Filing an Appearance FormAttorney Instructions
Before you begin the ECF docket transaction . . .
• Complete the Appearance Form, 6ca‐68, found on the court’s website.
• Please make sure the contact information that you provide in the form “matches” the contact information in your PACER account.
• When the form is completed, save it to your computer in pdf format.
• Give the form a name that you will recognize; during the docketing event, you will be prompted to upload the form to the ECF filing system.
Select your completed Appearance Form by highlighting the file name.
