To be part of HOM Cares:1. During the admission process, provide the email address of the
primary caregiver. Or you may give it to a Hospice of Michigan nurse, social worker or spiritual care adviser when convenient.
The primary caregiver’s email address will be entered into the patient’s electronic medical record. Within 24 - 48 hours, she or he will receive an email from HOM Cares inviting that person to download the HOM Cares mobile app. The message will include a temporary password.
2. Download the app on the phone (see instructions below).3. On the login screen, enter your email address and the temporary
password. You will be prompted to enter a personal password.4. Once you are logged in, scroll through the features of the app.
Members of your loved one’s care team will be listed, along with profiles and photos.
5. At a time that is convenient to you, set up a network of family and friends whom you wish to receive regular updates. Every 24 hours, the system sends updates on visits made by the team.

When a loved one is ill, families want to be informed about care, and they want to stay connected. Regular updates provide peace of mind to family members – whether they are near or far.
Hospice of Michigan developed a mobile app called HOM Cares that sends updates to the primary caregiver and to the network of family and friends that the primary caregiver sets up.
