The Hospitals with Medical Schools and other Hospitals mentionedon the last page admit holders of Fellowship Post-Graduate ticketsto their General Practice, inelutding Clinical Instruction in the Wardsand Out-Patient Departments, Clinical Lectures and Demonstrations,P.M. Demonstrations, &c.

Tickets for any period can be obtained at the rate of one week,2 gus.; two weeks, 3 gns.; one month, 5 gns.; two months, 9 gns.;three months, 12 gns.; six months, 18 gns.; and one year, 20 gns.Holders of general tickets for one month, or longer, can take theSpecial Courses (except Cardiology) during the currency of the ticketat considerably reduced fees. Cheques should be made payable tothe Fellowship of AMedicine and erossed " Barclay & Co."

The best subsequent treatment is to wait for aday or two till the more acute symptoms havepassed off, and then to operate and remove thepiles and cut away some of the swollen externalparts and shell out the clots. This wants doingcarefully as, owing to the swelling of all the tissues,it is difficult to gauge the amount of tissue toremove. The advantages of immediate operationare that the patient is saved a considerable amountof time, and is up and about again much soonerthan would be the case if one waited to removethe piles till all the swelling had subsided.

Lastly, let me give a word of advice. Never treata case of piles without first examining the patientboth visually and with your finger. More medicalreputations have been made and jeopardised bycases of piles than by appendicitis!

WEST LONDON HOSPITAL POST-GRADUATECOLLEGE.

THE monthly staff consultation was held at theHospital on Monday, Oct. 12th, at 4.30 P.M.Thirty-eight post-graduates and general practi-tioners were present, and among the cases werethe following, which showed points of considerableinterest.

1. A boy of 4 years of age had been brought up tothe hospital by his mother because she had noticed thathe had been limping for a week or two. The boy com-plained of no pain and there was no history of any injury.On cross-examination it was found that he had had abad sore-throat rather more than two months before.On examination a swelling was found on the upper thirdof the right femur. It was slightlv tender and quite hard.The X ray examination showed an area of localised rare-faction about the size of a small walnut, surrounded bya considerable amount of sclerosed bone. The diagnosismade was chronic abscess and treatment advised wasoperative.

2. A boy of 12 years of age who complained of painover the left internal femoral condyle. The duration inthis case was also about three weeks and there was nodefinite history of injury, but tenderness was com-plained of. On examination the end of the femur wasfound to be definitely enlarged and the X ray photographshowed a certain amount of spiculation, especially con-centrated in a small area which seemed to be about aninch outside the internal condyle. There was also someperiosteal thickening and sclerosis round the diaphysis.The differential diagnosis in this case lay between asarcoma and syphilis. As the child had only been in theout-patients' department a few hours before, a Wasser-mann reaction had not been taken. The child was admittedfor further investigation.

3. A boy of 8 years of age had been brought up tothe hospital by his mother with a history that he had bada fall three days before, and she had noticed a tenderswelling on the outside of his left knee. On examinationa hard, fixed nodule was felt about the epiphyseal line onthe outer side of the left tibia. There was bruising onthe skin over it and it was tender. Further examination,however, showed similar masses in the head of the oppositetibia and also in the shoulder and elbow-joints. X rayphotographs were shown of these and a diagnosis ofmultiple familial exostosis was made. No special treatmentwas advised, as it was pointed out by the surgeon showingthe case that these exostoses seldom require removal,

unless by their position they interfered with tendons ornerves in their neighbourhood.

4. A boy of 6 years old was shown with a fracture of thelower end of the humerus. The fracture had taken placethree weeks before and the X rays showed considerablemalunion with forward displacement of the upper end,which was almost ulcerating through the skin of the frontof the arm. The point of interest in this case was thatcomplete flexion was rendered almost impossible by theprojection forward of the lower end of the upper fragment,and the discussion on the case centred round the problemas to whether it would be better to operate immediatelyin order to deal with the lower end of the upper fragment,or whether, in consequence of the nearly ulcerated con-dition of the skin in this situation, it would not be wiserto leave things as they were.

5. A very rare case of blastomycosis was shown. Itwas pointed out that this disease is practically unknownin England, but the patient had lived all his life withintwo miles of the hospital and followed the occupation ofa wood-chopper. He lived in a very damp room near theriver. The occupation and the surroundings are thosewhich are practically always found in the history of thosesuffering from this disease. The patient was beingtreated with large doses of potassium iodide and deepX ray treatment for the ulcers.

