The Extraction Of Teeth

Objectives :

The student should be able to

Define the procedure of tooth extraction.

Enumerate the indication & contraindication

Evaluate the patient.

Defination :

The ideal tooth extraction is the

painless removal of the whole tooth

or tooth root, with minimal trauma

to investing tissues, so that the

wound heals uneventfully and no post

operative prosthetic problem is created.

Indications :

Pulp pathologic conditions

Periodontal disease

Traumatic effects (during fracture)

Impacted or supernumery teeth

Orthodontic consideration

Devitalized teeth

Prosthetic considerations

Esthetics considerations

Pathologic condition in surrounding bone (e.g. cyst, tumors, osteomyelitis)

Cyst “In a line of fire” (Radiotherapy)

Contraindication

Local contraindications :

Acute infection with an uncontrolled cellulitis

Acute pericoronitis

Acute infectious stomatitis

Malignant disease

Irradiated jaws

Systemic contraindications :

Uncontrolled diabetes mellitus

Cardiac disease

Blood dyscrasias

Debilitating diseases

Addison’s disease (any steroid deficiency)

Fever of unexplained origin

Nephritis requiring treatment

Pregnancy

Senility

Psychoses & neuroses

Methods of Extraction :

Forceps extraction “Intra-alveolar extraction”

Surgical method “Trans-alveolar extraction”

Mechanical Principles of Extraction:

Expansion of the bony socket

The use of lever & fulcrum

The insertion of wedge or wedges

Pre-oprative Assesment :

A history of general disease

Nervousness

Anesthesia

Pre-medication

Previous difficulty with extraction

Examination of the Patient :

Clinical examination

Radiographic examination

Blood pressure determination

Laboratory test

Objectives :

The student should be able to

Define the procedure of tooth extraction.

Enumerate the indication & contraindication

Evaluate the patient.
