The Desert Parkway Transitions program offers

specialized treatment for older adults via our

inpatient service, partial hospitalization and

intensive outpatient. The Transitions program is

designed to assist the older adult population with

life changing evnts such as retirement, divorce,

onset of chronic illness and loss of independence.

Desert Parkway’s specially trained staff will provide

care geared to the clinical needs and interests of our

older adult patients in a private, secure, comforting

and healing environment. Our goal is to partner with

patients, their families and providers to develop an

integrated, holistic treatment plan that will address

the needs of all our patient’s. Our objective is to

educate our patients through an interactive and

collaborative approach on different techniques to

manage illness, grief, loss or life changes through a

comprehensive wellness recovery plan.
