Central East LHIN Sub-Region Planning Tables Expression of Interest (EOI)
The Central East Local Health Integration Network (Central East LHIN) is seeking health system stakeholders patients,
caregivers, Indigenous representatives, Francophone representatives, New Immigrant representatives, Primary
Care (Physicians, Nurse Practitioners), Specialist Physicians, Hospital sector representatives, Public Health
Unit representatives, Municipal Services representatives, Home and Community Care services director
representatives, Community Support Services sector representatives, Long-Term Care Homes sector representatives
and Mental Health and Addictions sector representatives - to participate in Sub-region Planning Tables. Individuals
within the Central East LHIN’s geographic region are encouraged to complete and submit an Expression of Interest
for consideration as a member of the Central East LHIN Sub-Region Planning Tables.
Sub-region planning builds on the Central East LHIN’s strong foundation of local engagement and planning, including
Health Links, and will also advance the strategic aims and direct care priorities of the Central East LHIN’s Integrated
Health Service Plan 4 (IHSP 4) – Living Healthier at Home. To that end, the Central East LHIN has established
Sub-Region Planning Tables to foster joint accountability for innovative, integrated system redesign to address
health and service gaps, advance quality, and improve patient experience and outcomes

CCN In the News: CCN To Expand Its Meals on Wheels Program
Community Care Northumberland is pleased to announce that it will begin providing its Meals on Wheels program to residents in the Municipality of Port Hope effective September 1st, 2017. Currently provided by The Canadian Red Cross (CRC), the Meals on Wheels program promotes health and independence by providing quality and affordable meals to seniors living alone, adults with disabilities who are unable to shop for groceries or cook for themselves and those being discharged from hospital with limited/no help available during their recovery. As a result of strategic, organizational changes at the Red Cross in December 2016, the decision was made to transition the Meals on Wheels program in Port Hope to another provider in the community. As the funder of the program, the Central East LHIN worked with CRC and Community Care Northumberland on an integration plan to ensure the ongoing delivery of the Meals on Wheels program to existing and future clients in Port Hope. “Community Care Northumberland is excited for the opportunity to continue to deliver the Meals on Wheels program to residents in Port Hope and welcome the Red Cross volunteers to our team,” said Trish Baird, Executive Director, Community Care Northumberland. “We have a long history of delivering this program in other parts of Northumberland County and have seen the benefits of not only providing meals but of ensuring that residents have access to all of our other programs.” “On behalf of the Central East LHIN I would like to congratulate Community Care Northumberland and the Canadian Red Cross for collaborating on a voluntary integration that will ensure the ongoing delivery of this vital program to the residents of Port Hope,” said Deborah Hammons, Chief Executive Officer, Central East LHIN. “This integration supports the LHIN’s goal of advancing integrated systems of care so that local residents can live healthier at home.” QUICK FACTS The Local Health Services Integration Act, 2006
(LHSIA) S.24 provides that each LHIN and each health service provider (HSP) shall separately and in conjunction with each other identify opportunities to integrate the services of the local health system to provide appropriate, co-ordinated, effective and efficient services.
Established in 1988, Community Care Northumberland is a not-for-profit organization operating throughout Northumberland County with six local program offices. Qualified staff and volunteers provide a variety of important support programs to seniors and adults with disabilities, allowing them to remain independent within their own homes and communities.

Saying Goodbye to Community Care’s 2017 Summer Students
On August 25th we said goodbye to our summer students. For 12 weeks two of our summer students, Emma
Sheedy and Alex Macdonald, have been working out of the CCN offices doing work such as, aiding with programs,
general reception, and other office duties. CCN was able to hire these students thanks to the Canada Summer Jobs
Grant funded by Service Canada. Both students will be returning to post-secondary school in September.
(Pictured left) Emma, who has been working in the Cobourg, Port Hope, and
administration offices, will be returning to Algonquin College to begin her
second year in the Public Relations
program. “This has been an amazing
opportunity,” said Emma, “I’m so
grateful I got to spend my summer
working with great people who have all
taught me so much. I’m looking forward
to using what I have learned at
Community Care in the future.”
(Pictured right) Alex, who has been working in the Colborne and Brighton
Offices, will be returning to Trent University to continue her studies in the
Bachelor of Science program. “I learned a great deal working with
community care and it was fulfilling knowing that I was helping people. The
wonderful staff I was lucky enough to work with definitely made the
experience one I will always remember,” said Alex.
Along with the Canada Summer Jobs grant, CCN also received a
second grant from the Campbellford Seymour Foundation which
allowed them to hire a third summer student. Rachel Crowley
(pictured left) has been working in the Hastings, Warkworth and,
Campbellford offices. In September Rachel will be returning to
Queens University to study Nursing.

Recent Workshop in Campbellford: Marvelous Maple
Completely natural and naturally delicious, local maple syrup is one of the great harbingers of spring. And it's good
for more than just pancakes and waffles! In fact, participants learned how to have a meal with maple syrup in
each course. Dorothy Fletcher demonstrated several easy to make recipes including a salad, a main course and a
cake, all using maple syrup.
Participants learned how to express their own style
using acrylics with teacher and artist, Rachel Harbour.

Participants came out for a fun and a
relaxing class with Decorative Artist
Cheryl Lafferty as she guided them
through the steps to bring a Mason Jar to
life with a gorgeous design. Cheryl has
taught Acrylic painting for over 30 years
and is a Published Artist in National
Magazines.
