&CM tourism directory were printed for the state of Sarawak and other activities such as exhibitions, conferences and seminars were held. In 2010, two(2) T&CM units were set up at Sultanah Bahiyah Hospital, Kedah and Port Dickson Hospital, Negeri Sembilan. This bring to a total of eight (8) hospitals with T&CM services. These hospitals offer T&CM modalities for the management of chronic pain and post stroke.

To safeguard safety of the rakyat, T&CMD had collaborated with the Imigration Department Malaysia in screening foreign practitioners who intend to practice in Malaysia. Local Government Councils such as the Kuala Lumpur City Hall (DBKL) were sought in matters related to advertising and implementation of activities. To ensure more effective monitoring of the practices, five (5) zone offices were established in Butterworth, Kuala Terengganu, Johore Bahru, Kota Kinabalu and Kuching.

All the achievements made with the support and advice from the Director General of Health, Malaysia Tan Sri Dato Seri Dr Hj Mohd Ismail Merican and Deputy Director General of Health (Research and Technical Support), Dato Dr Maimunah A. Hamid.

Finally, I take this opportunity to thank all T&CMD staffs who have shown dedication and commitment in achieving T&CMD KPIs, programs, and activities for the betterment of T&CM practices in Malaysia. God loves a person who works with dedication and enthusiasm to improve and the best are those who bring good to other people (al-Hadith).

2 • Organisation Philosophy

Strategy

To achieve the above vision and mission, Traditional and Complementay Medicine Division (T&CMD) have planned the following strategies:

a. To increase the competency and efficiency of the organisaton through development of effective human capital, moderation and automation of office function, and to constantly improvise the services.

b. To develop practice guideline which ensures quality services are providrd and conforms with standard practice.

c. To strenghten mapping activities of premises which provides T&CM services.

d. To ensure good understanding and working relationship between T&CM practitioner bodies and T&CMD through meetings and dialogues.

e. To enhance T&CMD staffs’ expertise.

f. To create an dedicated and fully motivated staff through appropriate remuneration.

g. To accelerate activities in research, development and innovation.

h. To create a condusive environmenment which encourage staff to work as a team and to achive the assigned task professionally.

i. To strenghten working network with government agencies, private sector and also international organisations.

MiSSion

Traditional and Complementay Medicine Division will ensure that the practice of traditional and complementary healthcare is safe and effective for the benefit of the citizen.

ViSion

Traditional and Complementary Medicine Division aspires to be a centre of excellence for traditional and complementary healthcare practices.

Quality Policy

We are committed to implement regulatory and standard system of healthcare practices of traditional and complementary medicine that is efficient and effective towards enhancing level and quality of public wellness.

I. Issuance of support letter for foreign practitioners will be processed within 14 days

III. To ensure Traditional and Complementary Medicine Division staffs at all levels undergo a minimum of 7 days training in a year

V. Traditional and Complementary Medicine Units to be developed within 9 months

VI. Payment for supplies and services to be made within 14 days upon receival complete invoice

5.1 ADMINISTRATION AND FINANCE SECTION

For the year 2010, there is a positive growth in the human resource in Traditional and Complementary Medicine Division (T&CMD). T&CMD has a total of 56 positions in 2010 with a total of 43 positions being filled, which is about 77% of the total position. The list of T&CMD position is shown in table 1 while the T&CMD organization Chart is shown in chart 1.

In 2010, T&CMD received RM 2,179,635.00 budget for operational and development expenditure. A large percentage of the budget (75.1%) was spent on management services such as payment to T&CM practitioners and acquisition supplies for T&CM integrated hospitals. Figure 1 shows the distribution of the budget and expenditure from year 2006 to 2010:

T&CMD has settled payment for 94% of invoices received within 7 days. There was one invoice which was processed more than 14 days due to eSPKB problems (technical problems). Table 5 shows the summary of the Bill Payment Performance.

