DRIVING SCHOOL AFFIDAVIT

READ AND SIGN AFFIDAVIT BELOW IF ELECTING DRIVING SCHOOL

Commercial driver's license holders MAY NOT elect driving school.

Affidavit, certificate and payment are due w/n 90 calendar days of citation issuance.

I elect driving school. I swear or affirm that I have not made the election to attend a drivingimprovement course as provided by s.381.14 (9), Florida Statues, within the past l2 months and

no more than five times in my lifetime. I understand that the Clerk's Office will be notified bythe Florida Department of Highway Safety & Motor Vehicles if I am in violation of s. 381 .14(9),Florida Statutes. tf I do not pay the APPLICABLE FINE* PLUS A $7.00 AFFIDAVIT FEEand provide proof of completion from the driving course to your office within 90 calendar days

from the date my citation was issued, the following will occur:

1. I WIL FORFEIT ONE (1) OF MY FIVE (s) DRTVTNG SCHOOL ELECTTONS FORMY LIFETIME AND ALL MONIES PAID.

2. MY DRIVER'S LICENSE WILL BE SUSPENDED WITHOUT ANYFURTHER NOTICE.

3. I WILL BE REQUIRED TO PAY ADDITIONAL FINES AND PENALTIESINCLUDTNG AN $I8.OO PROCESSING FEE AND ANY DELINQUENCY FEE.POINTS WILL BE APPLIED TO MY DRIVER'S LICENSE.
