WITH OR WITHOUT CORRECTIONS IN PAN CARD DETAILS ?

CLICK HERE

Select the category of the entity for which the PAN Card is being appliedFor example, in a case of a Person select Individual.

SELECT‘INDIVIDUAL’

CLICK ON‘SELECT’

Fill up the PAN Card Change Request Form with your Personal Details.

SELECT THE CHECKBOX IF IT ‘S DETAILS ARE DIFFERENT FROM THE PREVIOUS PAN CARD

Tick Mark the field which has to be corrected / changed in the new PAN Card.Note that Date of Birth and Address field are by default tick marked as they will be

automatically updated.

Select the Documents that will serve as your Proof of Identity, Proof of Date of Birth and Address Proof. Note that you can also select Proof of PAN.

SELECTTHE PROOF OF ID,DOB AND ADDRESS PROOF

Select the mode of payment for the government fees of Rs.105.After completion of the Form, Click on Submit.

CLICK ON‘SUBMIT’

CHOOSE THE MODE OF PAYMENT

Check the details of the form produced.

Enter the confirmation code and Click on Confirm

CLICK ON‘CONFIRM’

CLICK ON‘AGREE’

After the successful payment, an acknowledgement form will be produced. Save and take a print of that form.

Affix your photographs on the space provided.

AFFIX YOURPHOTOGRAPH

Sign the PAN acknowledgement form.

SIGN INBLANK INK ONLY

List of Documents to be send along with PAN Request acknowledgement form:

1. Copy of Address Proof that you mentioned in the application .

2. Copy of ID Proof that you mentioned in the application.

Courier the signed Acknowledgement form along with the documents to the below address:

NSDL e-Governance Infrastructure Limited, 5th floor, Mantri Sterling, Plot No. 341, Survey No. 997/8, Model Colony, Near Deep Bungalow Chowk, Pune - 411016.