For the first time a processing fee of RM 200 was charged for application of foreign practitioners in the year 2010. A total sum of RM 15,400 was collected.

5.1.1 INFORMATION MANAGEMENT UNIT

For the year 2010, the information management unit has successfully completed two (2) main projects:

1. Coordinate the development of the Inspectorate and Enforcement (I & P) Module. The I & P Module has successfully been developed as planned and fully operational by 1 January 2011.

2. Upgraded T&CMD website to the new version. This version was ranked third (3rd) among MOH Divisions at headquarters, and ranked 31st out of 289 entries at the Ministry level. Furthermore, it was given 4 stars rating.

INSPECTORATE AND ENFORCEMENT MODULE

The purpose of I & P Module is to improve efficiency of enforcement activities in line with the objectives of the T&CMD and to facilitate retrieval of data and statistics upon request. It is an online web based module. Figure 2 shows the nine (9) major sub modules.

TRAINING ON INFORMATION AND COMMUNICATION TECHNOLOGY (ICT)

Information Management Unit has conducted ICT training courses for all T&CMD staffs. The objectives are:a) To upgrade the competency of the T&CMD staffs to carry out tasks in e-government

environment.b) To improve skills in ICT.c) To produce IT savvy T&CMD staffs.

5.2 PRACTICE AND REGISTRATION SECTION

FOREIGN T&CM PRACTITIONER REGISTRATION UNIT

For the year 2010, T&CMD processed a total of 183 applications and 17 applications for appeal. The number of applications approved for both T&CM practitioners and management position at T&CM centers in 2009 and 2010 is about the same. On the other hand, there’s a decrease of about 60% in the number of applications not approved in 2010 compared to 2009. The main factor is due to awareness by the T&CM operators that the subsector of spa, massage and reflexology are frozen by Ministry of Home Affairs (MOHA) and therefore no longer submit the applications in the relevant subsectors. Table 6 shows the breakdown of applications processed:

In 2008, T&CMD had formulated clients’ charter and issuance of approval letter for foreign T&CM practitioners in 14 days is one of the client’s charter formulated. Figure 6 shows the percentage of issuance of approval letter within 14 days has increased from 82.2% in 2008 to 97.4% in 2010. The main factor contributing to this increment is frequent meetings which were held on a weekly basis.

For applications that were rejected, 35.9% of the applicants have insufficient working experience followed by 20.5% who do not possess minimum qualification as stated in the application guideline. Figure 7 shows the breakdown of reasons for rejected applications.

DEVELOPMENT OF PRACTICE GUIDELINES

For the year 2010, T&CMD had conducted a series of meetings to discuss the development of practice guidelines for various modalities under traditional and complementary medicines. Due to the large number of modalities grouped under Complementary Medicine, development of the various guidelines will be done in phases. Two Good Practice Guidelines for Malay Massage and Acupuncture were printed in July 2010 and distributed to all Traditional and Complementary Medicine Units at the integrated hospitals.

Besides that, efforts to develop the practice guidelines as stated in Table 7 is ongoing and is expected to be completed in year 2011. Other activities conducted were meetings to discuss guidelines on National Qualifying Examination (NQE) for Traditional Chinese Medicine, Homeopathy and Islamic Medicine.

5.3 STANDARD AND QUALITY SECTION

DIALOGUE SESSION WITH GOVERNMENT AGENCIES AND TRAINING CENTRES

In 2010, T&CMD, Ministry of Higher Education (MoHE), Malaysian Qualifications Agency (MQA) and Department of Skills Development (DSD) have visited 18 academies / training centers which offered diploma and degrees programs in T&CM.

