Cosmetic Dentistry – If you’re looking for a smile-makeover, Dr. Edward Warr is highly skilled and trained in the latest dental procedures to give you the smile of your dreams! We o�er porcelain veneers that will improve your smile and boost your con�dence.

Wisdom Teeth – Wisdom teeth often require removal due to a lack of available space and poor hygiene. Dr. Warr is skilled in tooth removal and can comfortably remove most wisdom teeth.

Dental Implants – Implants are an excellent solution for replacing missing teeth. Dental implants work and feel like your real teeth and look perfectly natural.

Teeth Whitening – We can provide you with whitening trays, custom �t to your teeth, so you can whiten your teeth in the comfort of your own home.

Periodontics – Dr. Warr will identify your needs during your examination and recommend the best treatment and solutions for your long term gum health.

Dr. Edward Warr, DMDDr. Edward Warr graduated from the University of Louisville School of Dentistry in 2002. He is a Fellow in the International Congress of Oral Implantologists - the oldest and largest dental implant education organiza-tion in the world. Dr. Warr continually seeks more education in all disciplines in dentistry. Currently he is engaged in additional

surgical training for dental implants and is working toward earning Diplomate status in the International Congress of Oral Implantologists. Dr. Warr is a past president of the Southern Oregon Dental Society.

• The plan can NOT be used in conjunction with traditional insurance.

• The plan is active from the date of purchase by the primary member for one year.

• A Family plan involves (up to) 2 parents and children through the age of 20 (or 26 if they are college students). It does not apply to adults who are still living with their parents.

• On a Family plan the expiration date is the date the �rst member signed up for the current membership, regardless of when additional members are added.

• It is ultimately the member’s responsibility to get their appointments scheduled in a timely manner.

• The membership plan discounts do NOT apply to any other o�ce.
