Quick screening - lower Extremity

▪ Always observe even before your patients sit down with you so that you can judge

how to use your time most effectively

▪ How do they walk in?

How do they sit down? How do they get up from sitting?▪ Usually, you can tell how the lower extremity works by observing and interviewing the

patient about occupations that are difficult for them to participate in. Ask, “What

are the main things that you have difficulty with during the day?”

▪ Problems getting down - Butt and Quads (possibly trunk instability)

Quick screening - upper extremity

▪ Ask the patient, “How far are you able to lift your hands over your head?…Open your

hands.”

▪ Place your hand on their scapula and see how/if the scapula is moving correctly▪ Ask, “How far are you able to lift your arms up to the side?…Touch the back of your

head” (External rotation and abduction)

▪ “Touch the back of your waist.” (Internal rotation)

▪ Distal - Flex elbows, supinate and pronate while making a fist and undoing a fist

▪ Have them then move the wrist around for flex/extend and deviations

Proximal Muscle Strength

1. Lift shoulders up (“Don’t let me push you down”)

2. Under elbows (“Push down into my hands) (Being able to push up out of a chair)3. Resistance in front of biceps (Flexion)

3. Resistance on side of upper arm (Abduction)

4. Resistance on back of arm triceps area (Extension)

Distal Muscle Strength - Elbow Flexed

1. Press on distal supinated wrist (Elbow (On top)flexion/(On bottom)extension)

2. Internal and external rotation of shoulder (“Turn your arm in and out”)

3. Pronation/Supination - (Don’t stress wrist - Use normal MMT not Quick screen)

4. Grab palm and thumb of dorsal surface - Flex and extend wrist - Ask them to resist

each way and then squeeze your hands as much as they can.

Document quick screen in notes and then document what needs to be followed up on

Use meters also for quick screening for hands and wrists.
