Root Cause; Lack of Leadership !
‘Safety Culture’
-
Who Creates Culture ?
“Leaders Create Culture
It is their responsibility to
change it.
Leaders are responsible for
establishing the conditions that
lead to their teams success or
failure”.
Columbia Accident Report
-
“Imagine it’s a busy day and you shrink San
Francisco Airport to only one short runway
Make planes take off and land at the same
time at half the present time interval
Rock the runway from side to side
Make sure the equipment is so close to the
edge of the envelope that it’s fragile
Turn off the radar to avoid detection
Impose strict controls on radios
Wet the whole thing down with salt water
and oil
Man it with 20-yr olds
Oh, and by the way, try not to kill anyone…”
Senior Officer Air Division
So why is culture important ?

of small efforts,
repeated day in and
day out”
Robert Collier

Safety Conversations
1. Start with a fact.
• e.g. We make decisions that can impact the
safety of others.
2. Explain the problem.
• e.g. loss of workforce morale, reputational
damage, repair costs, etc.
3. Provide a rational solution.
• e.g. I want every member of the team to try
doing some small things …
4. Emotional reason why this matters.
• e.g. My sister was injured at work many
years ago …
5. Explain what you want to change.
• e.g. If ever you feel uneasy about a task,
stop and ask. etc.
-

and ask others to follow,
unless you know how to
follow too”
Sam Rayburn

Errors of Perception
-
“A concern for safety which is
sincerely held and repeatedly
expressed but, nevertheless, is
not carried through into action,
is as much protection from
danger as no concern at all.”
Anthony Hidden QC
Leaders need to act to make
safety happen
Leadership
-
“Great leaders are almost
always great simplifiers,
they can cut through
argument, debate, and
doubt to offer a solution
everybody can
understand”

-
Do the Right Thing !
“Walk the talk”.
Be a role model and act as a fully engaged
and visible in safety activities.
If you don’t believe safety is
important, why will anyone else?
Every time you turn a “blind eye” or
pay “lip service” to safety, you are
sending messages that its ok for
your team to do likewise.
The lowest standard you walk past without
intervening is the highest standard your
team will achieve.
-
“Example is not the
main thing in
influencing others;
it is the only thing”
Albert Schweitzer
1875-1965
Nobel Peace Prize Winner