Following the visits, a dialogue session was held on 2nd August 2010 at the Institute of Health Management. A total of 106 participants from 62 academies / training centers attended this dialogue session. The following topics were presented:

1. Establishment of Private Institutions of Higher Learning (IPTS), IPTS Act 1996 (555 Act) ~ by MoHE

2. Program accreditation and Quality Assurance System ~ by MQA3. Accredited system and Malaysian Skills Certification ~ by DSD

Important issues raised during the dialogue were as follow:

Objectives of these courses were:a) to train T&CMD internal facilitator in the process of developing CoS/NOSSb) to expose the participants on the use of CoS/NOSS c) to relay knowledge, understanding and skills on CoS/NOSS development processes d) strengthen the participants’ capability as a facilitator in conducting CoS/NOSS development

A total of 16 participants from T&CMD and practitioner bodies who are also the permanent members for CoS/NOSS Development Working Committee attended these courses. Participants were divided into 3 groups during individual and group practical session, namely Malay Massage, Reflexology and Energy Medicine (Crystal Vibration Therapy & Reiki).

CONTINUOUS MEDICAL EDUCATION (CME) SESSIONS

In the year of 2010, a total of 13 CME activities were held. The aim was to expose and disseminate information to the MOH staffs on the development of T&CM. These activities are shown in Table 11.

5.4 POLICY AND DEVELOPMENT SECTION

ESTABLISHMENT OF T&CM UNITS IN INTEGRATED HOSPITALS

For the year 2010, T&CMD has establish T&CM Units in two government hospitals (Integrated Hospital).The hospitals are Port Dickson Hospital, Negeri Sembilan, and Sultanah Bahiyah Hospital, Alor Setar, Kedah. These units are managing chronic pain cases and post stroke patients.

ESTABLISHMENT OF T&CM BRANCH OFFICE

In the 10th Malaysia Plan (10-MP), T&CMD targeting to establish at least 2 (two) T&CM branch office each year. In year 2010, in order to facilitate registration, inspection and enforcement activities, 5 T&CM branches have been established. They are at Johore Bahru (for Southern Zone), Kuala Terengganu (for Eastern Zone), Butterworth (for Northern Zone) and as well as in Sabah and Sarawak.

Work on upgrading the building and acquiring the assets have already been implemented for the year 2009. This branch office was fully operational in 2010 with minimum position filled by Medical Doctor/ Pharmacist, Administrative assistant and Driver.

T&CM TOURISM DIRECTORIES

To promote tourism for T&CM, T&CMD has prepared tourism directories. For the year 2010, the directory for the state of Sarawak was prepared. In this directory, the type of practice and address of premises are included. These directories are distributed at the entry points to the state and also other public venues for tourist guide.

CONSULTATIVE SERVICES

T&CMD gives consultative services to Local City / Municipal Council and Companies Commission of Malaysia on the matters related to the usage of license and registration names on T&CM related businesses. For the year 2010, T&CMD received 21 applications from City / Municipal Council and Companies Commision of Malaysia and all the applications were addressed within 3 working days upon receival.

T&CMD has formulated a guideline in setting up T&CM unit in MOH hospitals. This guideline is used as a template for future setting up of T&CM units in MOH hospitals.

5.5 INSPECTORATE AND ENFORCEMENT SECTION

In 2010, the Inspectorate and Enforcement Section (I&P) have successfully carried out enforcement activities such as mapping & inspection of premises as well as consumer/public education throughout Malaysia. The I&P Section has collaborated with other agencies such as the Pharmacy Enforcement Division, Immigration Department, Private Practice & Medical Control Section (CKAPS) and City & Local Government Councils in carrying out integrated enforcement activities. In addition to that, I&P Section also contributed to the development of Traditional and Complementary Medicine Bill which is planned to be tabled in Parliament in 2011.

Twelve (12) T&CM exhibitions were successfully held in hospitals, mosque and various places of attraction in 2010 (Table 13). These exhibitions were aimed to introduce and disseminate information on T&CM and integrating traditional and complementary medicine in the public healthcare system. Complimentary gifts such as bookmarks, keychain, car stickers and books were given to visitors.

