The Goods and Services shown below are those we can provide to our customers. You may choose
only the items you desire. However, any funeral arrangements you select will include a charge for our
basic services and overhead. If legal or other requirements mean you must buy any item you did not
specifically ask for, we will explain the reason in writing on the statement we provide describing the
funeral goods and services you selected.
"Prior to drafting any contract for goods and services, the survivor of the deceased who is handling the
Funeral Arrangements, or the responsible party, is entitled to receive a copy of any preneed agreement
in possession of the funeral establishment that has been signed and paid for, in full or in part, by or on
the behalf of the decedent."

Payment Policy
ACCEPTABLE METHODS OF PAYMENT
We will accept the following forms of payment: Cash, cashier check, bank money-order, non-contestable life
insurance policies, Visa, MasterCard, Discover or American Express. We do not accept out-of-state or personal
checks. All payments must be made at least 48 hours prior to funeral/memorial service or other service selected
by survivor/responsible party.

Funeral Goods and Services (Non Declinable Fee)
* Basic Services of Funeral Director, Staff and Overhead ………………………………… $2045.00
Basic service includes consultation with family, administrative and clerical services, coordinating with
cemetery, crematory, and/or other parties responsible for the arrangements made. A percentage of this fee is
necessary to cover a proportionate share of our established overhead costs such as licenses, insurance, and
indirect taxes (does not include sales tax on items in the merchandise category), maintenance of facilities and
equipment, utilities, and other costs related to being in business to serve the public. This fee is for our basic
services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is
included in our charges for direct cremation, immediate burial, forwarding remains, and receiving remains.)

Included in each Cremation Service Package price shown below is the cost of the services we will provide as
itemized. Our basic service fee has been adjusted. The Basic direct cremation fee includes an alternative
container (we use cardboard). Our Prices do not reflect any types of rental caskets, or cremation containers used
for viewings or ceremonies, nor do they include a permanent Urn. Standard refrigeration for up to four business
days for un-embalmed remains.

Miscellaneous/Cash Advance Items/Specialties
INSURANCE PROCESSING FEE
Families choosing to use insurance policies/assignments to pay for funeral services and merchandise will be
assessed a 5% insurance processing· on the total amount of insurance proceeds being used. Anderson-Ragsdale
Mortuary does not receive any portion of this fee. Anderson-Ragsdale Mortuary uses the services of an
insurance processing company to quickly process insurance claims and make funds available within the
payment schedule established by the mortuary. In order for the insurance processing company to process these
claims, the 7% processing fee will be included on your contract as a Cash Advance item because it is money
that is paid directly to the insurance processing company and not paid to the mortuary. Your counselor will have
you read this explanation before it is included in your contract. If you choose not to pay this fee, Anderson-
Ragsdale Mortuary will not be able to accept your policy to pay for the funeral service, but will file your
insurance claim(s) free of charge as a courtesy and an alternative acceptable method of payment for services
will be established.
A 9% fee may apply if the cause of death is undetermined or not known at the time the death certificates are
issued. The medical examiner may take from 6 months to 1 year to issue a cause of death. Most insurance
companies will not pay until a manner of death is known.
*We retain the right to refuse insurance assignments for payment when all merchandise is not purchased
through our mortuary.
*We do not accept contestable policies (policies issued less than two years)
