INVITATION TO APPLY FOR ELIGIBILITY

The Bureau of Customs through its Bids and Awards Committee (BAC) herein invitesgeneral contractors to apply for accreditation and eligibility for CV 2013 Infrastructure Projects.Interested contractors are required to submit their company profile and the followingdocuments for evaluation not later than February 28, 2013, to wit:

• DTI Business Name Registration and/or SECRegistration;

• Valid and Current Mayor's Permit/Municipal License:

• Audited Financial Statement

• 20!! Income Tax Return (form 1701/1702) and Business Tax Return (forms 2550M and

2550Q) for the last six (6) months (January to June 20l2) filed thru Electronic Filing and

Payment System (EFPS).

• Contractor's License issued by Philippine Contractor Accreditation Board (PCAB);

• PHILGEPSRegistration;

• BIR Registration Certificate;

• Photocopy of Accountant Certificate / Net Financial Contracting Capacity (NFCC);

• Certification of Non-Inclusion in the BLACKLISTEDContractors from DTI;

• List of On-Going & Completed Projects for the last 3 years.

• A sworn statement attesting compliance to the responsibilities of a prospective or

eligible bidder as provided in Section 17.7 ofthe IRR-A (Notarized)

All contractors who will pass our eligibility requirements will be allowed to bid on ourfuture infrastructure projects.
