Accident Care
o An on-the-job accident occurs every eight seconds
o Approximately 22% of the injury visits to an emergency room are the result of
sports-related activities for persons between the age of 5 and 22
o Helps pay the direct (medical) and indirect (non-medical) expenses that may result from a covered accident.
o Provides benefits for initial care and treatment, in addition to some follow-up care that you may need.

Accident Care
Initial Care - $150 ER benefit, $100 Ambulance benefit, $50 Doctors Office benefit
Injury Benefits – Broken bones, dislocations, burns, concussions, lacerations, and more
Hospitalization - $750 on admission, $200 daily for 365 days
Other benefits – Follow-up doctor visits, physical therapy, wheelchairs, crutches, accidental death
Optional benefits include – wellness benefit, sickness hospitalization and spouse disability
Off-Job Only or On/Off-Job benefits are available. On-the-Job benefits DO NOT interact with workers compensation.
The base accident plan required NO HEALTH QUESTIONS

Medical Bridge 3000 - Hospital Confinement Indemnity Insurance
Designed to supplement your existing major medical coverage.
Lump-sum benefit helps you to pay medical and non-medical expenses associated with a hospital stay, such as:
Deductibles or co-payments.
Transportation to and from the hospital.

Hospital Confinement Indemnity Insurance
Lump-sum benefit helps you to off-set the expenses associated with hospital related services, such as:
Hospital Confinement – pays a $500 per in-patient admission
Outpatient surgery - $500 or $1000 depending on procedure ($1500 annual maximum)
$250 for hospital based diagnostic tests (payable once annually)
$150 ER visits for accidents or sickness (payable once annually)
Wellness benefit - $50 annually for 18 different tests
Rehabilitation benefit - $100 daily for 15 days (30 days annual maximum) o Coverage available for Employees, Spouse and Children o GUARANTEED ISSUE for new employees!!

Hospital confinement - $200 daily for the first 30 days
After 30 days, $400 daily

Maximum confinement/ICU benefit = 180 days
Radiation/chemotherapy - $225 daily, $7500 annual maximum
Monthly maximums' apply depending on delivery method
Transportation – 50cents/mile for $700 mile round trip. Payable for insured and traveling companion
Lodging - $50/night for 70 nights
Additional First Diagnosis benefit, lump sum payable upon diagnosis of any internal cancer.
