An overview of Medicines Reconciliation

An Internet-based training package from CoAcS Ltd

Purpose

Written in conjunction with the South West Medicines Information and Training group (SWMIT).

The package delivers an online instructional guide covering the process of medicines reconciliation and prepares learners with the skills and knowledge necessary to enter into a more detailed training programme.

Three user roles

• Organisation lead– usually just one person (for system operations)– creates user accounts and assigns assessors to

locations / groups• Assessors

– set up and approve learner access– provide feedback on diary log content– award certificates

• Learners– access to training materials, reflective diary and

assessments

Setup

Assessors and Learners are created at Locations by the Organisation Lead for each Trust site.

User profilesAll Learners must be given a user name and password and be approved before they can access the materials.

Four modules

There is an introduction and then two Levels of training material.

There are also some personal and professional pre-requisites to be attained before a learner is entered onto the system.

Learning materials

Each learning activity is explained and presented in a logical order over a number of screens and pages.

Each page has further resources available including documents, references and electronic links.

Reflective diary log

A reflective diary log is available for each learner to record their learning experiences. These might be used for CPD purposes.

Assessment

Learners are then assessed on their knowledge of the learning materials.

There are 20 multiple choice questions selected at random for Level 1 and 10 questions for Level 2.

Results

On completion, learners will see the results of their assessment. The pass rate is 80%.

Certificates

Once the learner has demonstrated a suitable level of competence and Level 1 is complete and a certificate issued, the Level 2 assessment becomes accessible.
