Dear   Parents/Guardians, 

We   are   pleased   to   announce   that   your   child   will   be   receiving   a    FREE   t­shirt    in   navy   with   a   white   Jockvale Elementary   logo   on   the   front   compliments   of   the   Jockvale   Elementary   School   Council.   The   purpose   of   the   t­shirts is   to   help   promote   a   sense   of   community   among   the   students,   staff,   and   families   at   Jockvale.   All   students   are encouraged   to   wear   their   t­shirts   on   Friday   Spirit   Days   and   at   school   events.  

To   receive   your   child’s   t­shirt   simply   complete   the   form   below   by    November   30 th     and   return   it   to   the   school.   Please complete   a   seperate   form   for   each   of   your   children   attending   Jockvale.   We   hope   to   deliver   the   t­shirts   before   the holiday   break.  

Available   in   Youth   (2   ­   20)   and   Adult   Sizes   (S   ­   XL).   For   sizing   information   please   see   the   back   of   this   form.
