HAZOP STUDY

LOCATION – PROJECT MANAGEMENT OFFICE

GULF SPIC

OBJECTIVE:

To Identify the hazards with respect to the various severities of the pre defined parameters and recommend suitable mitigation/Control measures to eliminate to the extent of possible level.

METHOD OF STUDY

PARAMETERS CONSIDERED FOR STUDY:

Wind Temperature Humidity Fire Rain Theft Sabotage Violence Traffic Breakdown Shutdown Lightning Animals Air Quality

SEVERITY:

The pre defined parameters were classified in to Low, Medium and High severities based on their Causes and impact to the areas covered in the study.

Ground leveling, Asphalt road Reflectors on all Vertical columns of Car parking

Shelter.  Mirror at the corner of road Reserved Car parking Lighting at Main entrance. Lighting for Name board. Plant trimming in front of the Name board Leveling and Compaction at the junction of Main

road. Speed breakers at the junction of Main road

Permanent Lighting for the temporary car parking shelter 

Additional car parking shelter in front of the new Material yard

Smoking Shelter – 1 : Permanent Lights   Display of Caution Sign Boards

South side walk-way : Illumination to be improved Walkway width to be increased

Water tank Area : Water Pump starter to be covered Drinking water Quality Regular House Keeping

PMO OFFICE :

Reception : Exit Sign Board       Emergency Preparedness Training Emergency numbers to be displayed Fire fighting training should be provided Periodical electrical preventive

maintenance to be ensured Unwanted holes & openings to be closed

in all cabins Regular housekeeping. Emergency Exit Route display

PMO OFFICE :

Document Room : Data Backup to be secured outside of

PMO. Documents to be kept above floor level. Document Rack not to be kept under

the Air conditioning unit CCTV Security System to be installed Fire fighting training Duplicate office keys in different

location. Restrict Unauthorized person's entry

PMO OFFICE :

Office Rooms : Locker for accounts section. Smoke detectors in accounts Room.            

                                          Periodical Electrical preventive

maintenance Emergency exit for new office (1st floor) Regular housekeeping

PMO OFFICE :

Xerox & Server Room : Proper access to be ensured

Pantry Room : Display do/don’t do's instructions Flammable material should not be stored Flammable stove / coil stove should not

be used Heat / Smoke detector to be provided Provide Pest o flash Light   Regular housekeeping

PMO OFFICE :

Staff Toilets : Cleaning frequency to be increased &

Pest Control to be ensured Smoke detector Additional Toilet

Staircase : Reflecting Stickers on steps Emergency light in new office entry

PMO WORKSHOP:

PMO Workshop Area : Insurance Coverage shall be ensured       Close the gap between new office & main

office Water cooler’s cable to be changed Relocation of water heater near to

staircase Overall cable management to be improved Electrical cable dressing to be ensured Access way shall be free from obstacles Entry / Exit sign boards

PMO WORKSHOP:

Removal of unwanted electrical cables Removal of existing exhaust fan (wall

mounted) Fire Hydrant system to be provided EOT Crane stopper (l/t) shall be strengthen Periodical electrical maintenance Unwanted holes & openings to be closed in

the walls / floors Regular housekeeping

PMO WORKSHOP:

PMO Valve Shop : Focus lights to be changed Proper arrangements of material storage Pressure equipments sign boards to be

provided Structural earthling should be done Supports for pressurized water hoses to be

provided Piping Material open yard : Proper Arrangement of materials Access from the rear side of the yard should

be free from obstruction Regular housekeeping

PMO OUTSIDE PORTO-CABINS & CONTAINERS :

Security Cabin : Emergency numbers to be displayed Locking system key stand for office keys Regular housekeeping Fire fighting training Anchor bolts to be provided for

mounting cabin rigidly              Electrical Preventive Maintenance to be

Ensured Security cabin bottom floor opening to

be closed

PMO OUTSIDE PORTO-CABINS :

Porto Cabins : Fire alarm System         Unwanted holes & openings to be closed Provide Pest o flash Light in dining hall  Regular housekeeping Removal of Coiled type stove & Induction

stove to be used if required. Unused window type A/C to be removed. Vehicle keys should have easy

identification and access in case of emergency evacuation

PMO OUTSIDE CONTAINERS :

Storage Containers : MSDS to be displayed for all chemicals. Proper arrangements of material &

tools Fire alarm system to be provided      Electrical preventive maintenance to be

ensured Permanent Lights to be provided Regular Housekeeping

PMO CIVIL YARD :

Anchoring to be done for the security cabin.   

Air conditioner to be provided               Emergency numbers should be displayed Fire Extinguishers to be provided           Unwanted holes & openings to be

closed.

PMO CIVIL YARD :

Smoke detectors Proper Arrangement of materials Separate Area For Paint storage along

with MSDS display. Proper fire protection / fire fighting

system for Paint storage area.

PMO CIVIL YARD : Unwanted Scrap materials are to be

removed Proper Sign boards for fire retardant

materials Permanent Lights to be provided in

smoking shelter Daily housekeeping to be ensured
