THROMBOANGITIS OBLITERANS (BUERGER’S DISEASE)

Definition : Diffuse inflammation of arteries and veins with clot formation.

Etiology : unknown

Risk : cigarette smoking, genetic, males

S/ Sx. : intermittent claudication, edema ulceration, and gangrene

Management:

Arrest progress of the dse.Produce vasodilationRelieve painProvide emotional supportWound care Amputation

Raynaud’s disease

Definition : vasospasm causing ischemia

Etiology : unknown; related to release of serotonin, hypersensitivity to

cold

Risk : cigarette smoking, congenital, women

S/ Sx. : numbness of fingertips, pallor, CRT, cyanosis, ulceration

Management:

Promote vasodilationAvoid exposure to coldsStop smokingsympathectomy

VARICOSE VEINS

Definition : incompetent valve of the vein in the lower extremities

Risk : prolonged standing, pregnancy, trauma

Dx test : doppler test, trendelenburg test. phlethysmography

VARICOSE VEINS

S/ Sx. : edema, venous distention, stasis ulcer( medial aspect), muscle cramps, heaviness,

Management:

SclerotherapyVenous strippingVenous ligationElastic compressionElevate extremitiesLeg exercise

DEEP VEIN THROMBOSIS

Definition :clot in an inflammed vein

Risk : Virchow’s Triad

Dx exam : venography, UTZ

S/ Sx. : swollen limb, + homans sign, calf stiffness, induration of the vein

Management:

Thrombolytic therapyAnticoagulant therapyProper leg positioningProvide comfortPrevent complicationAvoid green leafy vegetables
