Airway

Interactive CPD Series | Episode 5 | Spotting the Sick Child

• Smaller face

• Compressible floor of the mouth

• Pressure on the eyes can cause slowing of the heart rate

• Mask sizing is therefore important!

• Obstruction of the airway is a common cause of death

• Chest movement does not always mean breathing

• Look, listen and feel for breathing

• Listen for stridor and/or wheeze

358 431

Breathing

Interactive CPD Series | Episode 5 | Spotting the Sick Child

• Smaller lungs means a reduced reserve volume

• Weak accessory muscles – breathing relies on the diaphragm

• High baseline metabolic rate – increases when unwell

• Increased respiratory rate is often the first sign

• Look for abnormal sounds – stridor, grunting, wheeze

• Recession, particularly >5 y/o is a more worrying sign

• Nasal flaring can occur in young children

• Accessory muscle use – ‘head bobbing’

359 431

Circulation

Interactive CPD Series | Episode 5 | Spotting the Sick Child

• 80ml/kg – a newborn will only have around 240ml of blood

• Increasing heart rate is the only way to increase blood pressure

• Children are able to compensate well for shock

• Blood pressure is of little use in practice

• Bradycardia is a serious sign – often a reason to start CPR

• Capillary refill time is a useful indicator

• Skin temperature and colour are also useful
