EFMP CASE LIAISONSWhat is a Case Liaison (CL)?A Case Liaison (CL) is a Fleet & Family Support Center staff member assigned to each sponsor/family who is enrolled in the EFMP.

What does a Case Liaison (CL) do?• Provides information and referral to resources within the San Diego area.• Offers assistance in developing an individual service plan.• Connects families with networking and social opportunities within the area.

EFMP COMMAND POCWhat is an EFMP Command POC?A Command POC is an active duty Service Member who serves in support of the EFMP at each command.

What does a Command POC do?• Monitors EFM enrollment status and expiration dates for their command.• Collaborates with EFMP case liaisons.• Assists with the enrollment, update and disenrollment processes.

MTF COORDINATORWhat is an MTF Coordinator?An MTF Coordinator is directly responsible for the enrollment and re-enrollment processes.

What does an MTF Coordinator do?• Reviews and verifies information submitted in the application for accuracy and uploads forms to the NFAAS system for processing.• Accepts application forms (DD 2792/DD2792-1) that have been hand delivered to their office for submission.

What is a Navy School Liaison Officer?The School Liaison Officer is responsible for assisting military families in addressing educational issues during times of transition and deployment.

How can the School Liaison Officer support EFMP families?• Offers assistance in navigating the administrative systems within local public schools.• Provides information about programs and services available on the installation and in the local community.• Makes referrals to the appropriate special education or medical professionals.• Provides information concerning local policies and regulations pertaining to special education.• Can assist families with the required signatures for EFMP enrollment form DD 2792-1.

*School Liaison Officers are not authorized to serve in an advocacy role, recommend educational intervention services, or attend special education meetings as counsel for military families.
