Final Medical and Dental Exam Memorandums Member will schedule an appointment on the

appointment line for final medical and dental exams.

Memorandums will need the doctor’s and dentist’s signature.

Applying for DD 214

Service Members go to vMPF on the AFPC Secure website to submit application for DD Form 214.

Provide source documents to the MPS for update of missing or inaccurate data such as training certifications, decoration orders, promotion orders, TDY orders, etc.

The member receives an email indicating it is available through the AFPC website and are normally processed and completed within two weeks of the member’s official retirement/separation date.

To request a copy call the TFSC at DSN 665-0102 or commercial (210) 565-0102

Proof of Service Letter

Proof of service letter is the equivalent to a Letter in Lieu of DD 214.

Found on vMPF Click Self-Service Actions. Click Personal Data. Proceed to Proof of Service Letter Tab, complete form and

submit.
