2. Avatar Resources – Staff Connection/HIM Website, Help Desk Tickets, and Avatar Super Users (ASUs). The Staff Connection/HIM website contains information about new features, tips and tricks, and News Flash emails!

3. Avatar Development/Enhancement Requests should besubmitted to your local ASU. All requests will be presented to the Avatar Core Team (ACT) for review and approval.

required rollout components.

2. Follow PowerPoint slides and take notes as needed.

3. Training materials are available on the Staff Connection/HIM website, Barium Springs University, and Relias.

1. Select the client. (Enter the client’s last name in Search Clients field and double click on client’s name.)

2. Access the Crisis Plan form. (Enter “Crisis Plan” in the Search Forms field and double click on the form name.)

3. If this is NOT the first Crisis Plan for the client, click Add in the pre-display. (Information from the previous Crisis Plan will default into the form.)

If no other Crisis Plans have been entered, a blank form will display.

1. Enter the Date of Crisis Plan.

2. Enter the Significant event(s) that may create increased stress and trigger the onset of a crisis.

3. Enter Crisis prevention and early intervention strategies that were effective.

4. Enter the Strategies for crisis response and stabilization.

5. Enter the Description of the systems prevention and intervention back-up protocols to support the individual.

6. Enter the Specific recommendations for interacting with the person receiving a Crisis Service.

7. Save the information entered by clicking the Backup Form button.

8. Review the form for accuracy.

9. Select Draft or Final.

10. Click Submit.

Viewing and/or Printing a Crisis Plan

1. Access the client’s Chart View.

2. If not already added, add the Crisis Plan to your My Forms list using the Customize Form functionality.

3. Double-click the Crisis Plan in the My Forms list. The form will then display in Chart View. The Print option in the top right corner of the Crisis Plan window.

NOTE: If a Crisis Plan has been completed as a part of a Standard Treatment Plan or Person Centered Plan, it will automatically print with the Standard Treatment Plan Report or Person Centered Plan Report.

1. Select the client.

2. Access the Crisis Plan.

3. Select the correct Date of Crisis Plan to edit.

4. Click Edit.

NOTE: Only plans in Draft status can be edited.

5. If the Restore Backup Data pop-up message displays, answer Yes or No. (To return to the form with data previously entered in the form, select Yes. To start with a blank form, select No.)

6. Edit any of the fields as needed.

7. Review the form for accuracy.

8. Select Draft or Final.

9. Click Submit.

1. Access a client.

2. Access the Crisis Plan form.

3. Complete the fields as outlined in the previous instructions.

4. Submit the Crisis Plan.

5. View the Crisis Plan via the client’s Chart View.