6. A man, aged 22 years, was shown with a painlessswelling about the size of half a walnut two inches behindthe left ear. There was a history of having been hitwith a cricket hall several years ago, and also a blowwhile boxing two years previously. The swelling wasgradually increasing in size, and the patient had comeup to the hospital complaining of the constant swishingor buzzing noise which it made in his head. On examina-tion it was obvious that the swelling was composed ofdilated blood-spaces, and the differential diagnosis laybetween cirsoid aneurysm and an arterio-venous aneurysmbetween the occipital artery and the lateral sinus.The pros and cons were very fully discussed by the

members of the staff present, but no doubt was expressedas to the treatment. The position of the swelling renderedthe patient liable to severe haemorrhage from any slightaccident, and the amount of inconvenience caused by thenoises in the head clearly indicated removal.

case, especially if it is acute, for he has to decide whetheror not prompt operation is essential. Mr. RutherfordMorison truily says: " The fate of a patient, the victim ofan abdominal emergency, depends chiefly upon the skill andpromptitude of his doctor." Teachers have, over and overagain, preached this, and the mortality among such victimscannot be lowered unless more and more doctors act as didthe writer of the letter given by the author on p. 61.We agree with Mr. Morison that this letter should be pastedup in a prominent position in the consulting room of everydoctor in the Empire.

This book is purely clinical; it relates almost entirely todiagnosis, no operation is described, most instructive casesare given, we are told when to operate and when not to doso, it is brief and yet not a mere catalogue of facts. All thesections show that the author has soundly and judiciouslygathered together the lessons of an immense experience.This he gives to the profession in clear language; each pageis interesting, and if everyone engaged in the practice ofmedicine would spend one evening, once a year, in readingthrough this small but delightful work, the gain to himselfand the community would be great.

EDITORIAL NOTES

IN the opening words of his article in the presentnumber, Mr. LOCKHART-MUMMERY proclaims thatthe days of the systematic lecture are over; itsdoom has been sealed by the profusion of medicalliterature, an efficient and, as he explains, a muchmore convenient substitute. Yet both his lectureand that of Sir THoMAs HORDER are strikingexamples in evidence of the value that the spokenword and the living personality of the lecturermust always possess. For we venture to believethat the presentation of these lectures is suchthat little is lost of the effect and impressivenessof their actual delivery. This has to a certainextent been achieved by publishing an almostverbatim report. It would be an affectation,however, to pretend that literary self-denial isresponsible for more than a small part of theparticular quality to which we refer. To a fargreater extent it must be contended that le stylec'est l'homme.

Sir THOMAS HORDER is happy in a subject ofperennial interest. An unexplained pyrexia hasbeen aptly described as " one of the mysteries ofmedicine." Doubtless cases will always occurwhen the mystery must remain unsolved even ifthe pyrexia most obligingly disappears. As SirTHOMAS HORDER points out, the skill, experienceand, most of all, the perseverance of the investigatorhave much bearing on what must be regarded as"unexplained." Although Sir THOMAS HORDERhas not attempted an exhaustive classification ofpyrexias, obscure and otlerwise, it is remarkablehow extensive is the ground he has found itpossible to cover in one lecture.

Mr. LoCKHART-MUMMERY'S lecture will also beread with eagerness. He, too, has selected one ofthose commonplace subjects on which any butthe most authoritative teaching must be avoided.He displays throughout a pleasing dogmatismwhere dogmatism is demanded with precisely thatdefiniteness and absence of ambiguity which thepractitioner will cordially welcome.

We have received a number of letters fromcorrespondents who have paid the Fellowship ofMedicine the embarrassing compliment of presumingits capability or at least ambition to revolutionisethe medical curriculum and even to redress certainhardships and grievances incidental to professionallife. In one instance it has been suggested thatimmediate pressure may be brought to bear uponthe Ministry of Health. We welcome the views ofcorrespondents and reiterate our anxiety to affordthem the opportunity to express their individual

requirements; but we are regretfully limited to,details which deal with post-graduate teachingand education. We have received several in--teresting letters which we hope to publish anddiscuss in our next issue.

The scope of the various courses differs veryconsiderably so that intending participants will bewell advised to obtain a syllabus of any course inwhich they are particularly interested. In someinstances the whole of the working day is occupied;_in other instances only a limited time; and insome, again, the classes are arranged on such days.that it is possible to attend two or more desirablecourses simultaneously. Certain courses have been-specially arranged for practitioners with leisure inthe late afternoon, among which may be speciallymentioned the General Course at the LondonTemperance Hospital and the course in neurologyat the West End Hospital for Nervous Diseases.
