My Evaluation Appointment

I will go the Thompson Center to work with some people.

I will follow directions. I will stay calm and be safe.

First, I check in at the front desk.

Then I wait in the waiting room. I can play with toys or watch a movie.

Next, the nurse will call my name and I go with her. My family can come with me. I get my weight, height, head, and blood pressure measured. This does not hurt.

Then, I go to a room where I will wait for the adults who will work with me. I will see several adults.

I may go with an adult to answer questions and do activi-ties. My family may stay and talk to another adult.I can follow directions, stay calm, and try my best.

I can ask for a break if I need one.

An adult may talk to my family. I can be patient, stay calm, and be safe.

A doctor may give me a check-up. Usually the doctor will listen to my heart beat and my lungs breathing in and out. This does not hurt.

Sometimes the doctor shines a light in my ears, nose and eyes to make sure I am healthy and clean.

When I am finished, I can get a sticker from the front desk for being a good patient.

I am all done!
