Leak in crossover flange• HiPo for injury if the high-pressure flange had popped off and

hit personnel within the limited high pressure area

• Job schedule changed several times:- Initially planned equipment was assigned to other operations and was not available

• Once job was called off, insufficient time was given to- Sub-contractor to prepare rented equipment

• Misconceived sense of urgency in communication from Asset/ResDev
