Medicaid Services Generally

Services are provided on a medical model, not a social model.

CSA has authority to provide services on a social model, beyond the scope of what DMAS can fund.

For medical model services, use Medicaid funds first.

Freestanding Psychiatric Hospital

State freestanding psychiatric hospitals serving children: Commonwealth Center for Youth and Adolescents, Southwestern Virginia Mental Health Institute

Private Freestanding Psychiatric Hospitals include: Poplar Springs, Dominion Hospital, Virginia Beach Psychiatric Center, Riverside Behavioral Health Center, and others.

Residential Treatment Center (C)

Service definition: 24 hour, supervised, medically necessary, out-of-home programs designed to provide necessary support and address the special mental health and behavioral needs of a child or adolescent in order to prevent or minimize the need for more intensive inpatient treatment.

Must include: Assessment and evaluation, medical treatment, individual and group counseling, and family therapy necessary to treat the child.

Group Homes

Level A, Level B

A combination of therapeutic services in a residential setting. Provides structure of daily activities, psychoeducation, therapeutic supervision, and psychiatric treatment.

Level A – licensed by DSS. Weekly individual therapy by licensed MH professional required.

Level B – licensed by DBHDS. Weekly individual and group therapy by licensed MH professional required.

Service authorization required for admission.

• Outpatient Substance Abuse Treatment

Mental health or substance abuse counseling by a licensed or certified professional in an office setting.

No authorization required for first 26 sessions in first year of treatment. In first year, an additional 26 sessions available with service authorization.

Subsequent years – limit of 26 sessions per year.

Independent Clinical Assessment

Required for Intensive In-Home; Therapeutic Day Treatment; Mental Health Support Services for individuals under 21.

An assessment by a licensed mental health professional at the local CSB. Assessment will recommend/not recommend certain services. Assessment is forwarded to service authorization contractor for review and determination on service eligibility.

Intensive In-Home

Service definition: Time-limited interventions provided typically but not solely in the residence of a child who is at risk of being moved into an out-of-home placement.

Services include: crisis treatment, individual and

family counseling, communication skills, case management activities, coordination with other required services, 24-hour emergency response.

Service authorization required. Granted for up to

12 weeks.

Therapeutic Day Treatment

Provides evaluation, medication, education and management, opportunities to learn and use daily living skills and to enhance social and interpersonal skills, and individual, group, and family psychotherapy.

Often provided during the school day, in an after-school program, or in a summer program.

Service authorization required. Granted for up to 6 months.

Mental Health Support Services

Training and supports to allow older adolescents (and adults) to achieve and maintain community stability and independence.

Provides training in and reinforcement of functional

skills and behavior related to the individual’s health and safety, activities of daily living, and use of community resources; assistance with medication management; and monitoring health, nutrition, and physical condition.

Service authorization required. Granted for up to 6

months.

Mental Health Case Management

Assists individual children and adults with accessing needed medical, psychiatric, social, educational, vocational, and other supports essential to meeting basic needs.

Includes assessment and planning, linking to services and supports, assisting with locating services, coordinating services, arranging community access and involvement, follow-up and monitoring, education and counseling.

Provided only by CSBs.

Service authorization required.

Crisis Intervention

Immediate mental health care, available 24 hours a day, seven days a week, to assist individuals who are experiencing acute psychiatric dysfunction requiring immediate clinical attention.

Includes assessing the crisis situation, providing short-term counseling to stabilize the individual, providing access to further immediate assessment and follow-up, and linking the individual and family to ongoing care to prevent future crises.

Service authorization not required currently.

Crisis Stabilization

Direct mental health care to individuals experiencing an acute psychiatric crisis that may jeopardize their current community living situation.

Includes psychiatric assessment including medication evaluation, treatment planning, symptom and behavior management, and individual and group counseling.

Service authorization not required currently.

Treatment Foster Care – Case Mgmt

Services are designed to address the special needs of children. Services to the children are delivered primarily by treatment foster parents who are trained, supervised, and supported by agency staff. Treatment is foster-family based and is delivered by a treatment team.

Includes assessment to determine need for psychosocial, nutritional, medical, and educational services; service planning; coordination and referral; follow-up and monitoring; education and counseling.
