Community Service Navigators

• Goal- to connect citizens with health & human services that will increase access to opportunities & choices that promote health, safety & wellbeing

• Target population- citizens with multiple or complex needs who are struggling to get by & have significant barriers to accessing health or social services to address these needs

Enhancing Choices for Our Citizens

• Safety, health & wellbeing start within our families, schools & neighborhoods

• Many citizens can’t afford medical care or have difficulty locating or getting to health or social service providers

• Everyone should have the opportunity to make choices that lead to health, safety & wellbeing

Enhancing Choices for Our Citizens

• Social factors like income, education & ethnicity mean some people are more vulnerable to poor safety & health outcomes

• Rural residents have fewer options for services and/or challenges in identifying & accessing resources