In terms of inspectorate activities, a total of 129 premises were inspected which is higher than previous year (109 premises). As for today (from 2007 till 2010), a total of 391 premises have been inspected with Kuala Lumpur area leading the statistics with the highest total of 149 premises (Table 14). T&CM also recorded the highest modality is from the Traditional Chinese Medicine with a total of 167 premises. These I&P activities were divided according to zones namely North, South, East, Sabah, Sarawak and Middle Zone.

With regards to public complaints, 46 complaints have been recorded which is 2 times higher than the previous year. The complaints were received from the public, various boards of practitioner, government and non-government agencies. Most of the complaints fall under the category of malpractice/misconduct and illegal advertising (Table 15).

Until December 2010, a total of 1100 premises have been mapped across Malaysia.

5.6 RESEARCH AND DEVELOPMENT SECTION

Main achievements of Research and Development Section year 2010 are;

A. Four (4) studies were completed

i. “A Qualitative Study on Urut Melayu – the Traditional Malay Massage”- This study has been published in The Journal of Alternative and Complementary Medicine

(JACM) November Issue- The journal can also be accessed at www.liebertonline.com/toc/acm/16/11

ABSTRACT

Objective: We conducted this study to gain an insight into the experiences and views of practitioners of urut Melayu, the traditional Malay massage, which will be used in developing a preliminary framework of the urut Melayu process.

Design: We adopted a qualitative study design. We carried out a total of five focus group discussions (FGDs) comprising 6–10 urut Melayu practitioners each.

Location: We carried out three FGDs at the Traditional and Complementary Medicine Division, Ministry of Health and two FGDs at a district Health Clinic.

Subjects: All participants of the FGDs were urut Melayu practitioners registered with the Ministry of Health. Three (3) FGDs comprised all females while two comprised all males. A total of 12 males and 24 females participated in the study.

Research monitoring visit for “The Effect of Acupuncture or Urut Melayu on the Quality of Life of Post-Stroke Patients”

In-depth interview with Malay traditional midwives at Kuala Selangor for “A Study on Malay Postnatal Care

in Johore and Selangor, Malaysia”

Results: We identified six themes from the study, namely, indications for urut Melayu, the urut Melayu technique, other treatments in conjunction with urut Melayu, outcome of urut Melayu, ethics of urut Melayu, and practitioners’ source of skills and knowledge.

Conclusions: Urut Melayu is a unique form of massage carried out for various purposes. Although it is common belief that there are vast differences in the way it is performed from one practitioner to another, this study revealed that similarities do exist and there is potential to develop a standard framework for urut Melayu for regulation and training purposes.

5.7.1 SOUTHERN ZONE

Introduction

Traditional and Complementary Medicine Division Southern Zone office is located at Johore State Health Department and has officially started operations on 15 September 2010. The branch office is in charge of tasks related to T&CM for three states, which are Johore, Melaka and Negeri Sembilan.

Mapping work started in October 2010. 153 premises were identified by 31st December 2010. A total of 5 premises were inspected comprising of Traditional Chinese Medicine and reflexology premises operating in Johore Bahru.

The Branch has received only one complaint in 2010. The complaint was regarding high service charges and unsatisfactory staff attitude.

Financial Management

This branch was allocated a budget of RM62,800.00 for the year 2010. The allocation received was shared with Traditional & Complementary Medicine Unit, Sultan Ismail Hospital.

It was originally located at Penang State Health Department but later moved to a new office at Butterworth Health Complex in October 2010. The zone office is in charge of tasks related to T&CM activities for three states, which cover Penang, Kedah and Perlis

The branch has conducted three (3) inspections of T&CM premises and mapping of fifteen (15) T&CM premises in 2010. Besides that, the branch has collaborated with T&CM Unit, Kepala Batas Hospital to organize a campaign for health care providers in August 2010. Nearly 100 participants from the Perak, Kedah, Penang and Perlis State Health Departments attended the seminar. In addition, the branch had also given 3 CPD sessions which were organized by the Penang Pharmaceutical Services Division.

