Contract Amendments

On August 12, 2011, the Florida Department of Education authorized Miami Dade County Public Schools (M-DCPS) to select premethods and postmethods for measuring the learning gains of students receiving Supplemental Educational Services (SES).

Chancellor Supplemental Educational Services, LLC has been contracted to conduct the pre and post assessments of all SES students in M-DCPS.

The School Board agrees to provide compensation to the Provider upon receipt of a complete and correct invoice as described in Section II. AA at a rate of $_________ per hour for each student. An instructional session shall be _________ minutes in length.

The 2011-2012 State-approved SES per pupil allocation for Miami-Dade County is $1,369.00. This amount will be reduced by an annual fee of $85.80(the per pupil cost of the pre and post assessment) for a net maximum per pupil payout of $1,283.20. In no event, shall the total cost per year for any individual student exceed the net maximum per pupil payout.

The Provider agrees that the School Board shall determine the per and post assessment for measuring learning gains of students enrolled in SES for the 2011-2012 school year.

The Provider further agrees that the third party assessment contractor selected by the School Board shall conduct the pre and post assessment of all students assigned to the Provider by the District.

The Provider also agrees to adhere to all district established timelines for the pre and post assessment of students.

The Provider shall be responsible for the cost associated with the pre and post assessment of all students assigned to the Provider by the School Board for the 2011-2012 school year.

Only the official, district-issued SES application shall be processed for student assignment by the School Board. It is the parent’s sole responsibility to complete the SES application.

The Provider agrees that it shall not modify, replicate, or complete the SES application. Any SES application completed and/or altered by a Provider, or Provider’s agent, shall be declared void and will not be processed for student assignment.

Student Learning Plans (SLPs)

Development of the SLP and Accessing the SLP Addendum

Providers will receive training on the development of the SLP and accessing the SLP Addendum during the Mandatory Technical Assistance Training for Providers, scheduled for September 12 –19, 2011.

Student Learning Plans (SLPs)Providers will be required to:

• Contact the parents of all students assigned to develop the schedule of tutoring.

• Submit an SLP online for all students assigned to their company prior to the start of services.

• Meet with the parent of students assigned subsequent to the administration of the pretest to obtain the parent signature on the SLP.

Student Learning Plans (SLPs)

• Provide a copy of the SLP and SLP Addendum to the Parent.

• Submit to the District Title I Administration office the original SLP Signature Page or Record of Attempt to obtain the Parent Signature Form prior to invoicing the District for services provided to the student.

Chancellor will be using a nationally normed, standardized achievement test (the SAT 10) in order to assess the learning gains of students participating in the SES program.

Test Coordinators will be hired by Chancellor to perform the pre and post testing of all students enrolled in SES.

Test Coordinators hired by Chancellor will:

Notify parents of the dates and times of the pre and post assessment.

Coordinate and monitor all procedures related to the administration of the pre and post assessment.

Assemble and deliver test materials to Chancellor.

Conduct make-up testing.

Test Coordinators are precluded from working with any SES Provider.
