National Programme - securing CCG progress on undertaking assessments for around 60,000 Previously Unassessed Periods of Care cases this has led to:

• NHS CHC now part of assurance programme for CCGs - reflecting the importance of NHS long term care for vulnerable individuals

The memory café, provides a meeting place for carers, and an opportunity for people with dementia to meet and take part in activities and quizzes to keep their mind active.

Children were invited by NHS North Derbyshire CCG to share experiences Children Take Over Day

• Waiting areas that were suited to young people. • More opportunities to see the school nurse in school time.• Doctor’s appointments out of school hours and first aid training for students.

17,541 children with child protection plans, looked after children and unborn children with pre-birth pans have been uploaded onto CP-IS and the number is rising each week.

CP-IS improves protection to vulnerable children, quality, efficiency and security of information sharing.