Financial Management

The branch was allocated a budget of RM62,800 for the year 2010. The allocation was shared with for renovation works of the T&CM unit in Kepala Batas Hospital. The types of renovations include separation of the male and female massage room, notice boards and incorporation of storage shelves.

Traditional and complementary medicine division Eastern Zone office has been established and operating since June 13, 2010. This zone office oversees T&CM activity for three states, which are Terengganu, Kelantan and Pahang.

Zone Activities and Financial Management

Sabah zone was allocated an operational budget of RM62,800. A total of RM58,667(93.42%) has been spent on purchases of furniture and fittings as well as to meet the requirements needed for services. The remainder of the budget was used to improve the standards of the Therapeutic Garden in T&CM Unit at Duchess of Kent Hospital, Sandakan.

Renovation work on the zone office was completed on 24th December 2010.

Zone Activities

A total of 37 premises were inspected consisting of Traditional Chinese Medicine and Reflexology centres operating in the area Kuching, Sibu, Bintulu and Miri. A total of 102 premises were mapped in the Kuching area.

Two official complaints were investigated which involved malpractice, substance abuse, and fraud.

The zone branch had organised two (2) seminars titled “Updates in T&CM” on:a) July 17, 2010. A total of 200 practitioners attended the seminar.b) December 14, 2010. A total of 183 participants consisting of MOH staffs attended the

seminar

Financial Management

This branch was allocated an operational budget of RM62,800 for the year 2010. The allocation was used for the repairing of damaged furniture, purchasing of office equipment and upgrading of services for the zone office building.

5.8 TRADITIONAL AND COMPLEMENTARY UNITS

5.8.1 KEPALA BATAS HOSPITAL, PENANG

The Traditional and Complementary Medicine Unit of Kepala Batas Hospital was operational since 18 October, 2007. The Opening ceremony was officiated by our then honourable Prime Minister, Tun Dato’ Seri Abdullah Haji Ahmad Badawi on 26 October, 2007.

5.8.3 PUTRAJAYA HOSPITAL, FEDERAL TERRITORY PUTRAJAYA

Introduction

Traditional and Complementary Medicine Unit in Putrajaya Hospital started to operate on 3rd March 2008 by offering traditional Malay massage and acupuncture treatments for post stroke and chronic pain patients. The treatments intend to help improve stroke patients’ rehabilitation and reducing the pain in chronic pain patients. Later on 1st October 2008, the unit has added herbal treatment into its service. Herbal treatment is provided to cancer patients as an adjunct to conventional cancer treatment. Malay traditional post natal care is another new service introduced on 17th July 2009.

Statistics

In general, there is reduction in numbers of new chronic pain patients for Malay Massage and Acupuncture in year 2010, compare to 2008. Beginning 2009, T&CM Unit only received referred cases, and no longer via walk in (like the practice done pre 2009). Thus, there is a decline in numbers of new patients.

However, each new patient received more sessions of treatment compared to the previous years. On average each patient receive more sessions of treatment in 2010 (average treatment session for Massage = 3.2 sessions, while Acupuncture = 6.9 sessions) compared to the year 2008 (average treatment session for Massage = 1.1 session, Acupuncture = 2.7 sessions). The number of treatment session is in accordance with T&CM Guidelines on Malay Massage (2nd Edition) and T&CM Practice Guidelines on Acupuncture (2nd Edition). These good practice guidelines have been introduced by T&CMD in year 2010.

Malay traditional post natal care has just been introduced by T&CM Unit on 17th July 2009, hence there is lack of data to compare. However, herbal therapy and post natal care number of patients is significantly increasing. The comparison of statistics from year 2008 to 2010 is shown in Table 17.

1. Antenatal Class

T&CM unit participated in antenatal class which is an activity of Obstetric and Gynaecology Department, by providing a talk on traditional practice for post natal care. This program has been conducted once a month since 2009. Four (4) classes have been successfully carried out for the year 2010.

