As the fall weather approaches we would like to remind clients to
dress appropriately. We will be having many events and programs

ANNUAL GENERAL MEETING
The Storefront Humber Inc. Annual General Meeting will be tak-
ing place on Thursday September 10th, 2015 at 5pm. All clients
and staff are welcome to participate. We look forward to seeing
you there. The meeting will be located at the Royal Canadian Le-
gion (Branch 101, Long Branch) 3850 Lake shore Blvd. West.

Storefront Humber is proud to announce we have been accred-
ited with commendation by Accreditation Canada! This award
is presented to organizations that go beyond the requirements
of Accreditation Canada and are commended for their commit-
ment to quality improvement. Accreditation helps health care
organizations improve their performance for the benefit of their
clients and the health system. We would like to thank all board
members, staff, clients and volunteers who helped prepare us
before and doing the Accreditation process. We are excited to
announce that we have been accredited through to 2019 at all
our locations.
Health care and social service organizations use the Accredita-
tion Canada standards to develop their services and thoroughly assess all that they do. The system-wide
standards address governance, leadership, infection prevention and control and medication management.
These standards focus on safe and effective health care based on five attributes of excellence—clinical
leadership, people, process, information and performance.
ACCREDITATION WITH ACCOMMODATION
Our new Home Help Coordinator Monika is a recent graduate at George
Brown College where she specialized in Human Resource Management and
Health & Safety. She is an enthusiastic learner and continues to further pur-
sue her education. Aside from academics, Monica has a six year old daugh-
ter who loves to perform gymnastics. She is an avid traveler whose been to
Cuba and Dominican Republic more than 10 times. We are excited to have
her a part of the team!
WELCOME MONIKA
We would like to welcome Veronica to our team! Veronica is a Registered
Nurse. She has a degree in B.Sc. Nursing since 2012 and has worked for
non profit organizations since she came to Canada. Veronica believes that
the practice of nursing is an art form and takes team work to be successful.
On top of her career achievements, Veronica speaks multiple languages and
is interested in foreign cultures. Welcome to the team Veronica!

National Seniors Day
is a way for Canada to
celebrate seniors for
their many and varied
contributions to their
families, workplaces
and communities. Cel-
ebrating the day annu-
ally will help raise
awareness about sen-
iors’ contributions and
the important roles
they play in Canadian
Society.

one of our congregate
dining programs on

The Pan Am/Para Pan Am games offi-
cially kicked-off with the passing of the
torch. The torch bearers stopped along
Mimico Square and continued along
Lake Shore. Members of Storefront
Humber were able to take pictures of the
torch passing as it past by.
Now that the games are over, we would
like to thank you during this time for
being patient while roads were closed
and/or congested. HOV lanes are now
gone, however construction on the Gar-
diner has begun and is expected to finish
in October. Please be mindful of this
when booking medical appointments
and give yourself enough time to reach
your destination.
TORONTO PAN AM TORCH
Our seniors visited what is often described as the prettiest town in Ontario,
Niagara-on-the-Lake! The town is a well-preserved 19th-century village,
along with its upscale restaurants and elegant architecture, and is situated in
the heart of Ontario’s wine region. Niagara-on-the-Lake features the world
class Shaw Festival, boutique shopping, glamorous hotels, Charming Inns,
historical sites etc…
NIAGARA ON THE LAKE TRIP
We are happy to announce that we were presented the LAMP Merit Award
June 2nd 2015 for the Home At Last Program! This award recognizes and
celebrates people who make a difference in the Lake Shore. The staff mem-
bers who accepted the award were Brenda Fairley, Marisol Grant, Liz
Keating, Edwin Laucy, Tomoko Montgomery and Chris Pawlak.
The Home at Last program is an innovative partnership between hospitals
and community support service agencies. It is a time limited service to tran-
sition hospital patients home safely, smoothly and comfortably following
an emergency department visit or hospital stay. When the patient is dis-
charged, transportation services arrives along with a Home At Last worker,
who rides home with the patient, gets the client settled in, makes a meal or
snack, and stays until family arrives or until a set time.

Flu season is fast approaching! It can occur in the fall and winter. Seasonal flu
activity usually peaks in January or February, but it can occur as early as Octo-
ber and as late as May. This is a friendly reminder to wash your hands often,
cover your mouth and nose with a tissue or your elbow, avoid touching your
face and keep common surfaces and items clean and disinfected.
BE AWARE OF FLU SEASON
Storefront Humber will no longer be accepting cash from clients. All payments
must be made by cheque or money order. You do not have to pay until your bal-
ance owing is more than $20. A Personal Support Worker is available to assist
you if you require help going to the bank—Finance Team
STOREFRONT HUMBER NO LONGER ACCEPTING CASH
HOME AT LAST
Home At Last is a pro-
gram designed to help
seniors reach their homes
and settle back in safely
after having spent time at
the hospital. The program
involves Personal Sup-
port Workers who meet
up with clients upon dis-
charge from the hospital
and escort the senior to
the vehicle. Services are
free of charge and are
covered by the Ontario
Ministry of Health and
Long-Term Care.

Wash Your Hands Properly: Infection Prevention
1. Wet Hands
2. Apply Soap
3. Lather for 15 seconds. Rub between fingers, back of hands, fingertips, under nails
4. Rinse well under running water
5. Dry hands well with paper towel or hot air blower
6. Turn taps off with paper towel, if available
Always wash your hands after you: sneeze, cough, or blow your nose, use the wash-
room or change diapers, handle garbage, and before you: prepare or eat food and
touch a cut or open sore.