2. “2nd Seminar of Introduction on Traditional and Complementary Medicine”

T&CM Unit, Putrajaya Hospital with T&CMD has co-organized the seminar on 6th July 2010. This seminar was attended by MOH staff and members of the public. A total of 290 participants attended this seminar.

6.1 CONFERENCE / SEMINAR

6.1.1 INTERNATIONAL CONFERENCE ON TRADITIONAL INDIAN SYSTEM OF MEDICINE: AYURVEDIC MEDICINE – THE WAY FORWARD

The conference was held on 2nd March 2010 at One World Hotel, Petaling Jaya, Selangor. It was officiated by Deputy Director General Of Health, Y. Bhg Dato Dr Hassan bin Abdul Rahman on behalf of the Director General Of Health Malaysia. This conference is a collaboration between Traditional and Complementary Division Ministry of Health (T&CMD), Malaysian Association of Traditional Indian Medicine (MATIM)and Department of AYUSH, Ministry of Health &Family Welfare, Government of India.

A total of 175 local and foreign participants participated in this conference. Apart from that, 8 internationaly recognized speakers presented lectures on various topic in Ayurveda, Siddha and Unani Medicine. The speakers are from India, Britain and Malaysia. The Malaysian speaker is the T&CMD director, Dr. Ramli bin Abd Ghani. There were 5 advertisement booths in which T&CMD took up one booth for promotion of T&CMD activities. A gala dinner was held later in the evening.

6.1.2 INTERNATIONAL CONFERENCE ON ACUPUNCTURE: ACUPUNCTURE … TREATMENT WITHOUT DRUGS

Traditional and Complementary Medicine Division (T&CMD) of Ministry of Health held the conference on 3rd and 4th April at Grand Seasons Hotel, Kuala Lumpur in collaboration with acupuncturists from Australia and Singapore. The Conference was officiated by Y.Bhg. Dato’ Dr. Maimunah A. Hamid, Deputy Director General of Health, Malaysia (Research and Technical Support).

The objective of the conference is to share current information on practices of acupuncture & Malay massage, to exchange latest technology on practice of acupuncture, to establish networking and collaboration as well as to discuss further issues on research in acupuncture.

The talks were given by speakers from local, international and T&CMD. There were 14 papers presented on topics which covers broad spectrum of acupuncture practices, including the latest practices such as laser acupuncture in maternity care. Apart from lectures there were also workshops on 5 fields .The number of participants exceeded the initial target by 43% .Apart from local participants there were also participants from Australia, New Zealand, China, Singapore, Taiwan and Hong Kong. All the Malaysian T&CM practitioner bodies also showed their support by participating in this conference. The feedback from the participants was encouraging where many stated that they would like more similar conferences to be held in the future.

6.2 HUMAN CAPITAL / TRAINING

6.2.1 LOCAL TRAININGComplying with the requirements of Ministry of Health, Malaysia, several training and module had been planned for the year 2010. These training is to upgrade the knowledge and competency of staffs. The training given were:

1. Training Need Analysis (TNA)

On the 7th May 2010 till 9th may 2010, the T&CMD Training Comittee had underwent TNA Workshop conducted a private consultant at Heritage Hotel Cameron Highland. This workshop was attended by the Training Committee member, officers of Grade 48 above and all Head of Sections of T&CMD. The purpose of this workshop is to strenghten the training plan for T&CMD staff.

6.3 5S PROGRAM

Traditional and Complementary Division (T&CMD), Ministry of Health had embarked the 5S program. The program was officiated by Y. Bhg. Dr. Ramli Abd Ghani, Director of T&CMD, on 24 September 2010 at the main meeting room of T&CMD. It was attended by all T&CMD staff and a gotong-royong project involving all the T&CMD staff was conducted on the same day. T&CMD is preparing towards 5S certification by Malaysia Productivity Corporation in mid 2011.
